Neuroplacental Interactions in early human development: Insights from selective acute neuronal necrosis.

INTRODUCTION: The correct development of the central nervous system depends largely on the last two trimesters of gestation and the early neonatal period. Neurological lesions are common in autopsies, being one of the most relevant, selective acute neuronal necrosis, a manifestation of hypoxic-ischemic encephalopathy. This study aimed to analyse the distribution of selective acute neuronal necrosis (SANN) and its association with placental lesions, clinical factors and other central nervous system (CNS) abnormalities. METHODS: From 1090 autopsies performed, 205 cases (18.8 %) with SANN were i

Advances in Fetal Repair of Spina Bifida Integrating Prenatal Surgery, Stem Cells, and Biomaterials.

Spina bifida (SB) is a congenital malformation of the central nervous system (CNS), resulting from incomplete closure of the neural tube (NT) during early embryogenesis. Myelomeningocele (MMC), the most severe form of SB, leads to progressive neurological, orthopedic, and urological dysfunctions due to both NT developmental failure and secondary intrauterine injury ("two-hit hypothesis"). Prenatal repair of MMC has progressed considerably since the Management of Myelomeningocele Study (MOMS, 2011) trial, which showed that open fetal surgery can decrease the need for shunting and improve motor

Dynamics of the Mammalian Placental Metabolome in Placentogenesis and Embryonic Development.

Mammalian placental metabolism is crucial for both placental and embryonic development. However, the metabolic profiles of placentas and their regulatory roles in placentogenesis and embryonic development at different developmental stages remain poorly characterized. To address these questions, we collect 501 mouse placentas spanning embryonic day (E) 8.5-14.5 and construct metabolomic-transcriptomic atlases of placentogenesis. Metabolomic and transcriptomic analyses reveal that placental samples from E8.5 to E14.5 are clustered into three separated states: E8.5, E9.5-10.5, and E11.5-14.5, pin

From light to sound: Seeing and hearing the placenta in health and disease.

The placenta is a pregnancy-specific organ, functioning as the maternal-fetal interface and mediating essential processes including exchange, protection, and endocrine regulation. Placental abnormalities contribute to the pathophysiology, onset, progression, and prognosis of major perinatal disorders, making imaging modalities that enable their detection and monitoring crucial. Ultrasound (US) imaging is the principal modality for placental imaging, providing structural assessment with B-mode and blood flow evaluation with Doppler mode. Furthermore, advanced techniques such as 3D power Doppler

Hemodynamics and transplacental oxygen exchange during intact cord resuscitation in control and congenital diaphragmatic hernia lambs.

Evolution of gas exchange during intact cord resuscitation (ICR) remains unexplored. We aimed to investigate changes in placental hemodynamics and gas exchange during ICR in lambs with and without congenital diaphragmatic hernia (CDH). Eight CDH lambs (4.2 kg, 4 females) and nine controls (3.9 kg, 3 females) underwent ICR. At 80 days gestation (term = 142 days), CDH was induced by diaphragmatic incision. After c-section at term, mean arterial pressure (MAP) was monitored via an aortic catheter. Umbilico-placental blood flow (Qup) was assessed with a flow transducer on the umbilical artery. Umb

Perinatal hyperandrogenization and immune activation in rodents model subtypes of autism.

Although increased maternal androgens, such as those in polycystic ovary syndrome (PCOS), are associated with a higher incidence of autism spectrum disorder (ASD) in offspring, a causal link has yet to be established. We assessed whether perinatal hyperandrogenization in a murine model recapitulates core ASD traits and compared this model to the maternal immune activation (MIA) model of ASD. Both models produced ASD-like phenotypes, yet they exhibited distinct behavioral subtypes and neurodevelopmental trajectories. Hyperandrogenized offspring showed greater reductions in social communication

High sensitivity ultrasound microvessel imaging for the assessment of placental health.

Placental insufficiency underlies common and clinically challenging pregnancy complications including fetal growth restriction, preeclampsia, and preterm birth. Although these disorders share impaired placental function, current clinical tools cannot reliably detect early or subtle vascular abnormalities while pregnancy is ongoing. Standard obstetric ultrasound lacks the sensitivity needed to visualize small vessel development; however, high sensitivity microvessel imaging has emerged as a promising tool that overcomes many limitations of conventional Doppler ultrasound. By leveraging high fra

Outcome after Selective early treatment for Closure of patent ductus ARteriosus in preterm babies, a multicentre, masked, randomised placebo-controlled parallel group trial (Baby-OSCAR trial).

BACKGROUND: In extremely preterm babies, born before 28 weeks' gestation, a large (≥ 1.5 mm in diameter) patent ductus arteriosus present beyond 3 days of age is associated with higher mortality and morbidity than infants without a patent ductus arteriosus. The cyclooxygenase inhibitor ibuprofen may be used to treat patent ductus arteriosus. Whether selective early treatment of a large patent ductus arteriosus with ibuprofen improves health and developmental outcomes is not known. METHODS: We conducted a multicentre, randomised, double-blind, placebo-controlled trial evaluating early treatmen

Maternal-fetal immune conflict contributes to male-specific impairments in a mouse model of neurodevelopmental disorders.

