Revisit of Histopathology in Rheumatoid Lung Nodules.

Diagnosing pulmonary rheumatoid nodules (RN) is challenging because they are rare and share clinical, radiologic, and histopathologic features with other necrotizing granulomatous diseases such as infectious granulomas (IG) and granulomatosis with polyangiitis (GPA). Herein, we revisit the histopathologic features of RN and the findings in the adjacent lung tissue, in comparison with IG and GPA, to identify distinguishing features. Twenty-eight cases with surgically resected RN evaluated at our institution (1991 to 2024), 33 IG (10 mycobacterial infection, 10 histoplasmosis, 13 coccidiomycosis

Evolving Epidemiology, Improving Diagnostic Tests and Their Importance for the Correct Diagnosis of Histoplasmosis.

Histoplasmosis has traditionally been described as having discrete geographic areas of endemicity. Over the last few decades, it has become more and more clear that these areas are not accurate depictions of where histoplasmosis can occur. Our understanding of where histoplasmosis occurs has improved in recent years due to improving access to diagnostic testing and increased reporting as well as larger at-risk populations (HIV and non-HIV immune suppression) resulting in more cases. Although areas of relatively higher risk and case numbers certainly still exist, histoplasmosis has been observe

Chronic Cavitary Pulmonary Histoplasmosis-Novel Concepts Regarding Pathogenesis.

Because the apices of the lungs are most commonly involved in chronic cavitary histoplasmosis (CCPH), it has been assumed by many to have a pathogenesis which is similar to post-primary tuberculosis. Fungi such as Aspergillus may colonize pulmonary bullae. Although less common, colonization by Histoplasma capsulatum in a heavily endemic area is possible or even probable. In chronic obstructive pulmonary disease (COPD), apical bullae are characteristic. Since COPD is common and CCPH is rare, the pathogenesis of CCPH remains incompletely understood. What is presently known about the pathogenesis

Acute Myelopathy as Initial Presentation of Disseminated Histoplasmosis: A Case Report.

Histoplasmosis is a fungal infection caused by the fungus Histoplasma capsulatum that can rarely present with central nervous system (CNS) manifestations that include meningitis, encephalitis, focal brain or spinal cord lesions, and stroke syndromes. Because of this variation from patient to patient, CNS histoplasmosis is a difficult clinical diagnosis to make, which can be further hindered by no highly sensitive diagnostic testing available. Here, we present a unique case of a 46-year-old male immunocompromised due to type 1 diabetes mellitus with disseminated histoplasmosis as an acute prese

Mapping Histoplasma in Bats and Cave Ecosystems: Evidence from Midwestern Brazil.

UNLABELLED: Caves serve as natural reservoirs for diverse microbial species due to their unique biotic and abiotic conditions. Histoplasma spp. is frequently associated with guano-enriched soil, low luminosity, and high humidity, particularly in Latin America, a region highly endemic for histoplasmosis. Despite the continent's diverse biomes, local environmental and host distributions of Histoplasma remain poorly understood. To address this knowledge gap, we conducted a Histoplasma -specific quantitative PCR (qPCR) assay targeting the hc100 gene on guano samples from seven bat-inhabited caves

High genetic diversity of Histoplasma in the Amazon basin, 2006-2017.

Genome sequencing has revealed that Histoplasma, the etiological agent of histoplasmosis is composed of several phylogenetic species. Nonetheless, the genetic diversity of the pathogen remains largely unknown, especially in the tropics. We sequenced the genome for 91 Histoplasma isolates from the Amazon basin, and used phylogenomics, and population genetic evidence to measure the genetic variation of the genus in South America. We report a previously unidentified clade of Histoplasma endemic to the Amazon basin. The lineage is widespread across the continent and contains five lineages that are

AngioVac-assisted management of histoplasma capsulatum endocarditis in a bioprosthetic aortic valve: challenges and outcomes.

BACKGROUND: Histoplasma capsulatum infective endocarditis (IE) is rare and often fatal, especially in prosthetic valve patients, due to delayed diagnosis and limited therapeutic options. This case demonstrates the utility of AngioVac for managing large fungal vegetations, underscores the importance of considering fungal IE in culture-negative cases, and highlights the role of a multidisciplinary approach in high-risk patients. We report a 76-year-old female with a bioprosthetic aortic valve who presented with persistent culture-negative fever, splenic infarcts, and large vegetations on her pro

Travelers From the Intestine to the Skin: A Useful Clue to Diagnose a Cryptic Histoplasma Infection.

