One hundred women for whom instrumental delivery had to be performed due to fetal asphyxia or uterine inertia were randomized to a vacuum extraction with the conventional Malmström instrument or the Silastic cup of Kobayashi. With the Silastic cup, extraction time was shorter and injuries to the scalp were less common. On the other hand, extraction with the Silastic cup failed more often, especially in occiput posterior position. There were no differences in Apgar scores between the two groups.

Uterine contraction can be brought about at any time with prostaglandins. This effect is exploited in the treatment of postpartal atonia. Prostaglandin was administered intravenously to 21 women for postpartal atonia. In 19 women the bleeding subsequently stopped. There were no side effects of the treatment. These results are presented and discussed with reference to other data in the literature.

In a prospective study 100 consecutive Saudi patients having their tenth or subsequent deliveries in a general hospital were observed. Unlike in some other reports fetal malpresentations, maternal anaemia or hypertension were few. However there was a high incidence of uterine inertia and primary cesarean sections. Early perinatal mortality rate was 50 per 1000. High parity is still a hazard to both mother and baby. Health education and family planning should be improved.

Sixteen women delivered by forceps and 20 women delivered by vacuum extraction (VE) owing to secondary uterine inertia were compared with a control group of 11 women who gave birth spontaneously. From 1978 to 1982, 70 cases of emergency hysterectomy for obstetric hemorrhage were performed at Los Angeles County/University of Southern California Women's Hospital. Intrauterine constraint is responsible for a number of structural defects of the craniofacial and peripheral skeleton including plagiocephaly, micrognathia, congenital hip dislocation, and talipes equinovarus. This report describes five infants with serious craniofacial alterations, all attributable to intrauterine constraint. A retrospective, off-line study of the relationship of the area under the intrauterine pressure waveform (Area) to amplitude was performed on contractions from patients in normal labor and hypocontractile labor before and during oxytocin therapy. Reported are 304 patients with whom programmed childbirths were introduced, between 1976 and 1978. A control group was made up of 100 women in labour who had been hospitalised for spontaneous onset of labour but then given slow drip oxytocin infusions for secondary uterine inertia. The parameters tested (time of labour, FHF behaviour, pH of umbilical artery, Apgar score, weight, length, and perinatal mortality) did not reveal any aggravated risk to mother and child. Programmed childbirth may be recommended as another important preventive step towards lower infantile morbidity.