Autism spectrum disorder (ASD) arises through a combination of genetic and environmental risk factors. One environmental risk factor is maternal immune activation (MIA), wherein pathogenic infections in pregnant individuals increase the risk of ASD in the offspring, an effect seen most prevalently in males. However, the reasons that males exhibit this heightened sensitivity to MIA, and the mechanisms through which inflammatory signals traverse the maternal-fetal interface to impact the developing male embryo, remain largely mysterious. Here, we harness the poly(I:C) mouse model of neurodevelop

Recurrent Diffuse Chorioamniotic Hemosiderosis of the Placenta in a Mother With Congenital Heart Disease.

A woman in her mid-20s with repaired congenital heart disease (CHD) delivered a 33-week neonate in 2024 following preterm labor and a 24-week neonate in 2025 for non-reassuring fetal heart tones. Placental examination in both cases revealed grossly abnormal, red-brown discolored membranes with a circumvallate insertion. Histologic evaluation demonstrated diffuse chorioamniotic hemosiderosis (DCH) in both cases. This report describes a novel case of recurrent DCH in a mother with CHD and proposes that Fontan physiology may contribute to impaired uteroplacental perfusion, increasing the risk for

The Association of Fetal Cyanotic Versus Non-Cyanotic Cardiac Malformations and Placental Vascular Malperfusion.

OBJECTIVE: Placental abnormalities are frequently observed in congenital heart disease (CHD) pregnancies, yet detailed histopathological evaluations are limited. This study investigated placental vascular lesions in CHD cases, with comparisons between cyanotic and non-cyanotic subtypes. METHODS: Term singleton pregnancies with prenatally diagnosed moderate-to-severe CHD were compared to gestational age-matched controls. Placentas were assessed using standardized histopathological criteria, and CHD cases were stratified based on cyanosis. RESULTS: Among 60 placentas analyzed (40 CHD and 20 co

Maternal gut microbiota mediates prenatal stress-induced fetal blood‒brain barrier dysfunction.

Maternal prenatal stress confers elevated neuropsychiatric risk to offspring, yet the mechanisms underlying fetal neurodevelopmental impairment remain elusive. The gut microbiota has emerged as a key regulator of brain development and behavior. However, the mechanisms mediating the interactions between the microbiota and the developing brain are still poorly understood. Here, utilizing a prenatal stress mouse model integrated with multi-omics approaches, comprehensive behavioral assays, and molecular validations, we demonstrate that prenatal stress not only induces maternal gut microbiota dysb

Endothelial microparticles in high glucose environment: molecular pathways of GDM-associated placental dysfunction and coenzyme Q10-based targeted therapy.

Gestational diabetes mellitus (GDM) is frequently linked to placental overgrowth and hypertrophy, while maternal hyperglycaemic environment consistently induces systemic vascular endothelial cell injury. Damaged endothelial cells stimulate the release of endothelial microparticles (EMPs), but their impact on placental function has not been fully characterized. In this study, we clinically observed that the levels of EMPs in the peripheral blood of GDM patients are significantly higher than those in healthy pregnant women. Furthermore, we identified that EMPs from GDM patients promote excessive

Discrepancy in menstrual and ultrasound-based gestational age is associated with chromosomal aberrations and adverse pregnancy outcomes-Results from a nationwide cohort study.

INTRODUCTION: In women with a regular cycle, reliable last menstrual period (LMP), and accurate crown-rump length (CRL) estimation, discrepancies in estimated date of delivery (EDD) may indicate impaired fetal growth and increased risk of morbidity. This study examines whether discrepancies between EDD based on LMP (EDDLMP) and CRL (EDDCRL) are associated with chromosomal aberrations, adverse pregnancy outcomes, and obstetric complications. MATERIAL AND METHODS: A Danish nationwide register-based cohort study including all singleton pregnancies with both CRL and LMP registered between 2008 an

New approach to document cannabis toxicity during pregnancy: a low abundance molecular species detection in long-term formalin-fixed paraffin embedded fetal tissues. A technological report.

BACKGROUND AND AIMS: Cannabis is a widely abuse substance during pregnancy. However, reports of effects on human fetal brain development and implication in brain malformations are scarce in the literature. This study aims to determine the usability of long-term formalin-fixed tissues, highly degraded as a result of long post-mortem delays, collected during post-mortem medical examinations of human fetuses for molecular identification of low-abundance molecular species, namely the cannabinoid receptors, as a prerequisite for studying cannabis role in human fetal brain malformations. Placental pathology in gestational diabetes mellitus: Associations with neonatal outcomes and postpartum dysglycemia.

INTRODUCTION: Gestational diabetes mellitus (GDM) is associated with adverse outcomes in both pregnancy and the postpartum period. Although placental pathology has been extensively studied in relation to neonatal complications, its relevance to maternal health after delivery remains less well defined. This study aimed to compare gross and histologic placental features between GDM and normoglycemic pregnancies according to the Amsterdam Placental Workshop Group Consensus Statement. It also evaluated associations between specific placental lesions and both neonatal outcomes and maternal dysglyce

Maternal Poly (I:C)-Induced Placental Inflammation and Endocrine Dysfunction Are Associated with Disrupted Corticogenesis in Mouse Offspring.

BACKGROUND/OBJECTIVES: Maternal immune activation (MIA) increases the risk of Autism Spectrum Disorders (ASD). Experimental models demonstrate that maternal exposure to bacterial endotoxin or the viral mimic polyinosinic:polycytidylic acid [poly (I:C)] reliably recapitulates ASD-like behavioral abnormalities in offspring, yet the underlying neurobiological mechanisms linking MIA to altered neurodevelopment remain incompletely understood. Increasing evidence highlights the placenta as a critical mediator in shaping fetal brain development through immunological and hormonal regulation. Likewise,
