Development of a robust method to derive human trophoblast stem cells from late-gestation placentas and its application to preeclampsia.

Trophoblasts are multifunctional cells in the placenta and essential for normal pregnancy. Although trophoblast dysfunction can cause pregnancy complications, the underlying mechanisms remain unclear, and effective treatments are limited, partly because of the scarcity of appropriate experimental models. We previously reported the derivation of human trophoblast stem cells (hTSCs) from 1st-trimester placentas and blastocysts, providing a powerful tool to investigate human trophoblast development and function. However, the difficulty in deriving hTSCs from late-gestation placentas has limited t

Placental Evidence of Intrauterine Stress: Mechanisms, Timing, and Links to Perinatal Brain Injury.

Placental examination is frequently performed following adverse fetal or neonatal neurologic outcomes to elucidate the cause or timing of injury. In practice, however, the placenta more often serves as a biologic archive of fetal stress, adaptation, and reduced physiologic reserve rather than a definitive record of injury. This review synthesizes current research linking placental pathology to perinatal brain injury, with emphasis on biological mechanisms and temporal interpretation. We examine how placental findings inform assessments of intrauterine stress, impaired placental or fetal perfus

Fetoplacental microcirculation abnormalities are reversed by aerobic exercise in mice with preeclampsia.

Preeclampsia (PE) is a hypertensive disorder of pregnancy characterized by hypertension and organ dysfunction, and is associated with impaired placental perfusion and systemic endothelial dysfunction. Aerobic exercise has been proposed as a potential nonpharmacological strategy to improve vascular function during pregnancy; however, its effects on fetoplacental microcirculation in PE remain poorly understood. Therefore, the objective of this study was to investigate whether maternal aerobic exercise during gestation could influence fetoplacental perfusion and maternal pathological features in

Naringenin protects against L-NAME-induced preeclampsia and fetal developmental abnormalities via modulation of inflammation, eNOS/NO signaling, AKT/mTOR pathway, and restoring endothelial function.

Preeclampsia considered as multi-organ disorder involving systemic inflammation and endothelial dysfunction. Our work was designed to investigate the protective role of Naringenin (NGN) against Nω-nitro-l-arginine methyl ester (L-NAME)-induced placental damage and congenital abnormalities in a rat model of preeclampsia. Forty-five adult female albino rats (weighing 160-180 g) were randomly allocated into nine groups (n = 6 per group) and treated for a duration of 18 days. Group 1 served as the untreated control, while Group 2 received 10% DMSO as a vehicle control. Group 3 was administered L-N

Mechanism of Shoutai Wan against recurrent spontaneous abortion: regulation of decidual vascular remodeling via ERβ-ANGPT2 signaling axis.

Shoutai Wan (STW), a classic traditional Chinese medicine formula used to tonify the kidney and prevent miscarriage, has been widely applied in the clinical management of recurrent spontaneous abortion (RSA). Increasing clinical evidence supports its efficacy in reducing miscarriage rates and improving pregnancy outcomes. However, the molecular basis by which STW alleviates defective decidual vascular remodeling in unexplained RSA remains insufficiently understood. Clinically, decidual ERβ and ANGPT2 expression, as well as serum estradiol, ANGPT2 and VEGFA levels were significantly decreased i

Preliminary embryo-fetal developmental toxicity study of pritelivir in CD1-IGS mice.

Pritelivir is a novel anti-herpes simplex virus drug that is currently in clinical trials in nonpregnant immunocompromised subjects. Here we conducted a preliminary embryo-fetal developmental toxicity study of pritelivir in CD1-IGS mice. Pritelivir was administered by oral gavage to time-mated mice at dose levels of 0, 10, 60 and 200 mg/kg/day from gestational day 6 through 15. Maternal food consumption, body weight and clinical observations were monitored throughout gestation. The mice were sacrificed at gestational day 18 and assessed based on embryo-fetal development and viability. Fetuses

Nanomaterial-Based Strategies to Modulate Placental Dysfunction in Preeclampsia.

Preeclampsia is a pregnancy-specific disorder characterized by hypertension and multi-organ dysfunction, originating primarily from placental abnormalities rather than maternal pathology alone. Impaired trophoblast invasion, defective spiral artery remodeling, angiogenic imbalance, oxidative stress, and sterile inflammation collectively drive placental dysfunction and subsequent systemic endothelial injury. Current clinical management remains largely supportive, highlighting the urgent need for mechanism-based therapeutic strategies that directly target the placenta while minimizing fetal risk

The role of placental anastomoses in severe brain injury after fetal demise in monochorionic twins: a multicenter retrospective study.

BACKGROUND: Monochorionic twins share a single placenta with vascular anastomoses including arterio-arterial and veno-venous anastomoses, which are low-resistance, superficial, and bidirectional anastomoses. Following single fetal demise, the survivor is at risk of brain injury due to acute exsanguination through arterio-arterial and veno-venous anastomoses into the low-pressure circulation of the deceased co-twin. OBJECTIVE: To assess whether the number and diameter of arterio-arterial and veno-venous anastomoses are associated with severe brain injury in the survivor following single fetal

Cord insertion anomalies and adverse pregnancy outcomes: more than an incidental finding.

OBJECTIVE: The aim of this study was to investigate the importance of routine evaluation of cord insertion during the prenatal period. To this end, the perinatal outcomes of marginal and velamentous cord insertions were compared to normal cord insertions. METHODS: We conducted a retrospective cohort study of singleton pregnancies delivered after 24 weeks between 2016 and 2023, comparing marginal and velamentous cord insertions diagnosed on second-trimester ultrasound with normal cord insertions in terms of preterm birth and adverse perinatal outcomes, after excluding cases with fetal anomalie

Roles and applications of artificial intelligence in fetal and placental MRI: a literature review.

