A 47-year-old healthy man developed disseminated coccidioidomycosis involving his mediastinal and cervical lymph nodes that was complicated by pericardial involvement despite continued fluconazole for primary pulmonary infection. Although he subsequently received an aggressive antifungal regimen with voriconazole and liposomal amphotericin B, he did not clinically improve until he underwent major debridement and drainage, highlighting the need for source-control procedures in some cases of severe disseminated coccidioidomycosis.

BACKGROUNDDisseminated coccidioidomycosis (DCM) is an often fatal and otherwise intractable condition requiring lifelong antifungal treatment. We have previously shown that a deranged polarization of CD4+ T cells toward a Th2 phenotype can exist in the context of DCM. Here, we studied a large population to determine the frequency of abnormal Th2 skewing of CD4+ T cells in patients with coccidioidomycosis and to identify underlying genetic mechanisms supporting this phenotype.METHODSWe collected PBMCs from 204 patients with coccidioidomycosis, including 96 patients with disseminated disease. Coccidioidomycosis is a dimorphic fungal infection endemic to the southwestern United States and parts of the Americas. Pregnant individuals have an increased risk of disseminated disease, yet the diagnosis and management of neonatal coccidioidomycosis remain poorly defined. Neonatal coccidioidomycosis is rare but associated with significant morbidity and mortality. Transmission may occur via transplacental spread, aspiration of infected decidua at delivery, or early postnatal exposure. Diagnosis is challenging due to overlapping signs with prematurity and limitations of current serologic, ant

Background: Disseminated coccidioidomycosis (DCM) often requires prolonged antifungal therapy (AFT). Real-world data on AFT duration in DCM are limited. We evaluated time to AFT discontinuation among patients with DCM in the United States clinical practice. Methods: This retrospective, longitudinal study used STATinMED data (2016-2024). Patients had ≥1 International Classification of Diseases, Tenth Revision (ICD-10) code for DCM (B38.3, B38.4, B38.7, B38.81) during January 2017-December 2023, ≥1 claim for a triazole or amphotericin B within 21 days of the DCM diagnosis (index date), and conti

BACKGROUND: Coccidioidomycosis is the most prevalent endemic mycosis in the United States. Diagnosis of coccidioidomycosis can be challenging due to limitations of conventional diagnostics. Detection of fungal plasma cell-free DNA (cfDNA) is a novel non-invasive testing modality for the diagnosis of invasive fungal disease. In this study, we characterized the performance of Coccidioides plasma cfDNA PCR.
METHODS: A retrospective study was conducted in patients with suspected coccidioidomycosis who underwent plasma cfDNA PCR testing and conventional fungal testing for C. immitis and C. posadas

BACKGROUNDCoccidioidomycosis ranges from self-limiting uncomplicated Valley fever (UVF) in most cases to life-threatening disseminated coccidioidomycosis (DCM) in rare individuals. A few patterns of immunologic deficits allowing for dissemination have been identified, although the specific defects in most individuals with DCM remain undefined. We hypothesized that chronic antigen exposure in DCM engenders a state of T cell exhaustion.METHODSFrom a cohort of over 300 individuals with confirmed diagnoses of coccidioidomycosis, circulating T cell phenotypes were characterized via flow cytometry a

Olorofim, an orotomide antifungal with orphan designation for coccidioidomycosis, has not been previously linked to drug-induced liver injury. We present a case of drug-induced liver injury in a 67-year-old man treated with olorofim for refractory disseminated coccidioidomycosis. The patient experienced an acute, almost 30-fold increase in liver enzymes in a hepatocellular pattern after administration of olorofim, followed by a rapid downtrend over the next few weeks. Rechallenge at a reduced dose did not reproduce the transaminase elevations, consistent with a probable Roussel-Uclaf Causality

