Associations of short-term changes in air pollution with spontaneous preterm birth: A time-stratified case-crossover analysis.

Spontaneous preterm birth (sPTB) is a leading cause of neonatal morbidity in the United States. Genetics incompletely explains sPTB risk and environmental contributions remain poorly understood. We analyzed associations of short-term fine particulate matter (PM2.5) exposure with sPTB in two datasets: 109,735 sPTBs in Michigan (2003-2020) and 5321 sPTBs in metropolitan Philadelphia (2008-2020). We used a time-stratified case-crossover design and daily high-resolution PM2.5 estimates to evaluate exposures in the week prior to sPTB using three complementary approaches: (1) nested daily lag terms;

Multiple Sclerosis During Pregnancy and the Postpartum Period: Diagnostic and Multidisciplinary Challenges in Two Cases.

BACKGROUND: Pregnancy may complicate the recognition and evaluation of new neurological symptoms suggestive of a central nervous system demyelinating disease. Diagnostic uncertainty may arise when clinical progression, neuroimaging findings, and supportive investigations are incomplete or inconclusive. CASE PRESENTATION: We report two pregnant women with progressive focal neurological symptoms and radiological findings suggestive of a demyelinating disease. Case 1 involved a 27-year-old woman with a history of autoimmune spondyloarthritis who developed dizziness, gait instability, asymmetric

Prenatal Diagnosis and Obstetrical Challenges of a Massive Fetal Ascites Affecting One Fetus in a Dichorionic Diamniotic Twin Pregnancy: A Case Report.

Fetal ascites is a rare prenatal condition; it may result from a wide range of underlying etiologies, including structural anomalies, genetic disorders, congenital infections, and metabolic diseases. Despite considerable advances in prenatal investigations, etiology remains unidentified in some cases. Massive fetal ascites may also complicate obstetric management by distorting fetal biometric measurements and influencing delivery planning. We report the case of a 34-year-old woman with a spontaneously conceived dichorionic diamniotic twin pregnancy. At 24 weeks of gestation, massive fetal asci

Temporal trends and recurrence risks of spontaneous and provider-initiated preterm birth in Canada - a population-based study.

BACKGROUND: Rates of preterm birth in Canada are increasing; however, national reports typically only include aggregate preterm birth rates and do not provide information on sub-types of preterm birth. This study aimed to describe temporal trends in preterm premature rupture of membranes (PPROM), spontaneous preterm labor, and provider-initiated preterm birth in Canada. METHODS: Hospitalization data on livebirths in Canada (excluding Quebec) between 2004/05-2024/25 were obtained from the Canadian Institute of Health Information Discharge Abstract Database. Cochran-Armitage tests for trend wer

The AerVia supraglottic airway device versus the endotracheal tube in neonatal delivery room resuscitation: a prospective cohort study.

OBJECTIVE: To evaluate a supraglottic airway device (SAD) as an alternative to the endotracheal tube (ETT) in infants requiring resuscitation in the delivery room. STUDY DESIGN: Newborns requiring resuscitation beyond face mask ventilation due to inadequate ventilation or persistent bradycardia at Al Bashir Hospital in Amman, Jordan, were resuscitated using the AerVia SAD (n = 73) and compared with a historic cohort resuscitated by the ETT (n = 80). RESULTS: AerVia and ETT cohorts had similar mean gestational age (32.0 ± 2.8 vs. 32.7 ± 2.3 weeks, p = 0.13) and birth weight (1.63 ± 0.44 vs. 1

Neonatal Gastric Perforation Temporally Associated with Nasogastric Tube Insertion: A Case Report.

BACKGROUND: Neonatal gastric perforation (NGP) is a rare but potentially life-threatening condition associated with substantial morbidity and mortality. Approximately half of reported cases are idiopathic or spontaneous, while the remainder are attributed to identifiable causes, including congenital gastric wall defects, hypoxic-ischemic injury, and iatrogenic trauma related to nasogastric tube (NGT) insertion, particularly in premature and low-birth-weight infants. Early diagnosis and prompt surgical intervention are critical for improving outcomes. We report a case of gastric perforation tem

Pregnancy Outcomes of Cervical Cerclage in a Tertiary Care Centre: A Retrospective Study.

BACKGROUND: Cervical insufficiency is an important cause of recurrent second-trimester pregnancy loss and spontaneous preterm delivery. Cervical cerclage is a surgery done to strengthen the cervix to prolong the pregnancy in women who need it. The result of a cerclage can vary based on the obstetric history, gestational age at the time of the cerclage, cervical length, pregnancy type, and timing of the removal. AIM: To compare the obstetric and demographic features, timing of cervical cerclage, gestational age at delivery, and maternal and neonatal outcomes of women who underwent cervical cer

Stabilization of cervical stiffness associated with cervical pessary placement in women at risk for preterm birth.

PURPOSE: Shortening of the cervix is a predictor of spontaneous preterm birth (PTB). It is assumed that a cervical pessary reduces mechanical stress on the internal os and consequently decreases the risk of premature delivery. The primary aim of this study was to compare longitudinal changes in cervical stiffness between women with cervical shortening (a cervical length ≤ 25 mm according to the ISUOG guidelines) treated with a cervical pessary and healthy controls. METHODS: A total of 89 pregnant participants were analysed in this prospective observational study. Among them, 30 received a cer

The Impact of Offspring Sex on Pregnancy and Neonatal Outcomes in Individuals with Pregestational Diabetes.

Pregestational diabetes mellitus (PGDM) is increasing in prevalence among pregnant individuals and is associated with adverse outcomes. Prior work suggests that offspring sex influences placental responses and may impact risk for adverse outcomes. We sought to assess the impact of offspring sex on adverse pregnancy and neonatal outcomes in individuals with PGDM.We conducted a retrospective cohort study of 503 pregnant individuals with PGDM and known offspring sex with care at a major urban academic center between 1998 and 2016. We assessed two co-primary composite outcomes: (1) a composite adv
