Revealing an enhanced cytotoxic immune microenvironment at the human maternal-fetal interface in preeclampsia.

Preeclampsia is a multifactorial pregnancy complication characterized by hypertension and immune dysregulation, which can lead to adverse maternal and fetal outcomes. Decidual immune cells are critical for pregnancy homeostasis, yet their functional abnormalities and mechanisms in preeclampsia remain poorly understood. Herein, we sorted decidua immune cells from preeclampsia patients for single-cell RNA sequencing to characterize cellular composition and functional heterogeneity. We delineated the immune cell landscape of the decidua in preeclampsia. A hallmark of decidual immune microenvironm

Optimal umbilical artery Doppler chart for predicting placenta-mediated fetal growth restriction.

OBJECTIVE: To compare the predictive accuracy of selected umbilical artery (UA) pulsatility index (PI) reference charts for outcomes associated with placenta-mediated fetal growth restriction (FGR). METHODS: This was a retrospective cohort study of individuals with a singleton pregnancy who underwent UA Doppler assessment ≥ 20 weeks' gestation between January 2012 and December 2022 at a single tertiary referral center, at which UA Doppler is measured routinely regardless of fetal size. Using 10 different UA-PI reference charts, we compared the predictive accuracy of an abnormal UA-PI (> 95th

Epigenetic Regulation of Imprinted Genes in the Neurodevelopment of Offspring Born to Women with Preeclampsia: A Review.

Preeclampsia (PE) is a severe hypertensive disorder of pregnancy that poses significant risks to both maternal health and offspring development. Increasing epidemiological evidence suggests that children born to women with PE have a higher risk of neurodevelopmental disorders (NDDs), including autism spectrum disorder (ASD), cognitive impairment, and attention-deficit/hyperactivity disorder (ADHD). However, the underlying biological mechanisms remain incompletely understood. This review aims to summarize current evidence on the association between PE and offspring neurodevelopmental outcomes,

Egfl7 regulates embryonic angiogenesis and placental vascular function in mice.

Epidermal growth factor-like domain 7 (Egfl7) is a secreted protein expressed in endothelial cells and plays an important role in angiogenesis and vascular stabilization. Previous studies have suggested an association between Egfl7 deficiency and vascular abnormalities; however, the specific functional roles of Egfl7 during the early stages of angiogenesis in embryonic development have not been fully elucidated. Therefore, in this study, we systematically investigated the functional role of Egfl7 during embryonic development using an Egfl7 knockout (KO) mouse model. Analysis of the embryonic m

Inhibition of circNUP93 (hsa_circ_0039480) activates the IRS-1/Akt pathway and alleviates insulin resistance in gestational diabetes mellitus via the MiR-767-3p/HIF1A axis.

Gestational diabetes mellitus (GDM) is characterized by insulin resistance (IR) and impaired glucose tolerance during pregnancy, yet the upstream regulatory mechanisms controlling insulin signaling remain unclear. The circular RNA hsa_circ_0039480 (circNUP93) has been proposed as a potential biomarker for GDM, but its functional role in disease progression is poorly understood. A high-fat diet-induced GDM mouse model and insulin-resistant HTR-8/SVneo trophoblast cell model were used to investigate the role of circNUP93. Knockdown experiments were performed to assess effects on glucose metaboli

The association between non-invasive prenatal testing and first-trimester preeclampsia screening.

OBJECTIVE: The non-invasive prenatal testing (NIPT) and preeclampsia screening test (PES) has been widely used to detect fetal chromosomal abnormalities and maternal preeclampsia during early pregnancy. Our aim was to investigate the correlation between PES parameters and NIPT aneuploidy results. MATERIALS AND METHODS: We retrospectively analyzed pregnancies that underwent both PES and NIPT. PES included multiples of the median (MoM) of PlGF, PAPP-A, MAP, and calculated risks for preeclampsia, intrauterine growth restriction (IUGR), and preterm delivery, while NIPT assessed risks for common a

Deep learning-based early prediction of gestational diabetes mellitus through first-trimester placental texture analysis.