A 72-year-old Caucasian woman from the Southwestern United States presented with chronic right ear pain, hearing loss, and progressive hoarseness. Prior treatments, including balloon sinuplasty and tympanostomy tube placement, failed to resolve her symptoms. Imaging revealed right mastoid opacification, while flexible laryngoscopy demonstrated erythematous and polypoid changes of the right larynx. Pathology from biopsies of both the middle ear and larynx revealed spherules consistent with Coccidioides infection. She was treated with systemic fluconazole and underwent tympanomastoidectomy with

Coccidioidomycosis, also known as Valley Fever, is a fungal disease endemic to the Americas that kills hundreds annually, yet the host factors that lead to increased risk of life-threatening dissemination of coccidioidomycosis remain poorly understood. We assembled the largest comprehensively sequenced coccidioidomycosis cohort to date, comprising 795 individuals with laboratory confirmed coccidioidomycosis and clinical disease severity phenotyping, many with paired whole blood genomic and transcriptomic data. Individuals with greater than 50% African genetic ancestry have increased risk for d

Coccidioidomycosis (CM) is primarily a pulmonary disease but can also present with significant extrapulmonary disease. Extrapulmonary manifestations vary across organ systems, but most frequently presents as infection of the bone, brain and skin. Due to the variety of clinical presentations in disseminated disease, CM can be underrecognized, resulting in a diagnostic conundrum. We report a case of a non-immunocompromised patient initially presenting for weight loss with imaging findings mimicking peritoneal carcinomatosis, ultimately found to have CM with peritoneal involvement.

BACKGROUND: Immunosuppression is a recognized risk factor for severe and disseminated coccidioidomycosis; however, data describing outcomes in patients with inflammatory bowel disease (IBD) receiving advanced therapy, including biologics and small molecules, remain limited. We aim to describe the prevalence, clinical presentation, treatment patterns, and outcomes of coccidioidomycosis in IBD patients receiving advanced therapy at an endemic tertiary care center.
METHODS: We performed a retrospective observational study of adult patients with IBD at Mayo Clinic Arizona diagnosed with coccidioi

BACKGROUND: Coccidioidomycosis is a fungal infection endemic to the southwestern United States that rarely involves the central nervous system (CNS) as a brain abscess or subdural empyema. These uncommon manifestations can mimic neoplastic or inflammatory lesions, making diagnosis particularly challenging.
CASE DESCRIPTION: We report two cases of disseminated coccidioidomycosis with atypical CNS presentations and imaging findings. In the first case, a patient with pulmonary sarcoidosis presented with a right parietal lesion and subdural empyema initially suspected to represent neurosarcoidosi

A Th2-favored phenotype can predispose individuals to symptomatic and disseminated coccidioidomycosis. Dupilumab, which impedes Th2 immunity, is sometimes used adjunctively for disseminated coccidioidomycosis. To determine whether dupilumab alters the clinical course of coccidioidomycosis, we retrospectively reviewed records of 1,561 patients who received dupilumab; 42 had coccidioidomycosis diagnosed from October 6, 2018, through July 16, 2025 (Mayo Clinic [Phoenix, Arizona]). Eleven patients received dupilumab before having symptomatic coccidioidomycosis, and 3 received dupilumab before asym

OBJECTIVES: Coccidioidomycosis (cocci) is a rare fungal disease with clinical manifestations ranging from asymptomatic to life-threatening severe pulmonary or disseminated infection. It is endemic to parts of the southwestern USA, Mexico and South America. Tourism and migration have led to cocci occurrences across Europe among those arriving from endemic areas. The pan-European epidemiology of cocci has not previously been systematically reviewed.
METHODS: A systematic literature review was conducted with no limits for publication year, article type and language. We initially identified 797 r

Disseminated cutaneous coccidioidomycosis (DCC) is an uncommon manifestation of Coccidioides infection resulting from hematogenous spread to the skin. While recurrence after treatment discontinuation has been reported in 17 to 50 percent of cases, associated frequency, timing, and risk factors are not well defined. We conducted a retrospective review of biopsy-proven or probable DCC cases between January 2008 and March 2024, and investigated for evidence of recurrence. Demographic, clinical, and treatment data were abstracted, including antifungal regimen, adherence, immune status, and coccidi
