Society for Maternal-Fetal Medicine Consult Series #76: Cancer in pregnancy.

Approximately one in 1000 pregnancies is complicated by the diagnosis of cancer each year, and the incidence of cancer among reproductive-age individuals is increasing. Management of a pregnant person with cancer can be complex and warrants a multidisciplinary approach to care. Recent data have demonstrated reassuring outcomes for pregnant persons and their offspring after treatment of many types of cancer in pregnancy. Treatment of cancer in pregnancy must be individualized based on the specific type and stage of cancer, gestational age at diagnosis, and the patient's desire to continue the p

The FADS insertion-deletion polymorphism drives the arachidonic acid/dihomo-gamma-linolenic acid ratio in preterm infants and may contribute to patency of ductus arteriosus - a pilot study.

BACKGROUND: The FADS Indel insertion-deletion polymorphism (rs66698963) modulates fatty acid desaturase 1 (FADS1), which converts di-homo-γ-linolenic acid (DGLA) to arachidonic acid (ARA). Adults homozygous for the I allele (I/I) have up to 84% higher ARA/DGLA ratios than D homozygotes (D/D), with heterozygotes intermediate. Whether this relationship extends to preterm neonates is unknown. ARA is the precursor to prostaglandin E2 (PGE2), a key mediator of ductal patency. Elevated ARA synthesis may contribute to patent ductus arteriosus (PDA). HYPOTHESIS: I/I infants have greater ARA/DGLA rati

Temporal Trends in Preterm Birth Associated With Hypertensive Disorders of Pregnancy in Victoria, Australia: A Population-Based Interrupted Time-Series Study.

OBJECTIVES: Hypertensive disorders of pregnancy (HDP) are a leading reason for medically indicated preterm birth (PTB), yet their contribution to population-level PTB trends is poorly understood. We examined temporal trends in PTB overall and HDP-associated PTB in Victoria over an 11-year period spanning the COVID-19 pandemic, and distinguished changes in HDP prevalence from changes in preterm delivery among affected pregnancies. STUDY TYPE: Population-based interrupted time-series study using seasonal autoregressive integrated moving average (SARIMA) models, with July 2018 (launch of a natio

Predictive value of uterocervical angle combined with phosphorylated insulin-like growth factor binding protein-1 testing for preterm delivery and adverse neonatal outcomes.

BACKGROUND: Preterm birth is a leading cause of perinatal morbidity. While transvaginal cervical length (CL) is the standard screening tool for symptomatic women, its low specificity frequently leads to overtreatment and unnecessary hospital admissions. Although the uterocervical angle (UCA) and phosphorylated insulin-like growth factor binding protein-1 (phIGFBP-1) are recognized as strong independent predictors, the clinical value of integrating these biomechanical and biochemical markers into a single triage model remains unclear. OBJECTIVE: To investigate the predictive accuracy of a comb

Comparison between cervicovaginal fluid interleukin-6 and fetal fibronectin in combination with cervical ultrasound characteristics for the prediction of spontaneous preterm delivery in threatened preterm labor.

BACKGROUND: About 80% of patients presenting with threatened preterm labor do not deliver preterm. Currently, cervical length and cervicovaginal fluid fetal fibronectin are tools used to predict spontaneous preterm birth. However, fetal fibronectin testing has been discontinued worldwide. OBJECTIVES: This study aimed to compare the predictive performance of cervicovaginal fluid interleukin-6 (IL-6) and fetal fibronectin, combined with cervical characteristics (cervical length, sliding sign, and elastography) and other maternal characteristics, for predicting spontaneous preterm delivery in pa

Rescue Cervical Cerclage in a Twin Pregnancy With Short Cervical Length or Cervical Dilatation: A Case Report.

Preterm birth is a leading cause of neonatal morbidity and mortality worldwide, with the risk being substantially higher in multiple pregnancies. Twin gestations are associated with several maternal and foetal risk factors that contribute to spontaneous preterm birth. Although cervical cerclage is an established intervention for cervical insufficiency in singleton pregnancies, its role in preventing preterm birth in twin pregnancies remains controversial, and its use continues to be the subject of ongoing research. This case report details the management of a woman with a twin pregnancy who pr

Spoken language and risk of nulliparous, term, singleton, vertex cesarean births in California, 2016 to 2021.

BACKGROUND: Non-English-speaking individuals have noted disparities in health outcomes in the United States, but the role of language in obstetric settings is not well characterized. OBJECTIVE: We examined the association between primary spoken (preferred) language and the risk of nulliparous, term, singleton vertex cesarean delivery (CD). STUDY DESIGN: We conducted a retrospective cohort study of nulliparous, term, singleton vertex deliveries in California (2016-2021) using birth certificates linked to hospital discharge records. The primary outcome was the risk of CD. Poisson log-linear re

Cerclage for singleton pregnancies with an extremely short cervix (≤10 mm) and no history of spontaneous preterm birth: A multisite observational study.

INTRODUCTION: There is uncertainty about the benefits of cerclage in patients with transvaginal cervical length (TVCL) ≤10 mm and no prior spontaneous preterm birth. Our aim was to assess whether cervical cerclage in these patients was associated with prolonged pregnancy latency. METHODS: This was an observational study of asymptomatic singleton pregnancies without a history of spontaneous preterm birth with extremely short cervix (TVCL ≤10 mm) in the second trimester. Exposure and outcome data were extracted manually from medical charts by obstetric providers, with all outcomes occurring pri

Predictive Potential of Plasma Acute Phase- and Adhesion-Related Proteins and Progranulin for Intra-Amniotic Infection/Inflammation and Spontaneous Preterm Birth in Women With Preterm Labor.

