Morphogenesis and morphometry of brain folding patterns across species.

Evolutionary adaptations associated with the formation of a folded cortex in many mammalian brains are thought to be a critical specialization associated with higher cognitive function. The dramatic surface expansion and highly convoluted folding of the cortex during early development is a theme with variations that suggest the need for a comparative study of cortical gyrification. Here, we use a combination of physical experiments using gels, computational morphogenesis, and geometric morphometrics to study the folding of brains across different species. Starting with magnetic resonance image

Urodynamic outcomes and prognostic determinants following endovascular treatment of spinal dural arteriovenous fistulas.

INTRODUCTION: The prognosis of untreated spinal dural arteriovenous fistulas (SDAVFs) is unfavorable. Current outcome scales used to assess the effect of surgery or endovascular treatment (EVT) rely largely on patient-reported symptoms and may underestimate actual impairment. Moreover, prognostic factors remain debated and conclusions in the literature are inconsistent. RESEARCH QUESTION: The aim was to quantify urological outcomes after SDAVF embolization using specialized urodynamic testing, compare these objective findings with subjective outcomes derived from traditional scales, and ident

Effect of primary uterine inertia on blood gases and acid-base parameters in a chronically cannulated bovine foetus during calving: A case report.

Foetal arterial catheterisation is an effective tool for conducting longitudinal in vivo studies on foetal blood gases and acid-base fluctuations. These surgical techniques have made obtaining daily blood samples under aseptic conditions feasible to determine arterial and/or venous blood gases and acid-base parameters. The present study aimed to elucidate the changes in arterial blood gases and acid-base variables obtained from a catheterised foetus during the final days preceding and during the calving process. It focused specifically on a case of primary uterine inertia and two typical cases

Placenta Previa Major: Prenatal Diagnosis and Uterus-Preserving Surgical Management in a Case Study From the Kingdom of Bahrain.

Placenta previa major is a significant concern in maternal healthcare due to its potential to cause postpartum hemorrhage (PPH), which can lead to morbidity and mortality following childbirth. PPH is generally defined as the loss of 500 ml or more of blood from the genital tract within 24 hours of the birth of a baby. PPH can result from various factors, including uterine atony, retained placental tissue, trauma, and placenta previa. Placenta previa, where the placenta is abnormally positioned over or near the cervical opening, is particularly concerning due to its significant risks for both m

Laparoscopic Repair of a Vesicovaginal Fistula After Total Abdominal Hysterectomy.

Several complications may arise postoperatively in gynecological surgeries including vesicovaginal fistula (VVF). VVF is an abnormal communication between the vagina and bladder which causes leakage of urine from the bladder into the vagina. It is associated with many etiologies such as sexual intercourse at an early age, prolonged/obstructed labor, malignancies, radiation therapy, and post-pelvic surgeries. Here, we report the case of a 50-year-old woman who developed a VVF after a total abdominal hysterectomy.

Enterocervical Fistula Six Weeks After Laparoscopic Mesh Sacral Colposuspension: A Case Report.

Laparoscopic colposuspension with mesh is a common procedure performed to treat apical pelvic organ prolapse refractory to non-surgical interventions. Mesh complications involving a fistula are an uncommon event that typically occurs months or years later. We report a case complicated by enterocervical fistula that occurred six weeks after mesh placement. This was the case of a 67-year-old female with no surgical history who underwent a laparoscopic supracervical hysterectomy, bilateral salpingo-oopherectomy, laparoscopic colposuspension with mesh, and midurethral sling for treatment of stage

Ectrodactyly, Cleft Lip/Palate, and Urinary Anomalies With a Tumor Protein p63 (TP63) Mutation: A Case Report and Literature Review.

Heterozygous mutations in the tumor protein p63 (TP63) gene underlie a spectrum of autosomal dominant syndromes, affecting ectodermal, limb, and orofacial development. We report an infant born with ectrodactyly (split-hand/foot malformation), cleft lip and palate, and a solitary kidney with hydronephrosis. Genetic testing revealed a heterozygous TP63 missense variant, c.740A>G (p.His247Arg), inherited from his affected father, confirming Ectrodactyly-Ectodermal Dysplasia-Clefting (EEC) syndrome. This case highlighted the clinical significance of the TP63 p.His247Arg mutation, previously report

Muscle force imbalances predict clubfoot recurrence risk: A musculoskeletal modelling approach.

