Mode of delivery among pregnant people with obesity undergoing labor induction in the late preterm period.

INTRODUCTION: Pregnant people with obesity are more likely than those without obesity to undergo induction of labor and to require unplanned cesarean delivery during induction at term. Whether they are more likely to undergo unplanned cesarean delivery during preterm induction of labor is unknown. This study examines the induction of labor success among people with obesity undergoing indicated late preterm induction of labor. METHODS: This is a secondary analysis of a multicenter, randomized trial of betamethasone versus placebo among pregnancies at risk for late preterm delivery, defined as

Effectiveness of Hysteroscopic Surgery for Intrauterine Lesions on Pregnancy Rates in Patients With Primary Infertility in Bahrain.

Background Infertility poses a substantial challenge in the realm of reproductive health, impacting numerous couples globally. Intrauterine abnormalities like polyps, submucosal fibroids, and uterine septa can impede the process of successful implantation and placentation, thereby contributing to infertility issues and miscarriages. Hysteroscopy has emerged as a promising therapeutic approach for addressing these intrauterine lesions. Our objective is to assess the efficacy of hysteroscopic surgery in treating intrauterine lesions and its impact on pregnancy rates among individuals experiencin

Implementation of the Robson classification for caesarean sections in Israel: a 10-year cross-sectional study.

BACKGROUND: In recent decades, caesarean section (CS) rates have dramatically increased; the reasons for this trend are multifactorial and not fully understood. This continuing trend has raised public health concerns regarding higher maternal and perinatal risks, high costs, healthcare efficiency, and inequality of services. We aimed to characterize and evaluate Israel's CS rate by applying the Robson classification system. METHODS: This was a national retrospective cross-sectional study. Data from 1,061,786 live births were collected from electronic medical records of women admitted for deli

Obstetric views on reduced preterm birth rates early in the COVID-19 pandemic.

A significant decrease in preterm births occurred during the first few months of the COVID-19 lockdown across the U.S. and in other high-income countries, particularly among medically induced births, or artificially initiated births. This unexpected phenomenon raised speculation that the reduction was influenced by changes in obstetric care or hospital policy shaped by the pandemic. Reductions in spontaneous preterm births also occurred and may have been related to reductions in stress, fewer infectious exposures, and less daily exertions due to the ability for some to work at home in early mo

Evidence for Maternal Autoantibodies in the Pathogenesis of Preterm Birth.

Complications from preterm birth are the leading cause of global mortality in children under age five1,2. Spontaneous preterm labor is the most common cause of preterm delivery and is associated with a breakdown of maternal-fetal tolerance3-5. However, the current understanding of the role of autoantibodies in this process has been limited to a handful of examples of pathogenic antibodies that occur with pregnancy complications6-14. Here, we employ proteome-wide autoantibody profiling via phage display immunoprecipitation and sequencing (PhIP-seq) to identify autoreactivities associated with p

Neonatal Outcomes of Umbilical Cord Milking, Early Cord Clamping and Delayed Cord Clamping in Term Infants: A Randomised Controlled Trial.

Introduction Optimal umbilical cord management at birth plays a crucial role in neonatal outcomes. While delayed cord clamping (DCC) has been associated with improved neonatal haematologic and hemodynamic status, early cord clamping (ECC) remains widely practised. Umbilical cord milking (UCM) has emerged as an alternative to DCC, potentially offering similar benefits while reducing the delay in neonatal resuscitation. This study aims to compare the effects of UCM, ECC, and DCC on neonatal outcomes in term infants born between 37 and 42 weeks of gestation. Methods A total of 225 pregnant women

Reproductive outcomes of pregnancy after breast cancer: an updated systematic review and meta-analysis.

BACKGROUND: Reproductive outcomes following pregnancy in premenopausal women with breast cancer (BC) remain contentious, and few meta-analyses have adequately addressed these questions. This systematic review and meta-analysis aimed to provide the most up-to-date and comprehensive evidence on the subject. METHODS: Ten electronic databases were searched in September 2024 using the terms "Breast Neoplasms" AND "Pregnancy OR Fertilization OR Parturition OR Fertility OR Obstetrics". Key inclusion criteria focused on reproductive outcomes in premenopausal women with BC compared to healthy populati

Evaluation of Women Presenting With Bleeding in the First Trimester of Pregnancy in a Tertiary Hospital: A Cohort Study.

Introduction First-trimester bleeding ranges from harmless spotting to signs of serious complications such as miscarriage, ectopic pregnancy, and molar pregnancy. It is associated with adverse maternal outcomes, including gestational hypertension and preeclampsia, as well as neonatal risks like preterm birth and low birth weight. Understanding the risk factors and outcomes can improve the monitoring and management of affected pregnancies. Methodology This prospective cohort study included 110 pregnant women presenting with first-trimester vaginal bleeding at a tertiary hospital. Participants w

The Prevalence of Perineal Tears Among Women Having Spontaneous Vaginal Births with Intrapartum Fever.

