Temperature and heat-load exposure effects on preterm births: Insights from a population-based study using distributed lag non-linear models.

OBJECTIVE: Preterm birth presents a substantial health, psychological, and economic burden worldwide. Environmental factors, including temperature fluctuations, are known to influence the risk of preterm birth. This study aimed to investigate the association between high temperatures and heat load and preterm births. METHODS: Using a time-series analysis, we examined data from all births in five Israeli districts between 2010-2016. Meteorological indices were computed for each district. Distributed lag non-linear models (DLNM) incorporating Poisson regression were employed to investigate the

Favoring Elective Hospitalization in Pregnancies With Vasa Previa.

There is international agreement that third-trimester hospitalization is indicated for the one-third of patients with vasa previa who have risk factors for preterm birth. For the remaining two-thirds, it is controversial whether elective hospitalization or outpatient management should be recommended. There are no randomized trials comparing outcomes of the 2 strategies. Observational studies report similar outcomes with elective hospitalization and outpatient management, but are fraught with biases and are underpowered to permit a confident conclusion that outpatient management is safe. Maternal age and perinatal morbidity: a comparative cohort study including advanced maternal age and adolescent pregnancy.

BACKGROUND: Maternal age at childbirth shows a bimodal risk distribution worldwide. While adolescent pregnancy remains common in many areas, the rate of advanced maternal age is rising in high-income countries. Both extremes are associated with adverse outcomes through different pathways. This study tests the hypothesis that adolescent (< 19 years) and older (> 35 years) mothers face higher obstetric and neonatal risks than young adults (19-35 years), regardless of key confounders. METHODS: A retrospective cohort study was conducted from 2019 to 2023 at a tertiary-care center. A total of 19,7

Impact of Short-Term Fluoride Supplementation in Pregnancy on the Maternal Microbiome, Pregnancy, and Neonatal Outcomes: A Pilot Randomized Controlled Trial.

Introduction This report presents secondary, exploratory analyses of a double-blind randomized trial originally designed to determine whether fluoride supplementation during pregnancy reduced preterm birth (PTB). The trial was terminated early because of enrollment difficulties and was not powered for its original clinical endpoint. We therefore examined whether randomized fluoride supplementation was associated with oral or vaginal microbiome composition and descriptively evaluated gestational and neonatal outcomes. Methodology In the original double-blind randomized controlled trial, pregnan

Small Molecules Prescribed On- and Off-Label for Thoracic Cancers During Pregnancy: A Combined VigiBase and Systematic Review Analysis.

BACKGROUND: Thoracic cancers during pregnancy are rare but increasingly encountered because of delayed childbearing and the rising incidence of cancer in younger individuals. Despite guideline recommendations, exposure to targeted small-molecule therapies occurs across all trimesters, but safety data remain limited. OBJECTIVE: To investigate the safety of targeted small-molecule therapies for thoracic cancers during pregnancy by combining VigiBase data with a systematic review. Impact of the 25-Hour Religious Fast of "Yom Kippur" on Pregnancy Outcomes in Low-Risk Singleton Pregnancies in Israel: A Multicenter Retrospective Study.

Evidence regarding the impact of prolonged religious fasting during pregnancy is limited and inconsistent. We aimed to examine the effects of Yom Kippur fasting, a complete 25-h religious fast, on pregnancy outcomes, with a primary focus on preterm delivery, in low-risk singleton pregnancies in Jerusalem, Israel. We performed a multicenter retrospective cohort study of Jewish women aged 18-45 years with uncomplicated singleton pregnancies delivered in our maternity wards between 2006 and 2021. Data were extracted from electronic medical records. A preliminary survey estimated the prevalence of

Progesterone levels on hCG trigger day and neonatal outcomes in fresh embryo transfers: a retrospective threshold analysis of GnRH antagonist versus agonist long protocols.

BACKGROUND: Elevated progesterone levels on the day of human chorionic gonadotropin (hCG) trigger are known to adversely affect pregnancy outcomes in fresh embryo transfer (fET) cycles, but their impact on neonatal outcomes remains poorly understood. This study aimed to investigate the association between progesterone levels on hCG trigger day and neonatal outcomes, and to explore the progesterone threshold, so as to provide reference for clinical decision-making. METHODS: This retrospective cohort study included 2, 201 patients who achieved singleton live births following fET between April 1

Twin pregnancies conceived by assisted reproductive treatments and the risk of long-term offspring pediatric neurological morbidity.

OBJECTIVES: Evidence is conflicting whether twins conceived by assisted reproductive treatment (ART(have higher rate of adverse outcomes than twins conceived spontaneously. Specifically, scarce data exist regarding the long-term neurological implications. Therefore, we studied the association between ART leading to twin pregnancy and short and long- term neurological morbidity of offspring. STUDY DESIGN: A retrospective cohort study including twins born between the years 1991-2021 at a tertiary medical center was conducted. Total and subtypes of neurological morbidity among twins born followi

Short-term obstetrical and neonatal complications in women with endometriosis.

OBJECTIVE: Endometriosis is a chronic gynecological condition that affects women in their reproductive age and could potentially lead to reproductive difficulties. Despite extensive research into the impact of endometriosis on fertility and reproductive health, its association with obstetric complications and short-term neonatal outcomes is still an area of limited understanding with conflicting evidence in published studies. We conducted this study to explore a potential effect of endometriosis on obstetric and neonatal outcomes. METHODS: A retrospective population-based cohort study was con

Double versus single emergency cervical cerclage combined with vaginal progesterone: a multicenter, nonblinded, randomized controlled trial.

BACKGROUND: Emergency cervical cerclage is used to reduce extreme preterm birth in women with advanced cervical dilatation and exposed fetal membranes. Although double-level cerclage has been proposed to provide greater mechanical support than single-level cerclage, evidence comparing these techniques in emergency settings is limited. OBJECTIVE: To compare the effectiveness of emergency double-level versus single-level cerclage, both combined with adjunctive vaginal progesterone, antibiotics and indomethacin in preventing delivery before 34 weeks of gestation. Investigation into biomarkers to predict preterm birth (INSIGHT): a longitudinal UK pregnancy cohort for spontaneous preterm birth research.

BACKGROUND: Spontaneous preterm birth (sPTB) remains a leading cause of perinatal morbidity and mortality worldwide. Progress in prediction and prevention has been limited by the biological heterogeneity of sPTB and a lack of longitudinal pregnancy cohorts integrating detailed clinical phenotyping with serial biological sampling. The INSIGHT study was established to enable mechanistic and biomarker-driven investigation of sPTB, with a focus on inflammatory pathways associated with cervical shortening. METHODS: INSIGHT is a prospective, multicentre UK pregnancy cohort that was active between 2

Associations of short-term changes in air pollution with spontaneous preterm birth: A time-stratified case-crossover analysis.

Spontaneous preterm birth (sPTB) is a leading cause of neonatal morbidity in the United States. Genetics incompletely explains sPTB risk and environmental contributions remain poorly understood. We analyzed associations of short-term fine particulate matter (PM2.5) exposure with sPTB in two datasets: 109,735 sPTBs in Michigan (2003-2020) and 5321 sPTBs in metropolitan Philadelphia (2008-2020). We used a time-stratified case-crossover design and daily high-resolution PM2.5 estimates to evaluate exposures in the week prior to sPTB using three complementary approaches: (1) nested daily lag terms;
