Exploring hemodynamic alterations in gastroschisis: insights into fetal growth restriction using prenatal MRI.

OBJECTIVE: Despite the high incidence of fetal growth restriction (FGR) in gastroschisis (GS) associated with placental fetal vascular malperfusion (FVM), the underlying mechanism remains unclear. We aimed to investigate whether umbilical vein (UV) stenosis at the abdominal wall defect impairs fetoplacental circulation, leading to FGR. METHODS: A single-center retrospective review of magnetic resonance imaging (MRI) scans in fetuses with GS was performed. The presence of FGR and complex GS was recorded after birth. Systematic measurements of UV diameter were collected, as well as placental ch

Placental pathology and weight following Intrauterine Insemination (IUI) with or without ovarian stimulation (OS).

INTRODUCTION: Ovarian stimulation (OS) alters the peri-implantation environment and may affect placentation. Therefore, our study aimed to compare placental pathology and PW among singleton live births conceived after IUI with gonadotropins (Gn), oral medications (OM; clomiphene/letrozole), or unstimulated/natural (Nat) cycles. METHODS: Retrospective review of 386 IUI(±OS) singleton live births with placental examination: Gn (n = 222), OM (n = 129), and Nat (n = 35). Outcomes were placental lesions (classified as anatomic, inflammatory, infectious, or vascular) and PW. Generalized estimating

Altered placental development is present in all types of fetal congenital heart disease.

BACKGROUND: Fetal congenital heart defects (CHD) are related to fetal growth restriction, pregnancy complications and placental abnormalities. We hypothesize that abnormal placental development in fetal CHD is not solely associated to the altered fetal circulation, but is also related to abnormalities in developmental pathways. OBJECTIVES: To assess placenta and umbilical cord histology in pregnancies with fetal CHD and relate this to cardiac development and fetal oxygenation and flow. STUDY DESIGN: Placental tissue from pregnancies with fetal CHD (n = 111) and healthy controls (n = 80) was

Human umbilical mesenchymal stem cell-derived mitochondria transplantation suppresses sFLT-1 secretion by regulating calcineurin-NFAT-dependent pathways in angiotensin II-induced preeclampsia rats.

BACKGROUND: Mitochondrial transplantation (Mito-T) is a novel therapeutic strategy for ischaemic cardiovascular diseases. This study aimed to test the efficacy of human umbilical mesenchymal stem cell-derived mitochondrial transplantation (Mito-T) on preeclampsia (PE). METHODS: PE was induced in Sprague-Dawley pregnant rats by infusing angiotensin II (Ang II) starting on gestation day 8 (GD 8). Mito-T (100 μg/μl) was injected via the jugular vein on GD 14. RESULTS: On GD 20, PE rats exhibited high blood pressure, kidney and placental vascular abnormalities, reduced placental and foetal weigh

The Impact of Maternal Obesity and Diabetes on the Development of Congenital Heart Defects (CHDs) in Offspring: A Narrative Review.

Congenital heart disease (CHD) is the most common anatomical malformation occurring in live-born infants and an increasing cause of morbidity and mortality all over the world. Population-based observations have described associations between maternal cardiometabolic disorders and the risk of CHD in offspring. The present article is a narrative review. The aim of this study was to review the epidemiological evidence and clinical observations relating maternal obesity and diabetes mellitus to the risk of CHD in offspring, with particular attention paid to first trimester disturbances of fetal ca

Maternal prenatal stress is associated with altered MRI-derived placental diffusivity in low-risk pregnancies and pregnancies with Congenital Heart Disease.

OBJECTIVES: To assess if maternal stress is higher in pregnancies with congenital heart disease (CHD) compared to low-risk pregnancies and if maternal stress is associated with placental microstructure and function. To explore if CHD alters the relationship between maternal stress and placental measures. METHODS: In this prospective observational study, 27 participants carrying a fetus with CHD and 42 participants with typical low-risk pregnancies underwent 1-2 combined diffusion-T2∗ relaxation placental MRIs from 20 weeks gestation (GA) and completed the Edinburgh Postnatal Depression Scale

A novel histopathological insight: caspase-3 and Azan staining reveal placental malformations ameliorated by ethanolic extract of propolis in streptozotocin-induced diabetes during gestation.