Perineal tears are common during vaginal delivery and are associated with significant maternal morbidity. While chorioamnionitis and intrapartum fever are known to affect labor dynamics and perineal tissue integrity, their relationship with perineal trauma in spontaneous vaginal deliveries has not been established. This study aimed to evaluate the prevalence of perineal tears among women with intrapartum fever who delivered spontaneously. This retrospective cohort study included women who underwent spontaneous vaginal delivery during 2013-2021 in Israel. The study group comprised women diagnos

Full-term delivery after antepartum vaginal myomectomy for prolapsed myoma causing uterus distortion: A case report and literature review.

OBJECTIVE: Prolapsed cervical myoma with uterine distortion during pregnancy are rare. CASE PRESENTATION: A 45-year-old woman at 30 weeks of gestation presented with a prolapsed mass and vaginal fluid leakage. A 9 × 8 × 8 cm3 prolapsed mass was observed outside the vaginal orifice, and preterm premature rupture of membranes (PPROM) was suspected. Transabdominal ultrasonography revealed an elongated cervix. Magnetic resonance imaging (MRI) indicated a cervical myoma causing mechanical pulling of the amniotic membrane, resulting in cervical elongation. The patient underwent antepartum vaginal m

The CASPAR study protocol. Can cervical stiffness predict successful vaginal delivery after induction of labour? a feasibility, cohort study.

BACKGROUND: Induction of labour (IOL) is a common obstetric intervention in the UK, affecting up to 33% of deliveries. IOL aims to achieve a vaginal delivery prior to spontaneous onset of labour to prevent harm from ongoing pregnancy complications and is known to prevent stillbirths and reduce neonatal intensive care unit admissions. However, IOL doesn't come without risk and overall, 20% of mothers having an induction will still require a caesarean section birth and in primiparous mothers this rate is even higher. There is no reliable predictive bedside tool available in clinical practice to

Characterization of cervical fluid Ureaplasma species in pregnant women with spontaneous preterm delivery.

Ureaplasma spp., commonly identified in the vagina/cervix of pregnant women with spontaneous preterm delivery, are the most frequently detected microorganisms in amniotic fluid. To date, 14 U. spp. serotypes have been characterized; however, modern molecular biology methods can distinguish different U. spp genotypes. Considering these factors, a knowledge gap exists regarding the association between U. spp. genotypes and the risk of the ascension of U. spp. from the cervix to the amniotic cavity. To fill this gap, an expanded multilocus sequence-typing scheme of U. spp. was performed to assess

Postnatal healing of cesarean scar: an ultrasound study.

BACKGROUND: Impaired healing of cesarean delivery scars results in long-term complications, including scar niche formation, which may adversely impact future pregnancies and lead to gynecological symptoms such as postmenstrual spotting, dysmenorrhea, and chronic pelvic pain. Moreover, there is increasing evidence that a cesarean delivery scar located close to or within the cervix is associated with an increased risk of subsequent spontaneous preterm birth. The factors influencing cesarean birth scar location and healing remain poorly understood. OBJECTIVE: We explored the impact of antenatal,

[Predictive tools for preterm birth in women with threatened preterm labor].

Preterm birth is a major cause of perinatal morbidity and mortality. In France, about 7% of births occur before 37 weeks of gestation, nearly two-thirds of which are spontaneous. Preterm labor (PTL), defined as the combination of uterine contractions and cervical changes between 22 and 37 weeks, represents the most frequent condition leading to prematurity. However, half of the women hospitalized for PTL ultimately deliver at term, highlighting the need for reliable tools to identify those truly at risk. Transvaginal ultrasound measurement of cervical length is currently the reference exam. It

Recurrent Necrotizing Enterocolitis in Late Preterm and Full-Term Babies: A Case Report and Literature Review.

Necrotizing enterocolitis (NEC) is a serious inflammatory condition of the intestines that primarily affects premature infants, though it can also occur in full-term infants. Diagnosing and managing recurrent NEC in full-term infants can be particularly challenging, with most requiring surgical intervention upon presentation if medical treatment is insufficient. An approach that emphasizes active medical management while minimizing excessive bowel resection tends to result in better postoperative outcomes, highlighting the need for increased awareness within the healthcare community regarding

Prenatal Diagnosis of a Large Oropharyngeal Teratoma and Airway Management with EXIT: A Case Report.

We present a rare case of a large fetal oropharyngeal teratoma (epignathus) diagnosed during the third trimester and managed successfully with a planned ex utero intrapartum treatment (EXIT) procedure followed by neonatal surgical resection. A 30-year-old pregnant woman was referred to our department at 29 weeks of gestation due to polyhydramnios and the detection of an oropharyngeal mass on ultrasound. Fetal magnetic resonance imaging confirmed a 5×5 cm heterogeneous mass filling the oral cavity, raising concern for airway obstruction at birth. At 32 weeks, spontaneous pre-term labor necessit
