Factors associated with stillbirth in four selected hospital maternity units: an unmatched case-control study in Eswatini.

BACKGROUND: Stillbirth is a substantially under-recognized adverse pregnancy outcome that predominantly occurs in low-middle-income countries. In 2019, Eswatini's stillbirth rate was 13.2 per 1,000 births, higher than global targets. However, there is limited research on stillbirths in Eswatini. This study analyzed factors associated with stillbirths among women giving birth in four selected hospitals. METHODS: A multi-center unmatched case-control study was conducted using secondary data sources from July 1 to December 31, 2021. Birth records of 268 stillbirths (cases) and 1,151 live births

Evaluating the 2023 European Association for the Study of the Liver Guidelines for Intrahepatic Cholestasis of Pregnancy: Risk Stratification and Outcomes in Over 4,000 US Patients.

INTRODUCTION: Intrahepatic cholestasis of pregnancy (ICP) is the most common liver disease of pregnancy, varying in incidence based on population and associated with adverse outcomes. We assessed the prevalence and outcomes of ICP categories defined by the 2023 European Association for the Study of the Liver guidelines in a diverse US population. METHODS: We conducted a retrospective cohort study (January 2009-April 2024) at our ethnically diverse health system. Suspected ICP patients were identified by pregnancy records with total serum bile acids (TSBA) ordered and categorized into Group A

The rate of stillbirth and its associated factors in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Globally, about 1.9 million stillbirths occurred at the end of 2021. This scenario has been worst in Sub-Saharan African countries, which contribute about 47% of the total stillbirths. In Ethiopia, though there was a previous study on the rate of stillbirths and its associated factors, it only explored the effect of antenatal care visits and previous stillbirth history. Yet, the meta-effects of other contributing factors such as antepartum hemorrhage, meconium-staining, labor duration, body weight, hypertension, prematurity, and premature rupture of membranes have not been investig

THE SIGNIFICANCE OF INTERLEUKIN-22 AND HOMOCYSTEINE IN THE PROGNOSIS OF PREMATURE ANTEPARTUM RUPTURE OF MEMBRANES IN PREGNANT WOMEN.

BACKGROUND: Premature preterm rupture of membranes (PPROM) is a leading cause of preterm birth and neonatal complications. Identifying reliable biomarkers such as interleukin-22 (IL-22) and homocysteine (Hcy) is critical for early diagnosis, prognosis, and improved maternal-fetal outcomes. OBJECTIVES: To systematically evaluate the prognostic significance of IL-22 and Hcy in predicting PPROM, preterm birth, and related maternal and neonatal outcomes in pregnant women. METHODS: This systematic review (2015-2025) analyzed 10 studies on IL-22 and Hcy in PPROM. Literature was searched in PubMed,

Application of the Surgical APGAR Score to Predict Intensive Care Unit Admission and Post-Operative Outcomes in Cesarean Hysterectomy for Placenta Accreta Spectrum.

Background and Objective: Placenta Accreta Spectrum (PAS) encompasses a continuum of abnormal placentation conditions associated with significant maternal and fetal morbidity. Management of PAS requires coordinated cesarean hysterectomy. Associated morbidities include blood transfusion, coagulopathy, and intensive care unit (ICU) admission. Accurate prediction of ICU admission allows for enhanced multidisciplinary management, coordination of care and utilization of resources. Scoring systems exist in other surgical specialties that can predict the likelihood of ICU admission, but these have no

How to Manage Venous Thromboembolism Risk during Pregnancy in Patients with Inherited Antithrombin Deficiency?

Inherited antithrombin deficiency (ATD) is associated with a high risk of venous thromboembolic complications. Association of ATD with other conditions such as pregnancy obviously increases thromboembolic risk and may require anticoagulant therapy for prevention. Although there are several/heterogenous international guidelines regarding thromboprophylaxis in pregnant patients with ATD, data on anticoagulant prophylaxis in this context are scarce in the literature. Thus, this situation remains a challenge both in the antepartum period and during delivery. Physicians from the French Society of T

Maternal and fetal outcomes in pregnant women with mechanical heart valves: A tertiary center experience.

OBJECTIVE: Patients with mechanical heart valves require lifelong anticoagulation to prevent thromboembolic complications. Pregnancy induces a hypercoagulable state, increasing the risk of those events. Although guidelines recommend anticoagulation with vitamin K antagonists (VKA), recent practice involves replacing VKA with low-molecular weight heparin (LMWH) to reduce fetal complications with an increase in maternal complications. This study aims to analyze outcomes in pregnant patients with mechanical heart valves anticoagulated with VKA or LMWH. METHODS: This was a retrospective case seri

Maternal outcomes among women with intellectual disabilities in comparison with the general population (IDcare).

