Identifying Infants <32 Weeks' Gestation at Low Risk of Early-Onset Sepsis: A 10-Year Retrospective Study from Western Australia.

UNLABELLED: <p>Introduction: Preterm infants are commonly treated with antibiotics on admission to the neonatal unit as part of routine care. We aimed to identify infants <32 weeks' gestation at low risk of early-onset sepsis (EOS) in whom antibiotics could be safely withheld. METHODS: This retrospective cohort study included infants <32 weeks' gestation admitted between January 2012 and June 2022. Data were extracted from electronic databases. Low risk for EOS (LR) was defined as caesarean section delivery, rupture of membranes <1 h prior to birth, no preterm labour and no features of matern

Risk Factors and Pregnancy Outcomes Associated with Insulin Addition to Metformin Therapy in Gestational Diabetes.

OBJECTIVE: Metformin is increasingly being used to treat gestational diabetes mellitus (GDM). But pharmacotherapy with metformin frequently requires insulin supplementation to achieve glucose control, and this remains poorly characterized. We identified factors associated with insulin supplementation of metformin versus receipt of metformin alone, and then examined whether these two groups differed in the frequency of adverse pregnancy outcomes (APOs) among individuals with GDM.We conducted a retrospective analysis of a U.S. GDM care program from 2018 to 2021, which first initiated metformin.

Association between depression during pregnancy and preterm birth: Results from population cohorts and mouse experimental models.

BACKGROUND: Depression is a prevalent psychological challenge during pregnancy, with established links to adverse outcomes like preterm birth (PTB) globally. However, epidemiological data from China's multiethnic regions are scarce, and experimental evidence supporting a causal relationship remains limited. This study aimed to investigate the association between prenatal depressive symptoms and the risk of PTB in a cohort from Yunnan, China, and to provide supportive evidence using a mouse model of depression. METHODS: We recruited 1,466 women during their first-trimester routine visits at Qu

Racial and ethnic disparities in early cervical length screening and spontaneous preterm birth.

BACKGROUND: Racial and ethnic disparities in preterm birth (PTB) persist despite advances in prevention. Cervical length screening and cerclage placement are established strategies for reducing recurrent spontaneous PTB (sPTB), yet it remains unclear whether access to and use of these interventions differ by race and ethnicity or contribute to observed disparities in outcomes. OBJECTIVE: To evaluate racial and ethnic differences in early cervical length screening, cerclage placement, and PTB outcomes among patients with a history of sPTB. STUDY DESIGN: This retrospective cohort study include

Risk factors and diagnostic challenges in a nullipara with spontaneous uterine rupture during vaginal delivery.

OBJECTIVE: To present a rare case of uterine rupture after vaginal delivery under epidural anesthesia in a nulliparous woman without prior uterine surgery. CASE REPORT: A 38-year-old primiparous woman with a history of infertility conceived through in vitro fertilization. At 39 weeks of gestation, she was admitted due to spontaneous premature rupture of membranes. Oxytocin was administered for labor augmentation, and fundal pressure was applied for maternal exhaustion, leading to the delivery of a healthy infant. Secondary laceration of perineum was found but no cervical laceration was detect

Rapid clearance of bacteria from maternal bloodstream after delivery in pregnancies complicated by preterm pre-labor rupture of the membranes.

Preterm pre-labor rupture of membranes (PPROM) increases maternal and neonatal sepsis risk, yet its association with maternal microbial translocation and systemic inflammation remains unclear. We investigated whether PPROM is linked to microbial DNA in maternal blood (MB) and inflammatory responses around delivery. In 66 PPROM patients (median GA: 32 weeks), MB was collected pre-delivery and within 1 h postpartum. Fetal membranes (FM) and placental tissues were sampled immediately after delivery. Bacterial load and diversity were analyzed via 16 S rDNA qPCR and sequencing. Maternal cytokines w

Risk and burden of adverse birth outcomes associated with flood exposure in China: A 4.4 million mother-infant cohort study.

The risks and attributable burden of adverse birth outcomes from flooding remain unclear. Here, we analyzed a nationwide cohort of over 4.4 million mother-infant pairs in China, matching a spatiotemporal flood database with multiple buffer zones centered on maternal delivery hospitals to assess the health risks of multi-window flood exposure before and during pregnancy. We found that flood exposure during the three months preconception and the first trimester significantly increased the risks of preterm birth (PTB), low birth weight (LBW), small for gestational age (SGA), and term low birth we

The role of housing instability in the association between ambient temperature and preterm birth.

BACKGROUND: Preterm birth is a leading cause of neonatal death and long-term health issues. Both environmental and social stressors, such as heat exposure and housing stability, may contribute to adverse birth outcomes, yet their joint effects are not well studied, especially in the Korean context. Therefore, we aimed to investigate the independent and joint associations of ambient temperature and housing instability with preterm birth. METHODS: This study analyzed eight years (2015–2022) of birth outcomes among women, derived from nationwide, register-based census data covering 100% of the po

[Significance of Early 75-g OGTT for the Detection of Glucose Disorders in High-Risk Women after Late Miscarriage or Preterm Birth].

INTRODUCTION: This study aimed to determine the proportion of pathological results in the early 75-g OGTT among women with a history of late miscarriage or preterm birth after prophylactic cerclage. The effects of early-diagnosed gestational diabetes on pregnancy outcome, neonatal parameters, preconceptional BMI, and mode of delivery were also analyzed. METHODS: In 140 pregnant women, a primary cerclage and a 75-g OGTT were performed between 16 and 21 weeks of gestation. Analysis was based on OGTT results (physiological vs. pathological). The primary endpoint was pregnancy outcome; secondary

Investigation of the association between maternal serum ferritin levels and preterm delivery: A systematic review and meta-analyses.

BACKGROUND: Preterm delivery (PTD) has been a significant cause of neonatal morbidity and mortality. Maternal serum ferritin, a biomarker of both iron stores and systemic inflammation, has shown inconsistent associations with PTD risk in individual studies. OBJECTIVE: This systematic review and meta-analysis aimed to clarify the relationship between maternal serum ferritin levels and the risk of PTD. METHODS: Following the PRISMA guidelines, we searched PubMed, Scopus, Web of Science, and Google Scholar up to August 2025. Twenty-four observational studies with more than 10000 participants we

The association of first and second-trimester serum biomarkers with adverse perinatal outcomes in late-preterm and term deliveries: a retrospective cohort study.

BACKGROUND: Maternal serum biomarkers, primarily introduced for aneuploidy screening, have also been investigated as predictors of adverse pregnancy outcomes. However, their value in late-preterm and term pregnancies remains unclear. METHODS: In this retrospective cohort study, 592 singleton pregnancies screened at a tertiary perinatology center between 2020 and 2025 were analyzed. Participants underwent either the first-trimester combined test (n = 461) or the second-trimester triple test (n = 131). Serum biomarker multiples of the median (MoM) were categorized as low (< 0.5), normal (0.5–2.0

The vaginal bacteriome of pregnant Rwandan and Kenyan women, unique regional genera associations with preterm birth.

BACKGROUND: Preterm birth, defined as gestational duration less than 37 weeks, often results in lifelong adverse health impacts for children. East African women have some of the highest rates of preterm birth globally. Previous studies have suggested that changes in the vaginal bacteriome may be associated with increased risk for preterm birth. The absence of Lactobacilli dominated vaginal flora and increased alpha and beta diversity have been associated with preterm birth in other contexts, primarily from US-based studies with African-American women. Few studies have been conducted with women
