Placental Abruption and Perinatal Mortality: Abnormal Placentation and Spontaneous Abortion as Contributors to Left Truncation Bias.

BACKGROUND: Generally, studies in perinatal epidemiology restrict cohort entry to 20 weeks of gestation, but exposures and outcomes may occur earlier. This restriction may introduce left truncation bias. OBJECTIVES: To examine the impact of left truncation bias when estimating the causal effect of abruption on perinatal mortality in the context of abnormal placentation, with spontaneous abortion (SAB) as a censoring event. METHODS: Through 80 Monte Carlo simulation scenarios based on realistic clinical assumptions, we estimated risk differences (RD), risk ratios (RR) and bias parameters for

Comparison of Seven-Day Versus Continuous Prophylactic Antibiotic Therapy Until Delivery in Preterm Premature Rupture of Membranes.

Background and aim Preterm prelabour rupture of membranes (PPROM) refers to the spontaneous rupture of fetal membranes before the onset of labor and prior to 37 completed weeks of gestation. PPROM is associated with significant maternal and neonatal complications. Maternal risks include chorioamnionitis, abruptio placentae, and postpartum infections. Neonatal complications commonly observed are respiratory distress syndrome (RDS), neonatal sepsis, cerebral palsy, and necrotizing enterocolitis (NEC). This study aimed to evaluate and compare maternal and neonatal outcomes in women with PPROM tre

Analgesic opioids in pregnancy and placental malperfusion-related disorders: a population-based cohort study.

BACKGROUND: Analgesic opioid use in pregnancy could increase the risk of disorders related to placental malperfusion, but this relationship is incompletely characterized. We aimed to study the causal association between analgesic opioids in pregnancy and placental abruption, pre-eclampsia, preterm birth, and fetal growth restriction (FGR). METHODS: We conducted a population-based cohort study of pregnancies resulting in birth at ≥20 weeks of gestation between July 2013 and December 2019 in New South Wales, Australia. Linked data on pregnancy, births, medication dispensation, and health servic

Placental Abruption: Temporal Trends, Risk Factors, and Associated Adverse Maternal Outcomes.

This study aimed to evaluate trends in placental abruption during delivery hospitalizations and associated risk factors and adverse outcomes.Delivery hospitalizations with and without placental abruption were identified using billing codes in the 2000 to 2020 National Inpatient Sample for this serial cross-sectional study. Temporal trends in abruption were analyzed with Joinpoint regression to determine the average annual percentage change (AAPC) in abruption. The association between hospital, demographic, and clinical factors and abruption was analyzed with adjusted logistic regression models

Maternal near miss and maternal mortality and their determinants among pregnant women at a rural tertiary centre in the Eastern Cape Province, South Africa: A cross-sectional study.

BACKGROUND: Maternal near miss (MNM) is a significant entity in the assessment of the quality of obstetric care, particularly at institutional level. It renders possible the assessment of events surrounding a woman surviving what would otherwise have been a fatal outcome during childbirth or within 42 days post termination of pregnancy. The World Health Organization (WHO) introduced a tool that eases the identification of such cases and the interventions that were offered during their care. There have been several assessments of maternal deaths within South Africa (SA), but no audit of materna

Tacrolimus treatment in reproductive failures.

Immunotherapy for repeated implantation failure (RIF) following in vitro fertilization (IVF), particularly treatment with the calcineurin inhibitor tacrolimus-has now been used for roughly a decade since its initial report. In that initial series, the clinical pregnancy rate was 64.0 %; however, acceptance was slow. A recent re-evaluation at our center yields a similarly high clinical pregnancy rate of 57.1 %. In our earlier cohort, 37.7 % of RIF cases showed Th1/Th2 cell ratio suggestive of immune-mediated rejection; a more recent survey found a comparable proportion (35.9 %), indicating that

Early-third-trimester uterine rupture in an unscarred primigravida with a didelphic uterus: a case report.

