Impact of Maternal Risk Stratification on Clinical Characteristics and Perinatal Outcomes in Singleton Pregnancies With Preeclampsia.

This retrospective study of 4143 women at Fujian Maternal and Child Health Hospital (2012-2022) investigated how maternal risk levels influence clinical characteristics and perinatal outcomes in singleton pregnancies complicated by preeclampsia. Utilizing class-imbalance-adjusted and random forest algorithms, participants were stratified into high-risk (17.38%), moderate-risk (34.08%), and low-risk (48.54%) groups. The analysis revealed that women in the high-risk category were diagnosed at a significantly earlier gestational age and faced higher rates of severe, early-onset, and preterm preec

Fetomaternal hemorrhage in monochorionic diamniotic twins at 37+ weeks' gestation: A rare complication.

Fetomaternal hemorrhage refers to a clinical syndrome in which fetal blood enters the maternal circulation through the placental villous spaces, typically triggered by specific stimuli, and may lead to fetal anemia or maternal hemolysis. The occurrence of fetomaternal hemorrhage is very rare in singleton pregnancies and even rarer in multiple gestations. In monochorionic diamniotic twin pregnancies, shared placental circulation can amplify the consequences of fetomaternal hemorrhage, potentially resulting in more severe outcomes than in singleton pregnancies or in twins with double chorions an

Fetal-Onset Autoimmune Hemolytic Anemia and Thrombocytopenia Followed by Giant Cell Hepatitis: A Case Report.

INTRODUCTION: While neonates may suffer from alloimmune or secondary autoimmune diseases caused by placental transfer of maternal antibodies, primary neonatal autoimmune disease is extremely rare. CASE PRESENTATION: We present a newborn girl with fetal-onset autoimmune disease, featuring prenatal hydrothorax, progressive Coombs-positive autoimmune hemolytic anemia, and autoimmune thrombocytopenia secondary to autoantibodies against glycoprotein IIb/IIIa, which were not present in the mother. Red blood cells and platelets were repeatedly transfused during the first 5 weeks after birth. At 6 mo

Concurrent Eclampsia, HELLP (Hemolysis, Elevated Liver Enzymes, and Low Platelets) Syndrome and Peripartum Cardiomyopathy in a Primigravida: A Case Report.

Hypertensive disorders of pregnancy can rapidly progress to multi-organ failure. Eclampsia and HELLP (hemolysis, elevated liver enzymes, and low platelet count) syndrome are obstetric emergencies, while peripartum cardiomyopathy (PPCM) is a rare but potentially fatal cause of heart failure in late pregnancy and the postpartum period. We report the case of a critically ill 32-year-old primigravida at 35 weeks' gestation with treated hypothyroidism who presented with generalized tonic-clonic seizures, severe hypertension, and pulmonary edema. Laboratory evaluation demonstrated thrombocytopenia a

Agreement between Mid-pregnancy Serum Placental Growth Factor Levels and Abnormalities in Uterine Doppler Ultrasound Findings.

This prospective observational study evaluated the concordance between mid-pregnancy serum placental growth factor (PlGF) levels and abnormal uterine artery Doppler findings in pregnancies at risk of placental insufficiency. Among 127 patients, PlGF testing was discordant with uterine artery Doppler findings in 17% of cases with minimal agreement (κ 0.07; 95% CI -0.13 to 0.28), and only 2 of 10 patients with abnormal uterine artery Doppler had low PlGF. These findings suggest that PlGF and uterine artery Doppler identify different at-risk populations and should not be used interchangeably in p

Single-cell-based non-invasive screening for fetal pathogenic microimbalances using maternal blood: comparison with invasive prenatal diagnosis.

OBJECTIVE: Pathogenic or likely pathogenic copy-number variants (p/lpCNVs) are a significant cause of perinatal morbidity and mortality. Current prenatal screening based on cell-free DNA (cfDNA) fails to detect the majority of microimbalances (microdeletions/microduplications), leaving a significant residual risk of undetected chromosomal abnormalities. This study evaluated the clinical performance of a novel single-cell-sequencing-based non-invasive prenatal testing (scsbNIPT) method utilizing circulating extravillous trophoblasts (cEVTs) for the detection of fetal p/lpCNVs, particularly micr

Pathologic abnormalities of deep placentation in the great obstetrical syndromes: Implications for understanding the pathophysiology, risk assessment in early pregnancy, and personalized prevention.

