BACKGROUND: Artificial intelligence (AI) has been increasingly integrated with fetal and placental magnetic resonance imaging (MRI) to enhance the detection of abnormalities and streamline diagnostic processes. MRI, known for its superior soft tissue contrast and multiplanar imaging capabilities, is a critical tool for evaluating complex fetal and placental conditions. However, the large volume of MRI data often poses challenges for clinicians in providing timely and accurate diagnoses. Fetal and placental MRI are subject to several inherent limitations, including fetal motion artifacts, low s

Zika virus (ZIKV) infection during pregnancy is a critical driver of Congenital Zika Syndrome (CZS), yet the mechanisms of pathogenesis at the placental barrier remain incompletely understood. This article is a translational, observational, and experimental study combining clinical placental analyses, placental explants cultures and in vivo murine model to investigate the mechanisms involved in ZIKV infection. We evaluate the histopathological analyses to verify presence of inflammation in ZIKV-infected human placentas from newborns with CZS and without CZS (N-CZS), identifying more intense Ho

BACKGROUND: Animal models remain essential for understanding developmental toxicology, providing insights into how in utero exposures affect fetal and placental outcomes. Litter-bearing species are widely used due to their efficiency and reproducibility, but conventional approaches often summarize outcomes at the litter level, masking meaningful within-litter variability.
AIM: This review highlights three primary sources of intralitter variability: litter size, uterine implantation location, and fetal sex, and their influence on fetal and placental growth, mortality, and malformations.

BACKGROUND: Predicting a diagnosis of pre-eclampsia is based on a combination of clinical assessment of blood pressure, presence of protein in the urine, symptoms and laboratory test abnormalities. Accurately detecting pre-eclampsia is important to avoid false-positive diagnoses which could lead to unnecessary antenatal admissions and/or preterm delivery. Four blood tests that measure the biomarkers of placental growth factor or the ratio of soluble fms-like tyrosine kinase-1 to placental growth factor are available (known as Triage, Elecsys, DELFIA Xpress, and BRAHMS Kryptor tests). Abnormal

INTRODUCTION: Postpartum hemorrhage (PPH) is a major obstetric emergency that contributes substantially to maternal morbidity following cesarean delivery. The identification of high-risk patients undergoing repeat cesarean sections may help improve perioperative preparedness and maternal outcomes. The present study aimed to identify predictors of PPH following repeated cesarean sections.
MATERIALS AND METHODS: A retrospective cohort study was conducted in the Department of Obstetrics and Gynecology on 200 women who underwent repeat cesarean section between January 2020 and December 2025. Demo

BACKGROUND: Postpartum haemorrhage (PPH) accounts for approximately 27% of maternal deaths worldwide and often requires blood transfusion, along with its associated risks. In resource-rich settings like South Korea, transfusion disparities persist, highlighting the need for predictive tools to optimise patient blood management (PBM).
OBJECTIVE: To apply machine learning (ML) to rank predictors of transfusion in PPH using national claims data.
METHODS: This was a retrospective cohort study of 123 763 women with PPH (2017-2019) from the National Health Insurance Service (NHIS) database (97% po

Eclampsia remains one of the most severe obstetric emergencies associated with substantial maternal and perinatal morbidity and mortality. Although classically regarded as a hypertensive complication of pre-eclampsia, atypical presentations without sustained hypertension or severe clinical features may occur, complicating risk prediction and timely intervention. We report a case of atypical eclampsia in a 21-year-old woman hospitalized with a diagnosis of pre-eclampsia without severe features. Despite predominantly normal blood pressure values and initially stable laboratory findings, the sFlt

Burosumab is a monoclonal antibody targeting fibroblast growth factor 23 (FGF23) and is approved for the treatment of X-linked hypophosphatemia. Its use during pregnancy has not been studied, and fetal exposure may affect neonatal mineral metabolism. We report a neonate exposed to Burosumab throughout gestation who developed prolonged phosphate retention, suppressed parathyroid hormone, and transient hypercalcemia after birth. Serial biochemical monitoring demonstrated persistently elevated tubular phosphate reabsorption and low urinary phosphate excretion beyond the early neonatal period, con