INTRODUCTION: Uterus didelphys, one of the rarest types of Müllerian anomaly, is characterized by the presence of two separate uterine horns and/or two distinct cervices due to the failure of Müllerian duct fusion. It is associated with poor obstetric outcomes, including spontaneous abortion, preterm labor, fetal malpresentation, and, rarely, uterine rupture. CASE PRESENTATION: A 19-year-old primigravida from Sela Dingay woreda (resident of Sela Dingay town) in the North Shewa Zone of the Amhara Region was urgently referred to Debre Berhan University Hakim Gizaw Teaching Hospital from the wor

Late Pregnancy Bleeding.

Late pregnancy bleeding, any vaginal bleeding that occurs after 20 weeks' gestation, can be due to obstetric emergencies or benign etiologies. The amount of blood loss does not always correlate with seriousness of the etiology, and the source of bleeding is often not immediately obvious. Even small amounts of blood loss should be evaluated carefully because this may be the initial symptom of emergent bleeding. Emergent obstetric etiologies include placental abruption, placenta previa, vasa previa, and uterine rupture. Placental abruption rarely presents with the classic triad of vaginal bleedi

Common sleep characteristics and the risk of common perinatal complications and adverse outcomes: a multi-sample, bidirectional Mendelian randomization study.

BACKGROUND: Improving maternal and child health has been a global priority since the early 2000s, with a focus on reducing perinatal complications and improving overall maternal well-being. Sleep characteristics influence various health outcomes, yet their role in perinatal complications and adverse outcomes remains poorly understood. METHODS: A Mendelian randomization analysis was conducted, using seven common sleep characteristics (sleeplessness, sleep duration, getting up in the morning, daytime napping, morning/evening person, narcolepsy, snoring) as exposure factors and twelve common per

Dual lives at risk: managing haemoperitoneum and splenic injury in pregnancy.

A pregnant woman in her 20s, at 31 weeks gestation, presented with severe abdominal pain, reduced fetal movements and mild vaginal bleeding following a fall. Examination revealed pallor, a tense abdomen and absent fetal movements. Point-of-care ultrasound indicated haemoperitoneum and placental abruption. Emergency laparotomy and lower segment caesarean section were performed, delivering a 1 kg neonate with Apgar scores of 5 and 7. Intraoperative findings included 3 L haemoperitoneum, placental abruption and splenic hilar vascular injury, necessitating splenectomy. The mother received blood tr

Identification of novel genomic regions associated with yield-related traits in cowpea (Vigna unguiculata [L.] Walp) landraces.

BACKGROUND: Hypersplenism is commonly seen in patients with non-cirrhotic portal hypertension (NCPH). While a splenectomy alone can effectively relieve the hypersplenism, it does not address the underlying portal hypertension. The present study was undertaken to analyze the impact of shunt and non-shunt operations on the resolution of hypersplenism in patients with NCPH. The relationship of symptomatic hypersplenism, severe hypersplenism and number of peripheral cell line defects to the severity of portal hypertension and outcome was also assessed. METHODS: A retrospective analysis of NCPH pa

Propensity score analysis of low-dose aspirin and bleeding complications in pregnancy.

OBJECTIVE: Low-dose aspirin (LDA) has been shown to reduce the risk of preterm pre-eclampsia and it has been suggested that it should be recommended for all pregnancies. However, some studies have reported an association between LDA and an increased risk of bleeding complications in pregnancy. Our aim was to evaluate the risk of placental abruption and postpartum hemorrhage (PPH) in patients for whom their healthcare provider had recommended prophylactic aspirin. METHODS: This multicenter cohort study included 72 598 singleton births at 19 hospitals in the USA, between January 2019 and Decemb

Labisia pumila as a Culprit of Primary Uterine Rupture Alongside Abruptio Placentae: A Case Report.

PURPOSE: Uterine rupture is a rare obstetrical and surgical emergency but is often associated with a catastrophic obstetric complication. Traditionally, unscarred uteri without risk factors are considered immune to rupture and information about its risk factors is only few due to its rarity. Complementary and alternative medicine is commonly used in pregnant woman in southeast Asian countries such as Indonesia, however information regarding its toxicity due to irrational use is few. This case report presents a G2P1A0 35-year-old woman with acute abdomen with spontaneous unscarred uterine ruptu