A 53-year-old woman from Missouri (United States) with a 2-year history of kidney transplant presented to our dermatology department with multiple acneiform lesions on the face and numerous erythematous papules and nodules on both arms and legs. Biopsies revealed a very atypical florid pseudoepitheliomatous hyperplasia, at first interpreted, in light of the history of immunosuppression, as an invasive squamocellular carcinoma, associated with an unusual dense histiocytic infiltrate and nodular aggregates of foamy histiocytes. After the acute onset of fever, pancytopenia, and signs of ulcerativ

Febrile Pancytopenia and Hemophagocytosis From Disseminated Histoplasmosis in HIV/AIDS Patients: Two Cases and a Review of Combined Antifungal and Steroid Therapy.

Hemophagocytosis is a clinical condition characterized by the engulfment of bone marrow cellular elements, including erythrocytes, leukocytes, platelets, and their precursors, by activated macrophages. It has been associated with several infectious organisms, including the Epstein-Barr virus (EBV) and histoplasmosis. Human immunodeficiency virus (HIV) has been known to trigger hemophagocytosis in the presence or absence of other infections. Disseminated histoplasmosis is a common opportunistic infection in advanced patients with acquired immunodeficiency syndrome (AIDS) in endemic areas; howev

Detection of Histoplasma capsulatum in Bats from the Brazilian Western Amazon.

Histoplasma capsulatum is a saprophytic dimorphic fungus that causes histoplasmosis, a systemic infectious disease of relevance to public health. Bats can be important agents in the epidemiological cycle of the disease since they act as reservoirs of microorganisms. The aim of this study was to detect Histoplasma capsulatum in the lung tissue of bats captured in urban forest fragments in the municipality of Rio Branco, Acre, in the Western Amazon. Twenty-two bat species were captured from five urban forest fragments. The samples taken were subjected to histopathological, mycological, and molec

Hemophagocytic Lymphohistiocytosis Due to Disseminated Histoplasmosis in a Young Patient with AIDS.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening syndrome of immune dysregulation that results in multiorgan failure; HIV and histoplasmosis are known triggers of HLH. A young patient with AIDS was found to have disseminated histoplasmosis and met criteria for HLH. Despite 10 days of treatment with liposomal amphotericin B, she developed shock and acute respiratory distress syndrome and ultimately died. The few reported cases of HLH due to histoplasmosis in AIDS offer a variety of treatment approaches. There have been successful outcomes combining amphotericin with chemotherapies

Molecular Detection of Histoplasma in Bat-Inhabited Tunnels of Camino de Hierro Tourist Route, Spain.

We detected Histoplasma capsulatum in 2 bat-inhabited tunnels of a tourist route in northern Spain. This finding confirms that the geographic distribution of this fungal pathogen is wider than previously thought. Our results highlight the need for surveillance and assessment of the potential infection risk for workers and visitors.

Cave-Associated Histoplasmosis Outbreak Among Travelers Returning from Costa Rica - Georgia, Texas, and Washington, December 2024-January 2025.

Histoplasmosis is a fungal infection that primarily affects the lungs. The condition is caused by Histoplasma organisms, which are often found in soil contaminated with bird or bat droppings. On January 17, 2025, a Georgia infectious disease physician notified CDC of suspected histoplasmosis cases among 12 members of an extended family from households in Georgia, Texas, and Washington. The ill family members included six adults aged 42-49 years and six children aged 8-16 years. They had recently returned from Costa Rica, where they toured a cave linked to a previous histoplasmosis outbreak (1)

Left Recurrent Laryngeal Nerve Vocal Fold Paralysis in Pediatric Patient Secondary to Mediastinal Histoplasmosis Infection.

Histoplasma capsulatum is a dimorphic soil-based fungus endemic to the Ohio and Mississippi River valleys and southeastern United States. An extremely rare presentation is the involvement of the mediastinum leading to vocal fold paralysis, with only 2 reported cases in the literature. This report presents the youngest patient to whom this pathology has been described and the third overall patient. In this case report, we present a 15-year-old boy with vocal fold paralysis secondary to left recurrent laryngeal nerve paralysis in the setting of confirmed histoplasmosis infection. On further work
