Rare vasculitis types and obstetric and neonatal outcomes - A population-based study.

OBJECTIVE: Vasculitis is an infrequent pathology among reproductive-aged women. While data exists regarding pregnancy outcomes in the more common vasculitis subtypes, data is limited regarding these outcomes in rare vasculitis subtypes. We aimed to compare pregnancy and perinatal outcomes between women who suffered from rare types of vasculitis and those who did not. METHODS: This was a retrospective cohort study using the Healthcare Cost and Utilization Project- Nationwide Inpatient Sample (HCUP-NIS). All pregnant women who delivered or had a maternal death in the US (2004-2014) with an ICD-

Ultrasound-guided high-intensity focused ultrasound is technically feasible and safe for occluding placental vessels in early-onset twin-twin transfusion syndrome.

BACKGROUND: Twin-twin transfusion syndrome is a complication unique to monochorionic pregnancy, where shared placental vascular connections can lead to a circulatory imbalance, resulting in preterm delivery and attendant in utero or neonatal mortality. The only disease-modifying treatment is fetoscopic laser ablation of blood vessels, which has attendant risks of invasive intrauterine therapy. OBJECTIVE: In this first-in-human technical feasibility and safety study, we investigated noninvasive, ultrasound-guided high-intensity focused ultrasound to occlude placental vascular targets as identi

Does the order of ablation of vein-to-vein placental connections during fetoscopic laser photocoagulation affect outcomes in twin-twin transfusion syndrome?

OBJECTIVE: To assess whether the sequence of laser occlusion of vein-to-vein placental anastomoses is associated with fetal survival after laser photocoagulation for twin-twin transfusion syndrome. STUDY DESIGN: This post hoc analysis used data from the Sequential Trial, a randomized controlled trial comparing sequential vs selective laser photocoagulation techniques. Within this trial, a nested randomization assigned patients with superficial anastomoses (ie, artery-to-artery or vein-to-vein) to ablation of these anastomoses either at the beginning or at the end of surgery. For this analysis

Twin-twin transfusion syndrome and related monochorionic disorders: historical perspectives, current controversies, and evidence-based opportunities.

Twin-twin transfusion syndrome remains a major cause of morbidity and mortality in monochorionic twin pregnancies, arising from placental vascular anastomoses and representing a frequent indication for referral to fetal therapy centers. Despite advances in ultrasound assessment and fetoscopic laser therapy, variability in diagnostic thresholds, Doppler interpretation, and recognition of overlapping phenotypes-particularly twin anemia-polycythemia sequence and selective fetal growth restriction-continues to complicate clinical decision-making and limits meaningful comparison of outcomes across

The role of placental anastomoses in severe brain injury after fetal demise in monochorionic twins: a multicenter retrospective study.

BACKGROUND: Monochorionic twins share a single placenta with vascular anastomoses including arterio-arterial and veno-venous anastomoses, which are low-resistance, superficial, and bidirectional anastomoses. Following single fetal demise, the survivor is at risk of brain injury due to acute exsanguination through arterio-arterial and veno-venous anastomoses into the low-pressure circulation of the deceased co-twin. OBJECTIVE: To assess whether the number and diameter of arterio-arterial and veno-venous anastomoses are associated with severe brain injury in the survivor following single fetal

[In utero treatment of twin-to-twin transfusion syndrome].

Twin-to-twin transfusion syndrome (TTTS) is a serious complication of monochorionic twin pregnancies in which an imbalance in blood flow between the twins is caused by vascular anastomoses in the placenta. TTTS affects 10 to 15% of these monochorionic pregnancies and can lead to high perinatal mortality if left untreated. The Quintero classification, introduced in 1999, helps stratify the severity of TTTS into five stages, thereby facilitating clinical decision-making. Laser photocoagulation of placental anastomoses, a technique developped in the 2000s, has become the standard of care for seve

Treatment Outcomes after Fetoscopic Laser Ablation of Type II versus Type III Vasa Previa.

INTRODUCTION: Perinatal outcomes were compared for patients treated with fetoscopic laser ablation for vasa previa (VP) type II versus III. In type II VP, fetal vessels connect the main placenta to an accessory lobe, and in type III, these vessels exit and reenter a single-lobed placenta without velamentous cord insertion. Vessel ablation can be challenging in the presence of an accessory placental lobe. METHODS: This was a retrospective cohort study (2013-2025) of women with singleton pregnancies ≥31 gestational weeks who underwent laser ablation treatment for VP type II vs. III. RESULTS: O

Improvement in Twin Survival After Fetoscopic Laser Photocoagulation in Monochorionic Diamniotic Pregnancies with Twin-to-Twin Transfusion Syndrome: A Retrospective Cohort Study.

