Obstetric outcomes in second-generation and migrant mothers: a cohort study in the Emilia-Romagna Region, Italy.

PURPOSE: The second generation (people born in the host country to foreign parents) has grown significantly in Italy. While migration is known to impact maternal and neonatal health negatively, evidence on outcomes for second-generation mothers (SGMs) is limited. This study aimed to describe the socio-demographic characteristics and obstetric outcomes of SGMs, comparing them with first-generation and native mothers in an Italian region. METHODS: All deliveries in the Emilia-Romagna Birth Registry (2012-2023) were linked with the 15th Population Census (2011); this allowed the definition of mi

Critical exposure windows for wildfire PM2.5 during pregnancy and preterm birth in Brazil.

Wildfire smoke is an increasingly important air-pollution exposure, yet evidence on critical windows of susceptibility for pregnancy outcomes remains limited in Brazil. We evaluated the association between wildfire-related fine particulate matter (PM2.5) and preterm birth, focusing on lag-specific effects across pregnancy weeks and heterogeneity by sociodemographic groups. We conducted a nationwide ecological time-series study using weekly municipality-level panels in Brazil from 2002 to 2022. The outcome was weekly counts of preterm births, modeled with a quasi-Poisson regression and the log

Pregnancy-related acute myocardial infarction after treatment with ritodrine hydrochloride: A case report.

Pregnancy-associated myocardial infarction (PAMI) is a rare but potentially life-threatening condition for the mother and fetus. Pregnancy also confers a three- to fourfold increased risk of MI compared with the non-pregnant state. However, recognizing PAMI promptly in obstetric settings remains challenging. A 34-year-old Japanese primigravida at 30 weeks of gestation had been receiving ritodrine for preterm labor. On hospital day 2, approximately 4 h after discontinuation of tocolytics, repeat electrocardiography (ECG) indicated new ST-segment elevation in leads I, aVL, and V3-V6 without ches

Telehealth Delivery of the Homeostasis-Enrichment-Plasticity Approach for Premature Infants With Developmental Risks: Exploratory Feasibility Study.

BACKGROUND: Preterm delivery is an increasing worldwide health concern linked to increased neurodevelopmental risks. Early intervention is crucial for harnessing neuroplasticity to enhance developmental and functional performance outcomes; however, access to early intervention is frequently hindered by logistical, financial, and labor constraints. The Homeostasis-Enrichment-Plasticity (HEP) Approach is a family-centered early intervention model based on enriched environments, designed to improve infants' sensory-motor, cognitive, and socio-emotional development. OBJECTIVE: This study aimed to

Dietary fiber supplementation mitigates gestational diabetes risk and preterm birth via gut microbiota modulation: a randomized controlled trial.

BACKGROUND: Gestational Diabetes Mellitus (GDM) poses severe health risks to mother and child, yet effective, non-invasive preventive strategies remain elusive. While the gut microbiota is known to influence glucose metabolism, its potential as a therapeutic target and predictive biomarker in high-risk pregnancies is underexplored. This study investigated whether soluble dietary fiber supplementation could remodel the gut microbiome to prevent GDM and improve pregnancy outcomes. METHODS: We performed a single-center, randomized controlled trial with 98 pregnant women at elevated risk for GDM.

Clinician perspectives on speculum exams in PPROM: a survey study.

OBJECTIVE: The American College of Obstetricians and Gynecologists recommends speculum exams rather than digital exams for cervical evaluation in preterm prelabor rupture of membranes (PPROM). However, making management decisions based on speculum exams has challenges. This study aims to investigate OB/GYN clinicians' self-reported adherence to and comfort with these recommendations. METHODS: We conducted a web-based survey distributed via email to labor and delivery clinicians at nine hospitals in 2024. The survey inquired about respondents' management of a hypothetical patient admitted with

Neighbourhood socioeconomic disadvantage and the risk of preterm delivery.

OBJECTIVE: Preterm birth is a major cause of childhood mortality and morbidity, but the aetiology of preterm delivery remains incompletely understood. We sought to examine whether cumulative exposure to neighbourhood socioeconomic disadvantage is associated with preterm delivery risk. METHODS: The association between neighbourhood socioeconomic disadvantage over the 20 years preceding delivery and risk of preterm delivery was assessed in a population-based cohort of 11 979 deliveries in Southwest Finland in 2008-2010. The statistical analyses were adjusted for a large number of potential indi