Gestational diabetes mellitus is a common metabolic disorder of pregnancy associated with maternal metabolic disturbances, placental dysfunction, and adverse fetal outcomes. However, data on placental histopathological and apoptotic alterations in experimental models remain limited. This study evaluated the protective effects of an ethanolic extract of propolis on metabolic, hematological, oxidative, and placental structural changes in a rat model of diabetes induced during gestation using streptozotocin. Pregnant rats were assigned to four groups: a control group, a control group treated with

The maternal and infant gut microbiome: implications for pregnancy outcomes, immune development, and health in the first 1000 days.

BACKGROUND: The maternal microbiota is recognized as an important regulator of pregnancy outcomes and early infant immune programming. Across gestation, microbial communities in the gut, vaginal, oral, and putative placental niches undergo dynamic changes driven by hormonal, metabolic, and environmental factors, with important consequences for maternal–fetal health. These microbial transitions shape maternal immune-metabolic balance and are associated with major pregnancy complications, including gestational diabetes mellitus, preeclampsia, intrahepatic cholestasis of pregnancy, fetal growth r

Twin-twin transfusion syndrome and related monochorionic disorders: historical perspectives, current controversies, and evidence-based opportunities.

Twin-twin transfusion syndrome remains a major cause of morbidity and mortality in monochorionic twin pregnancies, arising from placental vascular anastomoses and representing a frequent indication for referral to fetal therapy centers. Despite advances in ultrasound assessment and fetoscopic laser therapy, variability in diagnostic thresholds, Doppler interpretation, and recognition of overlapping phenotypes-particularly twin anemia-polycythemia sequence and selective fetal growth restriction-continues to complicate clinical decision-making and limits meaningful comparison of outcomes across

Three for the price of one: dynamically reconfigurable generation of Gaussian, flat-top, and vortex/donut beams.

BACKGROUND: Non-invasive prenatal testing (NIPT) is widely adopted for its high sensitivity in detecting chromosomal abnormalities, but its accuracy for placental and fetal mosaicism is limited due to reliance on placental DNA, leading to false results and missed diagnoses. Prenatal ultrasound may signal chromosomal risks via soft markers. This study investigates ultrasound's diagnostic value for mosaicism in NIPT false-negative cases. MATERIALS AND METHOD: A retrospective analysis was conducted on two unique cases of trisomy 21 diagnosed at our hospital's prenatal diagnosis center. Case One:

Placental Dysfunction and Congenital Heart Disease: Investigating the Placenta-Heart Axis.

OBJECTIVE: Concurrent development of the placenta and heart during early gestation suggests a shared biological basis for the co-occurrence of abnormal placentation and congenital heart disease (CHD). This study investigated the association between placental vascular pathology and CHD type. METHODS: A retrospective study at two institutions included CHD (n = 521) and unaffected (n = 122) infants. CHDs were categorized into three groups (left and right ventricular outflow tract obstruction and mixed lesions) based on the anticipated effect of placental venous return streaming toward the fetal

Development of a robust method to derive human trophoblast stem cells from late-gestation placentas and its application to preeclampsia.

Trophoblasts are multifunctional cells in the placenta and essential for normal pregnancy. Although trophoblast dysfunction can cause pregnancy complications, the underlying mechanisms remain unclear, and effective treatments are limited, partly because of the scarcity of appropriate experimental models. We previously reported the derivation of human trophoblast stem cells (hTSCs) from 1st-trimester placentas and blastocysts, providing a powerful tool to investigate human trophoblast development and function. However, the difficulty in deriving hTSCs from late-gestation placentas has limited t

Placental Evidence of Intrauterine Stress: Mechanisms, Timing, and Links to Perinatal Brain Injury.