BACKGROUND AND OBJECTIVES: Idiopathic clubfoot, primarily treated with the Ponseti method, recurs in 20-40 % of cases, often within the first two years post-treatment. This recurrence is commonly linked to imbalances in the inverting and everting muscle and ligament forces surrounding the foot, disrupting ankle-foot joint alignment during gait. This study aimed to evaluate the biomechanical factors contributing to clubfoot recurrence using advanced musculoskeletal modeling. METHODS: Experimental data, including 3D segment positions, lower limb muscle activity, and ground reaction forces, were

Bronchogenic Cyst of the Filum Terminale: A Case Report and Literature Review.

This case report delineates the case of a 35-year-old male patient who has experienced an acute onset of difficulty initiating and evacuating his bladder, with a nine-month background history of gradually progressive low back pain. The patient also had urinary urgency. The MRI spine revealed a well-defined intradural extra-medullary non-enhancing cyst at T12-L1 that measured 1.1 x 1.3 cm and resulted in significant cord compression. There was no other cutaneous, spinal cord, or vertebral abnormality associated. The patient underwent a T12-L1 laminectomy and a gross total excision of the spinal

Forward Simulations of Walking on Surfaces with Asymmetric Mechanical Impedance: Insights for Gait Rehabilitation.

Gait asymmetry, prevalent in stroke survivors and various other neurological and musculoskeletal conditions, leads to abnormal joint loading, increased fall risk, and reduced walking efficiency. Traditional rehabilitation methods often fail to consistently reduce weight-bearing gait asymmetry, necessitating innovative approaches. This study explores the potential of an adjustable mechanical impedance treadmill to amplify weight-bearing asymmetries, leveraging the "error amplification" technique akin to split-belt treadmill training. We developed a 2D optimal control gait model in OpenSim to si

Chance-Type Fracture of the Sacrum in an Elderly Patient: A Case Report.

Chance-type fractures involving the sacrum have not been previously reported to the best of our knowledge. We describe a rare case in a 79-year-old man who fell into deep snow, the pelvis became effectively fixed, and the trunk flexed forward, creating a seat-belt-like flexion-distraction mechanism, resulting in low back and left leg pain. Initial plain radiographs were normal, and the patient was managed conservatively for a presumed sacral fracture. Progressive bilateral buttock and leg pain prompted magnetic resonance imaging (MRI) at four weeks, which demonstrated marrow edema in S1, fract

Knee Pain: The Fine Line Between Malignant and Benign.

Focal periphyseal edema (FOPE) is a normal physiological finding that is often incidentally discovered on MRI of the knee. FOPE zones are areas of periphyseal edema typically observed near the time of physeal closure. This common physiologic phenomenon is related to changes in the distribution of forces around the physis as it closes during adolescence, occurring more frequently in females between 11 and 14 years of age. The condition may be associated with pain or may be asymptomatic. It is often mistaken for pathological bone marrow edema or a Salter-Harris fracture. We present the case of a

Hypophosphatasia: Clinical Clues and Management Considerations.

This case report identifies typical manifestations of a patient with hypophosphatasia, a rare genetic condition in which mutations in tissue-non-specific alkaline phosphatase (TNSALP) enzymes cause low levels of alkaline phosphatase and defective bone mineralization. It explores common diagnostic clues from the history and laboratory evaluation, which can help clinicians identify the disorder. This case introduces a 49-year-old patient with a long history of fractures and dental abnormalities who was referred to endocrinology for evaluation of osteopenia. Further review of her laboratory data

Comparison of different surgeries for correction of fixed flatfoot deformity studied through a dynamic model.

Different surgeries are used to treat stage III Adult Acquired Flatfoot Deformity. Some include hindfoot manipulation with subtalar fusion (single fusion), triple fusion (subtalar, talonavicular, and calcanealcuboid), and triple fusion with additional midfoot and forefoot manipulation. This study aimed to compare the effect of these surgeries on the unloaded and loaded foot, using a validated dynamic computational model. Five patients with stage III flatfoot underwent pre-surgical and post-surgical CT scans. Dynamic computational models were created for four groups: pre-operative, single fusio