Preeclampsia is a major cause of maternal and perinatal morbidity around the world. It is increasingly recognized as a disorder of systemic immunometabolic dysregulation rather than isolated placental dysfunction. Increasing evidence links chronic inflammation, insulin resistance, and changes in uric acid metabolism to the initiation and progression of preeclampsia. In addition, emerging evidence indicates that maternal gut dysbiosis is an upstream regulator of systemic immune and metabolic dysfunction via the gut-systemic-decidual axis. This review synthesizes current mechanistic, clinical, a

BACKGROUND: Hypoglycemia in pregnancy is relatively understudied, with conflicting evidence suggesting either outcomes similar to normal oral glucose tolerance test (OGTT) or associations with poor neonatal outcomes. Maternal glucose crosses the placenta, thus maternal hypoglycemia may affect the fetus.
OBJECTIVE: This study aimed to evaluate the association between maternal hypoglycemia and neonatal birth weight, specifically the risk of small-for-gestational-age (SGA) neonates, and to examine maternal metabolic outcomes in comparison to normoglycemic and hyperglycemic individuals.

Background: Cardiotocography (CTG) represents a cornerstone in modern intrapartum fetal surveillance, allowing continuous assessment of fetal well-being through the analysis of fetal heart rate patterns in relation to uterine contractions. Despite its widespread clinical use, the interpretation of CTG tracings remains complex and is often associated with high interobserver variability and limited specificity in predicting adverse outcomes. In recent years, increasing attention has been directed toward placental pathology as a key determinant in the pathophysiology of adverse perinatal events.

BACKGROUND: Monochorionic twin pregnancies provide a unique model to investigate fetal phenotypic discordance, because both fetuses share a single placenta and interconnected vascular circulation. Although most monochorionic twins are monozygotic, clinically relevant differences may arise through genetic, epigenetic, placental, and stochastic developmental mechanisms.
METHODS: This systematic review was conducted according to PRISMA 2020 and registered in PROSPERO (CRD420261432361). PubMed/MEDLINE, Embase, and Scopus were searched from inception to June 2026. Eligible studies included monocho

Beginning with a general understanding of catastrophic obstetric collapse (COC), it has been established that a catastrophic obstetric collapse is typically the result of sudden massive bleeding requiring emergency peripartum hysterectomy (EPH); this is different from historical views of what constitutes a catastrophic obstetric collapse. Current studies have found evidence that a catastrophic obstetric collapse can be the result of a longer-duration process involving gradual maternal physiological destabilization, the culmination of which creates a "maternal point of no return" for the mother

Transamniotic stem cell therapy (TRASCET) constitutes the least invasive approach to prenatal stem cell delivery described to date. This strategy is largely based on the recently discovered active transport of donor cells from the amniotic fluid to the placenta directly through the amnio-placental interface. From a simple amniocentesis, via the placenta, donor cells of different phenotypes can reach the fetal circulation and virtually any fetal anatomical site, including, and particularly, the fetal bone marrow. As a result, TRASCET has shown broad therapeutic potential in an assortment of ani

BACKGROUND: Determining placental functional capacity from a typical pathology report can be difficult. Our goal was to create a new way to report on the functional capacity of the placenta, using much of the typical information obtained in a routine placental pathology assessment.
METHODS: Placental functional capacity score (PFCS), a continuous variable between 0 and 100, was defined by a combination of placental weight compared to that expected for gestational age, villous maturational abnormalities, percentage of parenchymal lesions, and presence of inflammation. We compared PFCS distribu

Microplastic (MP) contamination has emerged as a global environmental concern, with increasing evidence of its presence in human placental tissue. However, no previous human studies have directly evaluated the association between placental microplastic exposure and fetal cardiac physiology during labor. Our study aimed to investigate the relationship between placental MP accumulation and the occurrence of intrapartum non-reassuring fetal heart rate (NRFHR). A prospective cohort study was conducted among 2738 pregnant women recruited from China between 2023 and 2024. Placental MPs were quantifi

BACKGROUND: Environmental and dietary exposure to sulfamethoxazole (SMX) has been detected in pregnant populations, but its impact on preeclampsia (PE)-related placental pathology-particularly through non-therapeutic chronic exposure-remains poorly understood.
METHODS: We assessed the effects of maternal SMX exposure on placental structure in pregnant mice. Placental transcriptomic data and Mendelian randomisation (MR) analysis were integrated to identify candidate targets. Molecular docking, molecular dynamics simulations, cellular thermal shift assay (CETSA), and microscale thermophoresis (
