DNAJC17 deficiency: A novel inborn error of immunity with TNF-driven autoinflammation.

BACKGROUND: Inborn Errors of Immunity (IEI) encompass a broad spectrum of monogenic disorders with variable clinical presentations, including recurrent infections, autoimmunity, and systemic inflammation. Loss-of-function mutations in DNAJC17 have been previously linked to retinal dystrophy and hypogammaglobulinemia; however, the complete immunological phenotype and underlying autoinflammatory mechanisms remain poorly characterized. METHODS: We conducted a comprehensive clinical, molecular, and immunological investigation of three affected individuals from two unrelated consanguineous familie

Sudden Release of a Fixed Contracture as a Clue to Occult Humeral Fracture in a Neonate With Arthrogryposis Multiplex Congenita.

A neonate with multiple congenital contractures consistent with arthrogryposis multiplex congenita was found to have an initially clinically occult humeral shaft fracture immediately after axillary cleansing. The first abnormal sign was the sudden release of a previously fixed upper-limb posture, which initially resembled improvement in contracture, before swelling, erythema, or deformity became apparent. Radiography confirmed a spiral fracture of the midshaft of the left humerus, which healed with conservative treatment. This case highlights that an abrupt apparent improvement in a fixed cont

Spectrum of Upper Gastrointestinal Endoscopic Findings in Patients With End-Stage Renal Disease Undergoing Maintenance Hemodialysis: A Single-Center Experience From Pakistan.

INTRODUCTION: Symptoms related to the upper gastrointestinal (GI) tract and bleeding complications are common presentations in patients with end-stage renal disease (ESRD) receiving maintenance hemodialysis. Various factors, including uremia, platelet dysfunction, use of anticoagulation during hemodialysis, anemia, impaired GI motility, diabetes mellitus, and polypharmacy, can lead to a wide range of abnormalities. Local literature from Pakistan is scant. Therefore, the aim was to assess the spectrum of upper GI endoscopic findings in hemodialysis patients with ESRD. METHODS: After approval f

Arrhythmic risk stratification in psoriatic arthritis: a retrospective, electrogram-based comparative analysis.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory systemic disease that is associated with cardiovascular comorbidities, particularly with an increased risk of arrhythmias. Despite the evidence, a structured population-based risk stratification that relates ECG parameters to clinical characteristics in PsA is still lacking. METHODS: Data from patients diagnosed with PsA who presented to our centre between 2014 and 2022 as well as from an age-matched and sex-matched control group of cardiovascular patients were collected. A variety of established ECG parameters associated with at

A Novel F5 Gene Variant Associated With Early-Life Ischemic Brain Injury in a Child With a Familial Coagulation Disorder.

Perinatal and early-life arterial ischemic stroke are important causes of hemiparesis and lifelong neurodevelopmental morbidity. While many cases are considered idiopathic, inherited thrombophilia is increasingly implicated in selected patients. We report an infant who developed right-sided motor impairment recognized at nine months of age. He was born at term after an uncomplicated pregnancy, although the neonatal period was complicated by transient respiratory distress and a small atrial septal defect requiring brief neonatal intensive care admission. Examination demonstrated right spastic h

Using Atrial Cardiomyopathy Metrics to Characterize Atrial Fibrillation in Endurance Athletes.

BACKGROUND: Atrial fibrillation (AF) is more prevalent in endurance athletes (EAs) than in the general population, but identifying athletes at greatest risk remains challenging. OBJECTIVES: This study sought to develop an AF risk assessment tool from associations between AF and electrophysiologic, imaging, and genetic markers of atrial cardiomyopathy in EAs. METHODS: EAs and nonathletic control subjects (NAs) underwent comprehensive phenotyping with the use of cardiopulmonary exercise testing, echocardiography, 12-lead electrocardiography (ECG), Holter monitoring for premature atrial contrac

Cognitive phenotypes in adults with temporal lobe epilepsy from South Africa - A contribution to the international classification of cognitive disorders in epilepsy (IC-CoDE).

OBJECTIVES: The International Classification of Cognitive Disorders in Epilepsy (IC-CoDE) provides a systematic framework to identify cognitive phenotypes in patients with various epilepsy syndromes and assess their consistency across diverse cultures and regions. Evidence-based consensus on cognitive profiles will enhance diagnostic processes and foster collaborative epilepsy research. This study evaluated the applicability of the IC-CoDE in adults with temporal lobe epilepsy (TLE) in South Africa. METHODS: Neuropsychology data from 301 adults with TLE were analysed and the IC-CoDE taxonomy

Activity-dependent neuroprotective protein (ADNP) derived peptide NAP prevents musculoskeletal abnormalities in ovariectomized mice.