BACKGROUND: Artificial intelligence (AI) has been increasingly integrated with fetal and placental magnetic resonance imaging (MRI) to enhance the detection of abnormalities and streamline diagnostic processes. MRI, known for its superior soft tissue contrast and multiplanar imaging capabilities, is a critical tool for evaluating complex fetal and placental conditions. However, the large volume of MRI data often poses challenges for clinicians in providing timely and accurate diagnoses. Fetal and placental MRI are subject to several inherent limitations, including fetal motion artifacts, low s

Inflammation Exacerbates Congenital Zika Virus Infection and Naringenin Provides Protective Effects.

Zika virus (ZIKV) infection during pregnancy is a critical driver of Congenital Zika Syndrome (CZS), yet the mechanisms of pathogenesis at the placental barrier remain incompletely understood. This article is a translational, observational, and experimental study combining clinical placental analyses, placental explants cultures and in vivo murine model to investigate the mechanisms involved in ZIKV infection. We evaluate the histopathological analyses to verify presence of inflammation in ZIKV-infected human placentas from newborns with CZS and without CZS (N-CZS), identifying more intense Ho

Mind the Litter: The Critical Role of Intralitter Variables in Reproductive and Developmental Toxicology Research.

BACKGROUND: Animal models remain essential for understanding developmental toxicology, providing insights into how in utero exposures affect fetal and placental outcomes. Litter-bearing species are widely used due to their efficiency and reproducibility, but conventional approaches often summarize outcomes at the litter level, masking meaningful within-litter variability. AIM: This review highlights three primary sources of intralitter variability: litter size, uterine implantation location, and fetal sex, and their influence on fetal and placental growth, mortality, and malformations. Placental growth factor (PLGF)-based testing to help diagnose suspected pre-eclampsia: a systematic review and economic evaluation.

BACKGROUND: Predicting a diagnosis of pre-eclampsia is based on a combination of clinical assessment of blood pressure, presence of protein in the urine, symptoms and laboratory test abnormalities. Accurately detecting pre-eclampsia is important to avoid false-positive diagnoses which could lead to unnecessary antenatal admissions and/or preterm delivery. Four blood tests that measure the biomarkers of placental growth factor or the ratio of soluble fms-like tyrosine kinase-1 to placental growth factor are available (known as Triage, Elecsys, DELFIA Xpress, and BRAHMS Kryptor tests). Prioritising parameters to minimise blood transfusions for postpartum haemorrhage: A machine learning study using national population data.

BACKGROUND: Postpartum haemorrhage (PPH) accounts for approximately 27% of maternal deaths worldwide and often requires blood transfusion, along with its associated risks. In resource-rich settings like South Korea, transfusion disparities persist, highlighting the need for predictive tools to optimise patient blood management (PBM). OBJECTIVE: To apply machine learning (ML) to rank predictors of transfusion in PPH using national claims data. METHODS: This was a retrospective cohort study of 123 763 women with PPH (2017-2019) from the National Health Insurance Service (NHIS) database (97% po

Atypical eclampsia without severe hypertension - the role of ophthalmic artery Doppler and sFlt-1/PlGF ratio: a case report.

Eclampsia remains one of the most severe obstetric emergencies associated with substantial maternal and perinatal morbidity and mortality. Although classically regarded as a hypertensive complication of pre-eclampsia, atypical presentations without sustained hypertension or severe clinical features may occur, complicating risk prediction and timely intervention. We report a case of atypical eclampsia in a 21-year-old woman hospitalized with a diagnosis of pre-eclampsia without severe features. Despite predominantly normal blood pressure values and initially stable laboratory findings, the sFlt

[In utero treatment of twin-to-twin transfusion syndrome].

Twin-to-twin transfusion syndrome (TTTS) is a serious complication of monochorionic twin pregnancies in which an imbalance in blood flow between the twins is caused by vascular anastomoses in the placenta. TTTS affects 10 to 15% of these monochorionic pregnancies and can lead to high perinatal mortality if left untreated. The Quintero classification, introduced in 1999, helps stratify the severity of TTTS into five stages, thereby facilitating clinical decision-making. Laser photocoagulation of placental anastomoses, a technique developped in the 2000s, has become the standard of care for seve

Prolonged neonatal phosphate retention and transient hypercalcemia following antenatal Burosumab exposure: a pharmacovigilance alert.

Burosumab is a monoclonal antibody targeting fibroblast growth factor 23 (FGF23) and is approved for the treatment of X-linked hypophosphatemia. Its use during pregnancy has not been studied, and fetal exposure may affect neonatal mineral metabolism. We report a neonate exposed to Burosumab throughout gestation who developed prolonged phosphate retention, suppressed parathyroid hormone, and transient hypercalcemia after birth. Serial biochemical monitoring demonstrated persistently elevated tubular phosphate reabsorption and low urinary phosphate excretion beyond the early neonatal period, con

Immunometabolic Dysregulation in Preeclampsia: Emerging Roles of Inflammation, Insulin Resistance, Uric Acid, and the Gut Microbiome.

Preeclampsia is a major cause of maternal and perinatal morbidity around the world. It is increasingly recognized as a disorder of systemic immunometabolic dysregulation rather than isolated placental dysfunction. Increasing evidence links chronic inflammation, insulin resistance, and changes in uric acid metabolism to the initiation and progression of preeclampsia. In addition, emerging evidence indicates that maternal gut dysbiosis is an upstream regulator of systemic immune and metabolic dysfunction via the gut-systemic-decidual axis. This review synthesizes current mechanistic, clinical, a
