Early gestational prediction of spontaneous preterm birth using a validated three-protein serum biomarker panel.

BACKGROUND: Spontaneous preterm birth (sPTB) remains a major contributor to neonatal morbidity and mortality, with limited reliable early prediction tools. Existing biomarkers, such as the insulin-like growth factor-binding protein 4 (IBP4) to sex hormone-binding globulin (SHBG) ratio, offer modest predictive performance and are restricted to mid-gestation use (18-20 weeks), limiting their utility for timely intervention. We aimed to develop and validate a novel serological test based on early-gestational sampling to predict the risk of sPTB. METHODS: We conducted a meta-analysis of 18 placen

Outcome after Selective early treatment for Closure of patent ductus ARteriosus in preterm babies, a multicentre, masked, randomised placebo-controlled parallel group trial (Baby-OSCAR trial).

BACKGROUND: In extremely preterm babies, born before 28 weeks' gestation, a large (≥ 1.5 mm in diameter) patent ductus arteriosus present beyond 3 days of age is associated with higher mortality and morbidity than infants without a patent ductus arteriosus. The cyclooxygenase inhibitor ibuprofen may be used to treat patent ductus arteriosus. Whether selective early treatment of a large patent ductus arteriosus with ibuprofen improves health and developmental outcomes is not known. METHODS: We conducted a multicentre, randomised, double-blind, placebo-controlled trial evaluating early treatmen

The Influence of the Vaginal Microbiome on the Prediction and Prevention of Preterm Birth.

BACKGROUND: Spontaneous preterm labour that leads to preterm birth is known to be associated with vaginal dysbiosis, particularly bacterial vaginosis, and this may explain why progress has been slow in the last few decades. Bacterial vaginosis was considered enigmatic with unknown aetiology, difficulty in diagnosis, different response to treatment, be that persistence or recurrence, and different phenotypic outcomes. METHODOLOGY: A narrative review. RESULTS: New information from the Human Microbiome Project using molecular-based, culture-independent technology has added important new knowled

An early LH surge may jeopardize ART outcomes of patients with PCOS and its predictive models.

ABSTRACT: This study aimed to investigate whether an early LH surge during controlled ovarian stimulation (COS) affects pregnancy outcomes in patients with polycystic ovary syndrome (PCOS) undergoing assisted reproductive technology (ART) and to explore methods for identifying high-risk patients for early intervention. In a retrospective case-control analysis of 786 PCOS cycles, an early LH surge was found to significantly increase the spontaneous abortion rate and reduce the live birth rate in in vitro fertilization and multiple embryo transfer cycles, while also correlating with a higher pre

Outpatient management of women with preterm prelabour rupture of the membranes: A retrospective multicentre cohort study.

Preterm prelabour rupture of the membranes poses maternal and perinatal risks including maternal sepsis, placental abruption, preterm labour and its sequelae, perinatal mental health complications, and mortality. Optimum management is not well understood. Most recent UK guidance suggested outpatient management may be appropriate, but guidance was not given on frequency or mode of monitoring, and outcomes following this are not well understood. This study aimed to determine monitoring practices and outcomes of women whose pregnancies complicated by preterm prelabour rupture of the membrane who

The association between breastfeeding and prevalence of metabolic syndrome in women with a previous major pregnancy complication.

INTRODUCTION: Maternal complications of pregnancy, including preeclampsia, gestational diabetes mellitus, spontaneous preterm birth and placental abruption are individually associated with an increased risk of premature heart disease. Breastfeeding may reduce early postpartum cardiometabolic risk in this group. This study aimed the association between breastfeeding and the prevalence of cardiovascular risk factors and metabolic syndrome (MetS) in the early postpartum period amongst women with previous pregnancy complication. METHODS: We conducted a cross-sectional analysis of 524 women who at

Geographic variation in spontaneous preterm birth incidence at the province and municipal level in the Netherlands: A population registry based study.

Preterm birth (PTB), defined as birth before 37 completed weeks of gestation, is a major public health concern with long-term health consequences. Risk factors for (spontaneous) PTB (sPTB) include medical, lifestyle, demographic, environmental, and obstetric factors. Low socioeconomic status (SES) and late initiation of antenatal care (ANC) are known contributors, but their municipal-level impact remains understudied. This study explores the distribution of sPTB across Dutch provinces and municipalities and assesses the role of SES and ANC in regional disparities to inform targeted, place-base

Maternal hyperuricemia and adverse maternal-fetal outcomes: a systematic review and meta-analysis of observational studies.

BACKGROUND: Maternal serum uric acid (SUA) has been suggested as a biomarker for adverse pregnancy outcomes, but findings remain inconsistent. This systematic review and meta-analysis evaluated the association between elevated maternal SUA levels and key obstetric and neonatal outcomes. METHODS: A comprehensive search of PubMed, Scopus, Web of Science, and the Cochrane Library was conducted through December 2024. Observational studies reporting associations between high maternal SUA levels and pregnancy outcomes were included. Data were pooled using a random-effects model as odds ratios (ORs)

Toward a point of care approach for intra-amniotic infection or early delivery using minimally invasive prediction models in women with preterm labor.

BACKGROUND: Among women with preterm labor and intact membranes, those with intra-amniotic infection or inflammation represent the group with the highest risk of spontaneous delivery and worse adverse outcomes. Identification of this group requires amniocentesis, which is perceived as too invasive by both patients and physicians. OBJECTIVE: This study aimed to develop a minimally invasive prediction model for intra-amniotic infection or early delivery to better stratify patients (low or high risk) and rationalize the use of amniocentesis, limiting the indications to the highest-risk group of

Minimally invasive surfactant administration in extremely preterm infants <28 weeks gestation with spontaneous breathing: a retrospective quality improvement study on reducing invasive ventilation burden.

BACKGROUND: Invasive surfactant delivery via endotracheal intubation increases exposure to invasive mechanical ventilation (IMV) in extremely preterm infants (EPIs, <28 weeks). This quality improvement (QI) initiative aimed to evaluate whether minimally invasive surfactant administration (MISA) was associated with a reduced IMV burden in EPIs. METHODS: A single-center retrospective QI study (2013-2024) included 115 infants (24-27⁺⁶ weeks) with spontaneous breathing diagnosed with respiratory distress syndrome (RDS). Controls (2013-2019, n = 55) received surfactant via intubation; the MISA gro

Exploring fetal cells in maternal blood in relation to rupture of the fetal membranes.

INTRODUCTION: Cellular derangements contributing to fetal membrane damage precede spontaneous rupture at term and are even more pronounced in preterm prelabor rupture of membranes. We hypothesized that such pathophysiological processes lead to shedding of fetal membrane-derived cells into maternal blood. This study aimed to identify fetal membrane cells in maternal blood and to examine whether their concentration correlates with membrane rupture. METHODS: Blood samples (30 mL) were collected from pregnant women. Using antibodies targeting proteins highly expressed in the fetal membranes, feta