Estrogen is an essential hormone that critically impacts bodily and brain functions, supporting learning, memory, and motor activities. A decrease in estrogen levels is associated with cognitive decline and motor dysfunction, such as muscle weakness. While conventional hormone replacement treatments (HRT) exist, those have limitations and potentially severe side effects. NAP (davunetide) is the smallest neuroprotective peptide site of activity-dependent neuroprotective protein (ADNP), a master regulator of cognition, essential for brain formation. It is known that NAP restores ADNP activity in

A Young Child With Global Developmental Delay and Epilepsy Secondary to a Novel Variant of KBG Syndrome.

KBG syndrome is a rare neurodevelopmental disorder caused by changes in the ANKRD11 gene (ankyrin repeat domain-containing protein 11). It is typically inherited in an autosomal dominant pattern. This syndrome is characterized by a distinct constellation of clinical findings, including short stature, distinctive craniofacial manifestations, skeletal abnormalities, and abnormal patterns of hair growth. One of the most important features for diagnosing KBG syndrome is the large size of the upper central incisors, which often facilitates the diagnosis between the ages of seven and eight years aft

Instrumented Three-Dimensional Gait Analysis in an Adult With KIF1A-Associated Neurological Disorder: A Case Report.

KIF1A-associated neurological disorder (KAND) encompasses recessive and dominant variants with wide clinical variability. Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia (HSP), frequently accompanied by peripheral neuropathy, cerebellar ataxia, and cognitive impairment. Instrumented three-dimensional (3D) gait analysis is underutilized in this population, and comparative data between KAND and pure HSP remain limited. We performed a comprehensive instrumented 3D gait analysis in a 19-year-old woman with KAND caused by a

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Perfectionism and objective physical health in older adults: a mediational analysis of psychological distress.

BACKGROUND: Research has suggested that the relationship between perfectionism and perceived health may be mediated by psychological distress. However, there is a lack of studies that demonstrate this relationship in older adults and that also use objective physical health measures (such as physical fitness). This study aimed to investigate whether the associations between perfectionistic dimensions and physical fitness in older adults are mediated by psychological distress. PARTICIPANTS AND PROCEDURE: Older adults (N = 121; 90 women) were evaluated for perfectionism (perfectionistic striving

Arrhythmogenic Right Ventricular Cardiomyopathy: The Importance of Biventricular Strain in Risk-Stratification.

Despite arrhythmogenic right ventricular cardiomyopathy (ARVC) being predominantly a right ventricular (RV) disease, concomitant left ventricular (LV) involvement has been recognized. ARVC is diagnosed by the RV-centric 2010 Task Force Criteria(TFC) using routine echocardiography, but previous studies have suggested that strain imaging may be more sensitive to detect RV and LV dysfunction. No data however are available regarding the additional value of combining biventricular strain for risk stratification. This study aims to assess the prognostic value of both LV global longitudinal strain (G

The Additional Use of Spirometry in Evaluating Chronic Cough in Children. "May the Force Be With You".

Spirometry is the most common and straightforward examination following the mandatory initial steps of personal history and physical examination when assessing chronic and/or recurrent lung symptoms in children, especially cough or specific conditions that can impact lung function. When dealing with a chronic cough (lasting more than four weeks), it is not uncommon to find that no specific clues regarding the cause of the cough can be deduced from the patient's history alone. Moreover, clinical examinations can be quite normal without any abnormal lung sounds. In the next step (spirometry), su

A Systematic Review of Pediatric Abusive Head Trauma: What a Surgeon Needs to Know Before Using a Knife.

Abusive head trauma (AHT) has been defined as an injury to the skull or intracranial contents of a baby or young child (age less than five years) due to inflicted blunt impact and/or forceful shaking. It has a substantial impact, such as blindness and severe developmental delay in the short and long run. We searched PubMed, Cochrane, Embase, and Google Scholar databases for articles published between January 2001 and December 2021. Two independent reviewers extracted the data from the included studies. The Joanna Briggs Institute (JBI) critical appraisal tool was used to assess the bias in the