Twin-to-twin transfusion syndrome (TTTS) affects 10-15% of monochorionic diamniotic (MCDA) twin pregnancies and is associated with high perinatal mortality if left untreated. Selective Fetoscopic Laser Photocoagulation (SFLP) is the current standard of care for advanced-stage TTTS. This retrospective cohort study aimed to assess survival outcomes following SFLP and evaluate the learning curve associated with its implementation. It included 300 MCDA twin pregnancies with TTTS treated by SFLP from October 2018 to December 2024. Patients were chronologically divided into three equal groups (n = 1

Identifying an optimal inter-twin weight discordance cutoff for predicting donor survival after fetoscopic laser surgery for twin-twin transfusion syndrome.

BACKGROUND: Increased inter-twin growth discordance has been associated with adverse pregnancy outcomes. An inter-twin growth discordance of at least 25% is often considered as a clinical threshold for pathological discordance in unselected monochorionic-diamniotic (MCDA) twins, but an optimal cutoff for predicting survival outcomes following fetoscopic laser surgery (FLS) for twin-twin transfusion syndrome (TTTS) is unclear. OBJECTIVES: This study aimed to (1) compare outcomes of FLS for TTTS based upon on the presence or absence of discordance ≥25%, (2) evaluate the association of different

Toward an endothelium-centered framework for obstetric disseminated intravascular coagulation: Harmonizing pathophysiology, diagnosis, and treatment.

BACKGROUND: Obstetric disseminated intravascular coagulation (DIC) is a life-threatening coagulopathy triggered by placental abruption, amniotic fluid embolism, HELLP syndrome, and postpartum hemorrhage. Although rapid hemostasis and transfusion are central to management, growing evidence implicates endothelial injury, particularly endothelial glycocalyx (eGCX) degradation, in amplifying coagulopathy and organ dysfunction. This review integrates endothelial pathophysiology into modern diagnostic and therapeutic frameworks for obstetric DIC. METHODS: We conducted a narrative synthesis of studi

Twin-To-Twin Transfusion Syndrome in Dichorionic-Triamniotic Triplets Successfully Managed Without Fetoscopic Laser: A Management-Focused Case Report.

Triplet pregnancies are rare (~0.1% of births) and often complicated by prematurity. When one placental unit is monochorionic, there is a risk of twin-to-twin transfusion syndrome (TTTS), a severe vascular imbalance affecting monochorionic gestations. We report a 29-year-old primigravida with a dichorionic-triamniotic triplet pregnancy complicated by Quintero stage II TTTS affecting the monochorionic twin pair. The patient was managed with close surveillance, serial amnioreductions, and antenatal corticosteroids. At 32 + 6 weeks' gestation, three male infants were delivered by cesarean section

Selective occlusion of placental vessels using non-invasive focused ultrasound delivered by a toroidal transducer: Preclinical application in a pregnant monkey model for the treatment of twin-twin transfusion syndrome.

Twin-to-twin transfusion syndrome (TTTS) arises in 15% of monochorionic twin pregnancies due to unbalanced vascular anastomoses and remains a major cause of perinatal morbidity and mortality. Current treatment with laser fetoscopy is invasive, limited to superficial vessels, and technically challenging with anterior placentas. This study evaluated the feasibility, safety, and efficacy of non-invasive vascular occlusion using a hand-held high-intensity focused ultrasound (HIFU) device in a pregnant primate model. Seven pregnant monkeys with singleton fetuses were treated under general anesthesi

Proximate cord insertion in monochorionic twins with selective fetal growth restriction.

BACKGROUND: Monochorionic (MC) twins share a single placenta which can be unequally shared, leading to selective fetal growth restriction (sFGR). Limited data is available on the prevalence and clinical consequences of proximate cord insertion (PCI) in sFGR pregnancies. OBJECTIVE: We aimed to investigate the prevalence of PCI in MC placentas with and without sFGR and per type of sFGR, and study the placental characteristics and perinatal outcome of PCI in sFGR pregnancies. Risk of Cord Entanglement After Iatrogenic Monoamnionicity, With Selective and Solomon Laser Treatment for Twin-To-Twin Transfusion Syndrome in Monochorionic Twin Pregnancies.

INTRODUCTION: Fetoscopic laser surgery (FLS) is the gold standard treatment for monochorionic (MC) twin pregnancies complicated by twin-twin transfusion syndrome (TTTS). The aim of our study was to evaluate the rate and risk factors for cord entanglement in the presence of iatrogenic monoamnioticity (iMA), a consequence of inadvertent septostomy during FLS. METHODS: This is a retrospective analysis of two consecutive cohorts of FLS performed either using the selective technique from January 2004 to January 2012, or with the Solomon technique, from that date onwards. Maternal and fetal charact
