Causes of intrauterine fetal death (IUFD) in term singleton pregnancies (2014-2023): a retrospective study.

BACKGROUND: The risk factors of intrauterine fetal death (IUFD) at term remain uncertain. The cause of death is often difficult to accurately define. This study analyzed the clinical characteristics and the causes of stillbirth at a tertiary maternity center over a 10-year period. METHODS: A retrospective study included 70 women with unexpected intrauterine fetal death (IUFD) in singleton term pregnancies (2014 - 2023) at Women's Hospital, Zhejiang University School of Medicine. Cases with fetal anomalies or intrapartum death were excluded. Fetal and maternal variables were analyzed to assess

Ascending infection is the leading cause of antepartum hemorrhage: A case-control study.

INTRODUCTION: Placental abruption is a significant obstetric complication characterized by the premature separation of the placenta from the uterine wall, affecting 2-3 per 100 pregnancies. This condition presents a dual diagnostic challenge: obstetricians rely on clinical signs such as antepartum hemorrhage, abdominal pain, and ultrasound findings to suspect abruption, often leading to emergency cesarean delivery. Conversely, pathologists diagnose abruption through macroscopic and microscopic examination of placental tissue, identifying hematomas and signs of maternal vascular malperfusion. N

Perioperative Setting in Emergency Caesarean Section for a Pregnant Woman With Coronavirus Disease 2019: A First Case Report.

Emergency caesarean section (ECS), also referred to as a "category 1 caesarean section," is less commonly performed but may become unexpectedly necessary in critical obstetric emergencies. During a trial of labour in high-risk pregnancies, the potential need for ECS should always be anticipated. Even during the coronavirus disease 2019 (COVID-19) pandemic, trials of labour in pregnant women with COVID-19 have remained common. While preoperative management protocols for ECS are essential as contingency plans, well-defined strategies for the safe and timely preparation of ECS in pregnant women w

Partial ischemic necrosis of the uterus following artery ligation for postpartum hemorrhage after placental abruption: a case report.

BACKGROUND: Uterine necrosis is a rare condition that most commonly occurs during the postpartum period as a severe side effect of conservative interventions for postpartum hemorrhage. The primary treatment for uterine necrosis is a hysterectomy, and conservative management is rarely reported. CASE PRESENTATION: We present the case of a 30-year-old Chinese woman, unipara, who underwent an emergency cesarean section at 35 weeks of gestation owing to a severe placental abruption. Postpartum hemorrhage occurred during the operation, and uterine artery ligation was employed to control the bleedin

Safety and efficacy of atosiban for fetomaternal resuscitation following severe placental abruption in preparation for an emergency cesarean section: a narrative review.

INTRODUCTION: Placental abruption is a significant cause of fetomaternal mortality/morbidity. Management requires a difficult balance between opposing advantages/disadvantages for the fetus and mother. Remote from term, prompt delivery is best for the mother, but delayed delivery better for the fetus. AREAS COVERED: Prevalence of fetomaternal morbidity/mortality; range of clinical situations: certainty of diagnosis, gestational age, labor status, fetal condition/viability, presence of maternal shock/consumption coagulopathy, feasibility of prompt delivery. Pathophysiology: maternal hypovolaem

Familial risk of placental abruption.

BACKGROUND: Placental abruption (PA), the partial or complete separation of the placenta before delivery, occurs in 1-2 % of pregnancies and is associated with severe maternal and perinatal morbidity and mortality. The etiology of PA is complex, and the increased recurrence risk in subsequent pregnancies suggests a potential genetic contribution. Investigating PA in large families may reveal important insights into inherited risk and the extent to which genetic factors contribute to PA. To guide future gene-discovery studies, we investigated the familial risk of PA using data from 1.2 million

Perinatal outcomes and predictors of placental abruption: a retrospective study in an Ethiopian tertiary care center.

BACKGROUND: Placental abruption is a critical obstetric condition characterized by the premature separation of the placenta from the uterus, leading to severe maternal and fetal complications. In Ethiopia, the maternal and perinatal morbidity and mortality rates are alarmingly high, and placental abruption significantly contributes to these adverse outcomes. Despite its severity, there is a lack of comprehensive data on the burden, risk factors, and outcomes associated with placental abruption in the Ethiopian context. Thus, the study aimed to investigate the adverse perinatal outcomes of plac

Does Adjusting for Causal Intermediate Confounders Resolve the Perinatal Crossover Paradox?

BACKGROUND: Mediation analyses of the preeclampsia-perinatal outcome association through preterm birth (PTB) have produced paradoxical findings. For example, preeclamptic births at preterm gestations show a lower risk of adverse outcomes than normotensive births. These results have been explained by unmeasured baseline confounding between PTB and outcomes, with PTB as the sole mediator. However, other intermediate variables, such as placental abruption, small for gestational age (SGA) births, and chorioamnionitis, are confounders yet are excluded because they occur after preeclampsia. METHODS

Urinary cotinine levels versus self-reported smoking status for assessing placental abruption risk: the Japan Environment and Children's study.

INTRODUCTION: Placental abruption is a serious complication of pregnancy, and smoking is a well-known risk factor thereof. However, evaluations of the effects of smoking mostly rely on self-reported questionnaires, resulting in incorrect representation of smoke exposure owing to incorrect reporting and missing data on passive smoking. Cotinine is a major nicotine metabolite and a sensitive biomarker in smokers. However, to date, there have been no reports on the relationship between urinary cotinine levels and onset of placental abruption. Hence, this study aimed to compare smoking status asse
