The Pathophysiology of Spontaneous Preterm Birth: Emerging Mechanisms Reviewed by the Preterm Birth International Collaborative.

Around 10% of global births are preterm (before 37 weeks of gestation), posing a significant challenge to maternal and neonatal health. Preterm infants face an increased risk of mortality and long-term health complications, impacting their survival and development across all life stages. Despite decades (~ 80 years) of research, effective methods to predict and prevent idiopathic or spontaneous preterm birth remain limited. Therefore, a deeper understanding of the pathophysiology of spontaneous preterm birth is warranted. This review explores some aspects of recent progress in unravelling the

The correlation between vaginal microbiome dysbiosis and recurrent spontaneous abortion.

In recent years, the role of the vaginal microbiota in pregnancy maintenance has garnered increasing attention. Extensive research indicates that vaginal dysbiosis is closely associated with various adverse pregnancy outcomes, including ectopic pregnancy, miscarriage, preterm birth, and premature rupture of membranes. Recurrent spontaneous abortion (RSA) affects approximately 1-5% of women of childbearing age and has a complex etiology, with infection implicated in about 4% of cases. This systematic review examines the compositional characteristics of the vaginal microbiota, its dynamic change

Massive subchorionic thrombohaematoma (Breus' mole) causing postpartum haemorrhage: a case series and management strategy.

BACKGROUND: Breus' mole (massive subchorionic thrombohaematoma, MST) is a rare placental disorder that can cause catastrophic postpartum haemorrhage due to acute rupture of a high-tension haematoma. However, the mechanism of this haemorrhage remains insufficiently understood in clinical practice. CASE PRESENTATION: Case 1: A 31-year-old woman, G4P1, was admitted at 26⁺¹ weeks of gestation for intrauterine foetal death. She had a history of chronic hypertension and obesity. Serial ultrasound revealed progressive placental thickening (39→87→97 mm) with a massive subchorionic haematoma. Her haem

ABOVE: cerclage after caesarean: protocol for a randomised controlled trial to assess the optimal preventative management for preterm birth secondary to caesarean section damage.

BACKGROUND: There is emerging evidence that caesarean section in labour is associated with an increased risk of recurrent mid-trimester loss (MTL) and spontaneous preterm birth (sPTB) in subsequent pregnancies. This is likely due to inadvertent damage to the cervical tissue at the time of caesarean section. Transvaginal cerclage (TVC) has been demonstrated to be less successful in this high-risk cohort of women. However, transabdominal cerclage (TAC) may be more effective as the suture is placed above the level of cervical caesarean damage. A TAC can be placed before or during early pregnancy,

Mitochondrial dysfunction and cellular senescence drive accelerated gestational aging in spontaneous preterm birth: a narrative review.

Spontaneous preterm birth (sPTB) is a leading cause of neonatal mortality and long-term morbidity worldwide, affecting approximately 15 million infants annually. Despite advances in obstetric care, its incidence has remained largely unchanged, reflecting an incomplete understanding of the biological mechanisms governing the timing of parturition. While infection and inflammation have traditionally dominated etiological models, these alone do not fully explain the heterogeneity in disease onset, progression, and outcomes. Emerging evidence suggests that sPTB may represent a state of accelerated

Spontaneous rupture of degenerating leiomyoma in the third trimester complicated by preterm delivery and recurrent necrosis requiring interval myomectomy: A case report.

Uterine leiomyomas are a common benign neoplasm of the female reproductive tract. They are often asymptomatic and can be present during pregnancy without complication. When leiomyomas outgrow their blood supply, the tissue undergoes degeneration. This process is most often seen in pregnancy. A rare complication of this process is spontaneous rupture, which is a surgical emergency often resulting in intra-abdominal leakage, bleeding, and peritonitis. This report describes the case of a 36-year-old White woman (G1P0) at 35 weeks of gestation with a known enlarging anterior fundal leiomyoma with

Molecular Mechanism and Pathways of Spontaneous Preterm Birth in Different Gestational Tissues: A Systematic Review of Transcriptome Studies.

This systematic review assessed transcriptomic evidence on the molecular mechanisms underlying spontaneous preterm birth (sPTB). Major electronic databases were searched from inception to October 2025. Eligible studies examined RNA transcriptomic profiles from maternal pregnancy-related tissues or biofluids in spontaneous preterm labor (sPTL) or preterm prelabor rupture of membranes (PPROM), while indicated or iatrogenic preterm births were excluded. Two reviewers independently screened studies, extracted differentially expressed genes (DEGs), and assessed study quality. DEGs were summarized b

Intraoperative Discovery of Placenta Percreta With Bladder Invasion During Emergency Cesarean Section Following Cardiac Arrest: A Case Report.

Placenta percreta, the most severe form of placenta accreta spectrum (PAS), is a rare but life-threatening obstetric condition in which abnormal placental invasion extends beyond the uterine myometrium into adjacent organs, most frequently the urinary bladder. Preoperative imaging may suggest the diagnosis yet substantially underestimate the true depth of invasion, a discrepancy that carries potentially catastrophic consequences. We report the case of a 34-year-old woman with two prior cesarean sections who presented at 25 weeks of gestation in active preterm labor with a dichorionic-diamnioti

Severe malaria in pregnancy in the United States: A case report and management considerations.

Malaria in pregnancy is associated with substantial maternal and fetal morbidity, yet pregnancy-specific guidance remains limited, particularly in non-endemic settings where low clinical suspicion, non-specific symptoms, and testing barriers may delay diagnosis. This report presents a case of severe third-trimester Plasmodium falciparum malaria and reviews management considerations. A nulliparous patient at 36 weeks and 4 days of gestation presented to a U.S. community hospital with fever, headache, and abdominal discomfort, after recent travel to Ghana. Initial evaluation showed maternal feve

Postpartum visit attendance after spontaneous and medically indicated preterm birth.

INTRODUCTION: Individuals with preterm birth (PTB) are less likely to attend postpartum visits compared to those with term birth but are more likely to have conditions such as hypertensive disorders. PTB can be classified as spontaneous PTB (sPTB) (premature rupture of membranes or preterm labor) and medically indicated PTB (mPTB) (clinician-initiated due to maternal or fetal conditions). We compared postpartum visit attendance after sPTB and mPTB to term birth. We hypothesized that sPTB and mPTB would be associated with increased risk of not attending postpartum visits compared to term, but r

Vaginal progesterone and risk of preterm birth in asymptomatic women with short cervix diagnosed after 24 weeks of gestation.

BACKGROUND: The optimal management of asymptomatic women with a short cervix first identified after 24 weeks of gestation remains uncertain. Although vaginal progesterone is effective for preventing preterm birth when cervical shortening is detected in the mid-trimester, its role in women with incidentally detected late cervical shortening is unclear. OBJECTIVE: To evaluate whether vaginal progesterone treatment is associated with a reduced risk of spontaneous preterm birth among asymptomatic women with a short cervix first identified after 24 weeks of gestation. STUDY DESIGN: This retrospec