OBJECTIVE: To determine whether plasma acute phase-, adhesion-related, and multifunctional proteins, alone or in combination with ultrasound and traditional blood tests, can predict the microbial invasion of the amniotic cavity and/or intra-amniotic inflammation (MIAC/IAI) and spontaneous preterm birth (SPTB) in women with preterm labor (PTL). STUDY DESIGN: A retrospective cohort study was performed on 249 singleton pregnant women with PTL (at 23+0 to 34+0 weeks) who underwent transabdominal amniocentesis. Plasma samples were obtained at amniocentesis. MIAC/IAI was characterized by a positive

The cytokine prokineticin-1 in preterm labor with and without intra-amniotic inflammation.

OBJECTIVES: Prokineticin 1 (PROK1), known as endocrine gland-derived endothelial growth factor (EG-VEGF), is a key regulator of pregnancy, playing various roles from implantation to birth. We have shown that maternal serum concentrations of PROK1 are highest during the first trimester and are elevated in pregnancy complications such as spontaneous preterm birth (sPTB). Additionally, PROK1 is described as being largely expressed by the chorion in fetal membranes, suggesting its presence in amniotic fluid. Despite these findings, no information is available about PROK1 concentrations in the amni

Intra-amniotic infection: diagnosis, nomenclature, clinical significance, management, and microbiologic tools used for the diagnosis.

SUMMARYIntra-amniotic infection is the main cause of spontaneous preterm birth and adverse maternal-fetal outcomes; therefore, rapid, robust, and accurate diagnosis remains a clinical priority. Conventional microbiological techniques, especially culture-based methods, are limited by long turnaround times and the inability to detect fastidious or unculturable organisms. This review summarizes the diagnosis, nomenclature, clinical significance, management, and laboratory approaches for diagnosing intra-amniotic infection. Targeted nucleic acid amplification methods, including species-specific po

AI-assisted point of care ultrasound and an innovative vaginal speculum: a proposed combined approach to reducing spontaneous preterm birth.

The Bouquet Speculum™ is an innovative vaginal speculum developed to enhance patient comfort, improve cervical visualization, and facilitate ease of use for healthcare providers during gynecologic examinations and procedures. Studies show AI-assisted ultrasound can identify women at risk for spontaneous preterm birth (s-PTB) more accurately than standard ultrasound. Cervical cerclage, often performed to delay delivery, is sometimes difficult with standard 2-bladed speculums. The Bouquet Speculum™ combined with AI-assisted ultrasound offers the potential for improved outcomes and reduction of m

Pathologic abnormalities of deep placentation in the great obstetrical syndromes: Implications for understanding the pathophysiology, risk assessment in early pregnancy, and personalized prevention.

The concept of the Great Obstetrical Syndromes was introduced to explain the unique nature of obstetrical disease, which differs fundamentally from disorders in other areas of medicine. These syndromes, including preeclampsia, fetal growth restriction, fetal death, and spontaneous preterm birth, represent clinical endpoints rather than single diseases and share defining characteristics: they arise from multiple etiologies, have a prolonged subclinical phase, involve the fetus as an active participant, are adaptive in nature, and result from complex genetic and environmental interactions betwee

Causal inference of multiple interrelated exposures: Contributions of ischemic placental disease to preterm delivery.

Ischemic placental disease (IPD) involves three obstetrical complications - preeclampsia, placental abruption, and fetal growth restriction - and is implicated in up to 55% of early deliveries. Despite substantial overlap in their etiology and epidemiologic risk profiles, existing studies have assessed their effects independently. Using data from the Consortium on Safe Labor (2002-2008; n = 203, 990), we examined the composite and separate effects of the IPD conditions on spontaneous and indicated preterm delivery (sPTD and iPTD). We systematically specified joint IPD probabilities, correspond

Mendelian randomization study: investigating the causal impact of Covid-19 on adverse pregnancy outcomes.

BACKGROUND: Due to limitations in observational studies, the link between COVID-19 and adverse pregnancy outcomes (APOs) remains inconclusive. This study uses two-sample Mendelian randomization (MR) analyses to assess COVID-19's causal effects on APO traits. METHODS: We applied inverse variance weighting (IVW), MR-Egger, weighted median, weighted mode, and simple mode to thoroughly evaluate the effects of COVID-19 infection, hospitalization, and critical status on eight APO traits. RESULTS: Our findings indicate that COVID-19 infection is associated with a decreased risk of eclampsia (OR: 0.

Maternal and obstetric factors associated with successful vaginal cerclage treatment: a prospective cohort study.

OBJECTIVE: To identify maternal and obstetric factors associated with cerclage effectiveness in preventing preterm birth (PTB) < 32 weeks, in order to subsequently develop a prediction model. METHODS: Patients with prior spontaneous PTB < 34 weeks and a singleton pregnancy who received a history-based or ultrasound-indicated cerclage were included. Main outcome was PTB < 32 weeks. Maternal and obstetric factors were included in multivariate logistic regression to compose weighted prediction models for PTB < 32 weeks, separately for both indications. Individual risk scores were calculated usin
