Maternal Morbidity and Complications in Hemolysis, Elevated Liver Enzymes, and Low Platelet Count (HELLP) Syndrome: A Retrospective Case Series.

Background HELLP syndrome (Hemolysis, Elevated Liver Enzymes, and Low Platelets) is a severe obstetric complication associated with significant maternal morbidity and mortality. While its epidemiology is well-described in high-resource settings, data from conflict-affected and resource-limited regions like Afghanistan are critically scarce. This study aimed to delineate the incidence, risk profiles, complications, and outcomes of HELLP syndrome at a major tertiary healthcare hospital. Methodology A retrospective, descriptive case series was conducted at Rabia Balkhi Hospital in Kabul, reviewin

Cardiovascular Disease in Singleton Offspring Born of Pregnancies Complicated by Placental Abruption: A Population-Based Retrospective Cohort Study.

BACKGROUND: Placental abruption engenders powerful risks of maternal cardiovascular disease (CVD), but offspring risks are lacking. We examine the associations between abruption and the risks of all-cause mortality and fatal and nonfatal CVD events in the offspring. METHODS: We designed a retrospective cohort study using the PACER (Placental Abruption and Cardiovascular Event Risk) cohort (births in New Jersey, 1993-2020). We fit Cox proportional hazards models to estimate the abruption-associated risks of mortality in singleton offspring and of fatal and nonfatal CVD events. These associatio

Associations between maternal injury and adverse perinatal outcomes: evidence from a retrospective cohort study.

BACKGROUND: Maternal injury affects 6% to 8% of pregnancies and is associated with perinatal complications. Some outcomes remain understudied, with limited evidence on variation of associations by injury characteristics. OBJECTIVE: To estimate associations between maternal injury and adverse perinatal outcomes. STUDY DESIGN: We conducted a retrospective cohort study using electronic health record data at the Atrium Health Wake Forest Baptist health system (2018-2024 births). We identified maternal injury using validated ICD-10-CM diagnostic codes and stratified injury by mechanism (transport

The impact of antinuclear antibodies and complement levels in the prognosis of pregnant women within the antiphospholipid syndrome spectrum.

INTRODUCTION: To assess the prevalence and clinical impact of antinuclear antibodies (ANA) and low complement levels on fetal-maternal outcomes in pregnant women within the spectrum of obstetric antiphospholipid syndrome (APS). METHODS: A total of 285 ever-pregnant women with APS-related obstetric morbidity were included. Among them, 85 fulfilled APS classification criteria, 139 were classified as non-criteria APS (NC-APS), and 61 as seronegative APS (SN-APS). Patients with other autoimmune diseases were excluded. Adverse pregnancy outcomes (APO) included early pregnancy loss, fetal death, pr

Impact of Maternal Risk Stratification on Clinical Characteristics and Perinatal Outcomes in Singleton Pregnancies With Preeclampsia.

This retrospective study of 4143 women at Fujian Maternal and Child Health Hospital (2012-2022) investigated how maternal risk levels influence clinical characteristics and perinatal outcomes in singleton pregnancies complicated by preeclampsia. Utilizing class-imbalance-adjusted and random forest algorithms, participants were stratified into high-risk (17.38%), moderate-risk (34.08%), and low-risk (48.54%) groups. The analysis revealed that women in the high-risk category were diagnosed at a significantly earlier gestational age and faced higher rates of severe, early-onset, and preterm preec

Association between small-for-gestational-age at birth and placental abruption in normotensive pregnancies: a retrospective cohort study.

BACKGROUND: Hypertensive disorders in pregnancy (HDP) are well-established risk factors for placental abruption (PA). Impaired fetal growth is another candidate, yet its association with abruption in the absence of hypertension remains unverified. We used small-for-gestational-age (SGA) at birth as a pragmatic proxy to investigate whether it is independently associated with PA in normotensive pregnancies. METHODS: A retrospective cohort study was conducted at a tertiary hospital in South China. The study population consisted of women with singleton deliveries between January 1, 2019, and Decem

An Unusual Cause of Post-cesarean Intraperitoneal Hemorrhage: Inadvertent Intra-abdominal Placement of a Femoral Venous Catheter.