Placental examination is frequently performed following adverse fetal or neonatal neurologic outcomes to elucidate the cause or timing of injury. In practice, however, the placenta more often serves as a biologic archive of fetal stress, adaptation, and reduced physiologic reserve rather than a definitive record of injury. This review synthesizes current research linking placental pathology to perinatal brain injury, with emphasis on biological mechanisms and temporal interpretation. We examine how placental findings inform assessments of intrauterine stress, impaired placental or fetal perfus

Fetoplacental microcirculation abnormalities are reversed by aerobic exercise in mice with preeclampsia.

Preeclampsia (PE) is a hypertensive disorder of pregnancy characterized by hypertension and organ dysfunction, and is associated with impaired placental perfusion and systemic endothelial dysfunction. Aerobic exercise has been proposed as a potential nonpharmacological strategy to improve vascular function during pregnancy; however, its effects on fetoplacental microcirculation in PE remain poorly understood. Therefore, the objective of this study was to investigate whether maternal aerobic exercise during gestation could influence fetoplacental perfusion and maternal pathological features in

Naringenin protects against L-NAME-induced preeclampsia and fetal developmental abnormalities via modulation of inflammation, eNOS/NO signaling, AKT/mTOR pathway, and restoring endothelial function.

Preeclampsia considered as multi-organ disorder involving systemic inflammation and endothelial dysfunction. Our work was designed to investigate the protective role of Naringenin (NGN) against Nω-nitro-l-arginine methyl ester (L-NAME)-induced placental damage and congenital abnormalities in a rat model of preeclampsia. Forty-five adult female albino rats (weighing 160-180 g) were randomly allocated into nine groups (n = 6 per group) and treated for a duration of 18 days. Group 1 served as the untreated control, while Group 2 received 10% DMSO as a vehicle control. Group 3 was administered L-N

Mechanism of Shoutai Wan against recurrent spontaneous abortion: regulation of decidual vascular remodeling via ERβ-ANGPT2 signaling axis.

Shoutai Wan (STW), a classic traditional Chinese medicine formula used to tonify the kidney and prevent miscarriage, has been widely applied in the clinical management of recurrent spontaneous abortion (RSA). Increasing clinical evidence supports its efficacy in reducing miscarriage rates and improving pregnancy outcomes. However, the molecular basis by which STW alleviates defective decidual vascular remodeling in unexplained RSA remains insufficiently understood. Clinically, decidual ERβ and ANGPT2 expression, as well as serum estradiol, ANGPT2 and VEGFA levels were significantly decreased i

Preliminary embryo-fetal developmental toxicity study of pritelivir in CD1-IGS mice.

Pritelivir is a novel anti-herpes simplex virus drug that is currently in clinical trials in nonpregnant immunocompromised subjects. Here we conducted a preliminary embryo-fetal developmental toxicity study of pritelivir in CD1-IGS mice. Pritelivir was administered by oral gavage to time-mated mice at dose levels of 0, 10, 60 and 200 mg/kg/day from gestational day 6 through 15. Maternal food consumption, body weight and clinical observations were monitored throughout gestation. The mice were sacrificed at gestational day 18 and assessed based on embryo-fetal development and viability. Fetuses

Nanomaterial-Based Strategies to Modulate Placental Dysfunction in Preeclampsia.

Preeclampsia is a pregnancy-specific disorder characterized by hypertension and multi-organ dysfunction, originating primarily from placental abnormalities rather than maternal pathology alone. Impaired trophoblast invasion, defective spiral artery remodeling, angiogenic imbalance, oxidative stress, and sterile inflammation collectively drive placental dysfunction and subsequent systemic endothelial injury. Current clinical management remains largely supportive, highlighting the urgent need for mechanism-based therapeutic strategies that directly target the placenta while minimizing fetal risk