BACKGROUND: Women with intellectual disabilities face significant barriers to gynecological, reproductive, antenatal, and perinatal care, which may adversely impact maternal and fetal health. Previous research indicates increased risks for gestational diabetes, pre-eclampsia, caesarean birth, preterm birth, and other complications in pregnant women with intellectual disabilities. However, studies on the reproductive health in this group remain scant, and comprehensive research on maternal and fetal health from pregnancy to the postpartum period remain missing. OBJECTIVES: Using high-quality d

Foetal disruptive brain injuries: Diagnosing the underlying pathogenetic mechanisms with cranial ultrasonography.

Antenatal destructive events affecting the central nervous system of the foetus lead to disruptive brain lesions that are often associated with impaired neurodevelopment. The pathogenesis of these lesions encompasses a range of causes, including haemorrhagic, embolic, or other vascular events; exposure to teratogens, such as drugs or substance abuse; congenital brain infections; genetic conditions; and metabolic disorders. Cranial ultrasonography is the first-line imaging modality to diagnose these antepartum brain lesions in the newborn infant; it is often complemented by brain magnetic reson

Pregnancy-related Acute Kidney injury (PrAKI): an observational study of 500 cases from a public hospital in South India.

BACKGROUND: Pregnancy-related acute kidney injury (PrAKI) is a global issue with a significant impact on the society. High maternal and fetal mortality are reported with PrAKI. AKI can occur during antepartum or postpartum periods. In low-middle-income countries, common causes of PrAKI are sepsis and preeclampsia. We undertook this study to analyze the epidemiology and outcomes of pregnancy-related acute kidney injury in our region. METHODS: This is a retrospective study analyzing 500 cases of PrAKI between 2014 and 2024 in a tertiary care referral public hospital. The setting was Gandhi Medi

Pregnancy Outcomes in Women with Low and Ultra-Low Ejection Fraction: A Retrospective Study in a Tertiary Care Center.

The data about pregnancy while having a low ejection fraction are scarce, since pregnancy is not recommended for women with an ejection fraction of less than 30%. There is an increased risk of obstetrical complications and adverse maternal-fetal outcomes. Pregnancy is a rough journey for this group of patients. However, a successful pregnancy can be achieved when cardiac complications are managed during pregnancy. The early recognition of women at risk of cardiovascular events and early referral can optimize the maternal and neonatal outcomes with close collaboration between the maternal-fetal

Obstetric Outcome After Trachelectomy for Cervical Cancer Without Uterine Artery Preservation.

AIM: A trachelectomy is a fertility-preserving surgery that is performed for cervical cancer. Transecting the uterine arteries (UAs) during abdominal radical trachelectomy (ART) or abdominal modified radical trachelectomy (AmRT) has the advantage of simplifying other surgical procedures. However, the effect of UA transection on subsequent pregnancy outcome is unknown. The purpose of this study was to clarify the pregnancy outcomes in post-RT pregnancies in which the UAs were not preserved. METHODS: This was a retrospective cohort study of electronic case records involving pregnant women after

Hemophagocytic Lymphohistiocytosis in Pregnancy: A Systematic Review of Case Reports and Case Series.

AIMS: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening syndrome caused by excessive immune activation, leading to severe inflammation and multi-organ failure. While adult-onset HLH is well documented, pregnancy-related HLH remains under investigated, with limited data on its triggers, presentation, and management. We systematically review cases of pregnancy-related HLH in the literature, focusing on presentations, triggers, utilized treatments, and outcomes. METHODS: A systematic search of PubMed and Embase identified case reports and series describing HLH during pregnancy

Multiple micronutrient supplementation for maternal anemia prevention (MMS-MAP): an individually randomized trial of higher-dose iron (60 mg, 45 mg) compared to low-dose iron (30 mg) in multiple micronutrient supplements in pregnancy.

BACKGROUND: Antenatal multiple micronutrient supplementation (MMS) has been shown to be more effective than iron-folic acid (IFA) alone in reducing adverse pregnancy and birth outcomes. However, there is a concern that MMS containing 30 mg of iron may be less effective in reducing maternal anemia compared to IFA supplements containing 60 mg of iron. This poses a clinical and programmatic dilemma for countries with a high burden of maternal anemia (> 40% prevalence) where the World Health Organization (WHO) recommends using IFA with 60 mg of iron. METHODS/DESIGN: We will conduct an individuall