Severe abruptio placentae is a major obstetric emergency that may require aggressive resuscitation and central venous catheterization. Femoral venous access is often selected in this context because of its accessibility and absence of pneumothorax risk; however, inadvertent intraperitoneal malposition of the catheter is an exceptionally rare and potentially life-threatening complication. We report the case of a 25-year-old woman at 36 weeks of gestation, admitted for abruptio placentae with intrauterine fetal death and hemorrhagic shock, who underwent emergency cesarean section and was transfe

Adverse Pregnancy Outcomes and Risk of Subarachnoid Hemorrhage: A 50-Year Population-Based Cohort Study in Sweden.

BACKGROUND: Adverse pregnancy outcomes (APOs) are increasingly recognized as early indicators of maternal cardiovascular risk. However, their associations with nontraumatic subarachnoid hemorrhage (SAH) remain poorly understood. METHODS: We conducted a nationwide cohort study of 1 785 088 primiparous women in the Swedish Medical Birth Register between 1973 and 2014, followed from first birth through December 31, 2023. APOs included hypertensive disorders of pregnancy, gestational diabetes, placental abruption, preterm birth, abnormal fetal growth, and stillbirth. SAH was the primary outcome,

Perinatal outcomes and predictors of placental abruption: a retrospective study in an Ethiopian tertiary care center.

BACKGROUND: Placental abruption is a critical obstetric condition characterized by the premature separation of the placenta from the uterus, leading to severe maternal and fetal complications. In Ethiopia, the maternal and perinatal morbidity and mortality rates are alarmingly high, and placental abruption significantly contributes to these adverse outcomes. Despite its severity, there is a lack of comprehensive data on the burden, risk factors, and outcomes associated with placental abruption in the Ethiopian context. Thus, the study aimed to investigate the adverse perinatal outcomes of plac

Does Adjusting for Causal Intermediate Confounders Resolve the Perinatal Crossover Paradox?

BACKGROUND: Mediation analyses of the preeclampsia-perinatal outcome association through preterm birth (PTB) have produced paradoxical findings. For example, preeclamptic births at preterm gestations show a lower risk of adverse outcomes than normotensive births. These results have been explained by unmeasured baseline confounding between PTB and outcomes, with PTB as the sole mediator. However, other intermediate variables, such as placental abruption, small for gestational age (SGA) births, and chorioamnionitis, are confounders yet are excluded because they occur after preeclampsia. METHODS

Urinary cotinine levels versus self-reported smoking status for assessing placental abruption risk: the Japan Environment and Children's study.

INTRODUCTION: Placental abruption is a serious complication of pregnancy, and smoking is a well-known risk factor thereof. However, evaluations of the effects of smoking mostly rely on self-reported questionnaires, resulting in incorrect representation of smoke exposure owing to incorrect reporting and missing data on passive smoking. Cotinine is a major nicotine metabolite and a sensitive biomarker in smokers. However, to date, there have been no reports on the relationship between urinary cotinine levels and onset of placental abruption. Hence, this study aimed to compare smoking status asse

A retrospective analysis of the incidence and risk factors for amniotic fluid embolism in cesarean deliveries.

BACKGROUND: Amniotic-fluid embolism (AFE) is a rare occurrence but life-threatening disease that can occur in process of delivery and postnatal women. The topic of cesarean section surgery and its associated risk factors has been neglected due to the infrequent occurrence of AFE. However, AFE significantly contribute to maternal morbidity and mortality. PURPOSES: We aim to examine the correlation between clinical issues, prenatal complications, comorbidities, medical perioperative complications, and cesarean section surgery of AFE in the US, utilizing a Nationwide Inpatient Sample (NIS) datab

Placental Abruption and Perinatal Mortality: Abnormal Placentation and Spontaneous Abortion as Contributors to Left Truncation Bias.

BACKGROUND: Generally, studies in perinatal epidemiology restrict cohort entry to 20 weeks of gestation, but exposures and outcomes may occur earlier. This restriction may introduce left truncation bias. OBJECTIVES: To examine the impact of left truncation bias when estimating the causal effect of abruption on perinatal mortality in the context of abnormal placentation, with spontaneous abortion (SAB) as a censoring event. METHODS: Through 80 Monte Carlo simulation scenarios based on realistic clinical assumptions, we estimated risk differences (RD), risk ratios (RR) and bias parameters for