The concept of the Great Obstetrical Syndromes was introduced to explain the unique nature of obstetrical disease, which differs fundamentally from disorders in other areas of medicine. These syndromes, including preeclampsia, fetal growth restriction, fetal death, and spontaneous preterm birth, represent clinical endpoints rather than single diseases and share defining characteristics: they arise from multiple etiologies, have a prolonged subclinical phase, involve the fetus as an active participant, are adaptive in nature, and result from complex genetic and environmental interactions betwee

Velamentous Cord Insertion With Vaginal Delivery: A Case Report and Review of the Literature.

Velamentous cord insertion (VCI) is a rare placental abnormality in which the umbilical cord enters the chorionic membranes instead of the placenta, leaving the umbilical vessels unprotected. This pathological condition carries an increased risk of adverse perinatal outcomes, such as fetal growth restriction, preterm delivery, abnormal fetal heart rate patterns during labor, and perinatal morbidity, which often lead healthcare professionals to choose cesarean section to reduce postnatal risks. Early prenatal diagnosis using color Doppler imaging and comprehensive ultrasound is of utmost import

Maternal cardiovascular, endothelial and inflammatory biomarker dysregulation in twin-twin transfusion syndrome: insights into shared mechanisms with pre-eclampsia.

BACKGROUND: While pre-eclampsia incidence is increased in twin pregnancy, this risk reduces following fetoscopic laser ablation (FLA) for twin-twin transfusion syndrome (TTTS). We evaluated maternal cardiovascular, inflammatory and angiogenic biomarkers in TTTS compared with gestation and maternal characteristic matched uncomplicated monochorionic diamniotic (MCDA) twin pregnancies and examined biomarker changes following laser ablation. METHODS: Maternal serum biomarkers assessed before and after FLA included heart-type fatty acid-binding protein (H-FABP), midkine, soluble tumour necrosis fa

Perinatal outcomes and challenges in higher-order multiple gestations: insights from a retrospective analysis at a tertiary care centre.

BACKGROUND: Higher-order multiple gestations (HOM), defined as pregnancies with three or more fetuses, have increased due to assisted reproductive technologies (ART). However, they are associated with significant maternal and fetal risks, including preterm birth and complications from monochorionic placentation. OBJECTIVE: To evaluate maternal and neonatal outcomes in HOM pregnancies managed at a tertiary care center in the UAE, assess the impact of chorionicity, and examine the role of ART in these high-risk pregnancies. METHODS: A retrospective study was conducted on 102 cases of HOM pregn

Analysis of metabolites in serum and villous tissue of missed abortion patients based on targeted metabolomics.

OBJECTIVE: In recent years, advancements in metabolomics have highlighted that changes in metabolic states within the body might also be closely related to missed abortion. However, the specific impacts of metabolic abnormalities on missed abortion remain unclear. This study screened differential metabolites in serum and villous tissues from patients with missed abortion, analyzed possible mechanisms linking metabolic disturbances to disease occurrence, and explored their clinical research value in early pregnancy abnormality research. METHODS: Focusing on the effect of abnormal levels of met

Obstetrical, perinatal, and children's health outcomes following fresh embryo transfer after extended embryo culture.

STUDY QUESTION: Does extended embryo culture (EEC) associate with an increased risk of obstetrical, perinatal, or children's health complications? SUMMARY ANSWER: After thorough adjustment, EEC was not associated with widespread increased risks, although a moderate excess risk persisted for a few specific outcomes, notably cardiac anomalies, whereas reduced risks were observed for gestational diabetes, small birthweight, and musculoskeletal-limb anomalies. WHAT IS KNOWN ALREADY: EEC is increasingly used in IVF cycles. While blastocyst transfer (day-5/6) often improves birth rates, concerns r

Serological Screening of Coxiella burnetii Phase II IgG Antibodies for the First Trimester of Ante-natal Women Using ELISA and Immunofluorescent Assay (IFA).

OBJECTIVES: Both liver fibrosis and osteoporosis share inflammatory pathways, with liver fibrosis potentially contributing to decreased bone mineral density (BMD). The rising prevalence of non-alcoholic fatty liver disease (NAFLD) and associated liver fibrosis, especially in older populations, may increase the risk of osteoporosis, but evidence remains inconclusive. This study aims to investigate the relationship between liver fibrosis and osteoporosis in individuals over 50 years old. METHODS: This cross-sectional study used data from the Bushehr Elderly Health Program (BEHP), a cohort of 2,