INTRODUCTION: This study aimed to develop a multi-parameter fusion model for early GDM risk prediction and validate its performance through external multicenter testing. METHODS: A total of 628 pregnant women at 11+0-13+6 weeks were enrolled from two medical centers. The Center I cohort was divided into training (n = 356) and testing sets (n = 153). Radiomic features (1,289) and deep learning features (2,048) were extracted from placental ultrasound images. Feature-level fusion resulted in 3337 features, which were selected using Spearman correlation, mRMR, and LASSO. Five models were built:

Different Effects of the Anticonvulsant Drugs Phenytoin and Phenobarbital on Forskolin-Induced BeWo Syncytialization.

BACKGROUND: The placenta plays important roles in pregnancy maintenance and fetal development, and chemical-induced functional or structural abnormalities can lead to adverse pregnancy outcomes. However, information on the placental effects of chemicals remains limited. To help address this gap, this study aimed to investigate the effects of two model chemicals, phenytoin and phenobarbital, on syncytialization (the fusion of cytotrophoblasts into multinucleated syncytiotrophoblasts), a critical process in placental development, using the human choriocarcinoma cell line BeWo. METHODS: Phenytoi

Exposure to polycyclic aromatic hydrocarbons during maternal pregnancy: Effects of DNA damage on neural tube defects in offspring.

Prenatal polycyclic aromatic hydrocarbon (PAH) exposure may contribute to neural tube defects (NTD), but combined effects and underlying mechanisms remain unclear. We conducted a matched case-control study (128 pairs) in six high-risk counties in Shanxi Province, China (2004-2016). At delivery or pregnancy termination, placenta samples were collected. Sixteen placental PAHs and 11 hydroxylated metabolites were quantified by GC-MS/MS as biomarkers of fetal intrauterine exposure, alongside 16 DNA adducts measured by UPLC-MS/MS. Complementary ICR mouse experiments with benzo[a]pyrene (BaP, 100 mg

False negativity for trisomy 18 in non-invasive prenatal testing: A case report.

False-negative results in non-invasive prenatal testing are rare but clinically significant, particularly in the presence of biological factors such as feto-placental mosaicism. This report describes a pregnancy affected by fetal trisomy 18 in which three consecutive non-invasive prenatal tests reported low risk despite abnormal first-trimester ultrasound findings (increased nuchal translucency, hydrops fetalis, and tricuspid regurgitation). Amniocentesis subsequently confirmed trisomy 18 in the fetus. Post-termination placental analysis revealed mosaicism, with high trisomy 18 levels on the f

Onset of embryonic and placental defects coincide in 19 of 22 novel mid-gestation lethal murine knockout lines.

The focus on assessing embryo-specific defects in lethal knockout (KO) mouse lines has resulted in an underrepresentation of documented placental abnormalities. Presented herein is a uniform analysis of 22 distinct KO mouse lines that exhibit homozygous lethality in a narrow mid-gestation window. All genes altered by each KO have human orthologs, most are implicated in human disease, yet almost all are understudied. To unravel the role each plays in mammalian development, null embryonic and placental phenotype analysis was performed, and wild type (WT) expression of each KO gene was assessed.

Umbilical cord antiseptics for preventing sepsis and death among newborns.

RATIONALE: The umbilical cord, composed of blood vessels and connective tissue, connects the fetus to the placenta during pregnancy. After birth, the remaining cord stump may serve as an entry point for harmful bacteria, potentially leading to serious infection (neonatal sepsis) and death, particularly in low- and middle-income countries (LMICs). Applying antiseptics - substances that prevent the growth of microorganisms on living tissues - may help reduce the risk of infection through the umbilical stump. OBJECTIVES: To evaluate the benefits and harms of the application of antiseptics on a n

Multi-Omics Analysis Reveals Inflammatory Activation and Maternal-Fetal Interface Remodeling in Spontaneous Abortion.

BACKGROUND: Spontaneous abortion (SA) is a common adverse outcome of early pregnancy, yet its underlying pathophysiological mechanisms remain incompletely understood. Accumulating evidence suggests that dysregulated inflammatory responses at the maternal-fetal interface play a critical role in pregnancy loss. However, the potential associations between alterations in gut microbiota, metabolic disturbances, and localized decidual inflammation in patients with SA have not been systematically characterized. METHODS: Women with SA (n = 30) and those with normal early pregnancy (NP, n = 28) were e

Development of a simplified MRI model for diagnosing placenta accreta spectrum disorders.

INTRODUCTION: Accurate prenatal diagnosis of placenta accreta spectrum (PAS) disorders is crucial for reducing the risk of severe maternal morbidities. This study aims to develop and evaluate a simplified MRI model for PAS, assessing its performance across radiologists with varying levels of experience. METHODS: We conducted a retrospective study of surgically or pathologically confirmed PAS cases at our institution from 2016 to 2024. Two senior and two junior radiologists independently and blindly reviewed MRI images, focusing on two key categories: (1) uteroplacental interface abnormalities

A clinically oriented scoring system for placental pathology tested in late-onset fetal growth restriction and small-for-gestational-age pregnancies.

INTRODUCTION: Fetal growth restriction (FGR) is thought to originate from placental insufficiency, but late-onset FGR fetuses often lack signs of impaired uteroplacental circulation in Doppler sonography and distinction from small for gestational age (SGA) is challenging. SGA fetuses are often mistaken for constitutionally small and healthy. We aimed to investigate placental pathology in late-onset FGR and SGA compared to controls and to validate a novel semiquantitative scoring system for its clinical utility. METHODS: In this prospective cohort study of singleton pregnancies ≥32 weeks, plac

Differential expression of VEGF, PlGF and their receptors in placentas from anti-Ro/SSA- and anti-La/SSB-positive pregnant women.

Members of the vascular endothelial growth factor (VEGF) family of growth factors and their receptors, including VEGF-A, placental growth factor (PlGF), VEGF receptor (VEGFR)-1, and VEGFR-2 with co-receptor neuropilin-1 (NRP-1), are known to play a crucial role in the normal development and maintenance of the morphofunctional features of the placenta, also contributing to the peculiar immune tolerance occurring during pregnancy. Indeed, an altered expression of VEGF family members and their receptors has been implicated in inadequate placentation and dysregulation of the immune tolerance in a

Multi-omics insights into immune tolerance at the maternal-fetal interface in recurrent pregnancy loss: mechanisms, integration challenges, and translational perspectives.

Recurrent pregnancy loss (RPL) is a heterogeneous reproductive disorder in which dysregulation of maternal-fetal immune tolerance, aberrant decidual immune remodeling, and altered inflammasome signaling have been implicated within a complex multi-omics landscape. Multi-omics profiling (genomics, epigenomics, single-cell/spatial transcriptomics, proteomics, metabolomics, microbiome analyses, and immunomics) is increasingly being used to characterize mechanistic heterogeneity in RPL and to support biomarker discovery and immune-informed stratification. Genomic studies have associated chromosomal

Placental pathology and maternal risk factors as predictors of adverse neonatal outcomes in small-for-gestational-age births: A prospective cohort study in Northern Uganda.

BACKGROUND: Births that are small-for-gestational-age (SGA) represent a major cause of neonatal morbidity and mortality, particularly in sub-Saharan Africa. The contribution of specific placental histopathological lesions and their association with maternal risk factors remain poorly characterized in this high-burden setting. This study aimed to determine the incidence of adverse neonatal outcomes in SGA births and to identify associated placental and maternal predictors in northern Uganda. METHODS: A prospective cohort study was conducted at Lira Regional Referral and Kiryandongo General Hos
