BACKGROUND: Successful delay of aftercoming siblings is an infrequent event in obstetrics. No case of second-trimester operative vaginal evacuation of a fetus followed by delayed delivery of the remaining siblings has been described previously.

Prolonged interdelivery periods in preterm twin and triplet gestations have resulted in a good outcome for the fetus(es) remaining in utero. This is the second reported case of delayed delivery intervals in quadruplets who were born on 3 separate days. The purpose of the study was to compare acid-base parameters in capillary blood of newborns from groups with hypoxaemia symptoms and from the control group (physiological deliveries). 111 newborns were divided into 8 groups. A 12-year follow-up study was carried out to evaluate the risk factors for unspecified low-back pain and sciatic pain. Middle-aged farmers who did not report any low-back or neck-shoulder pain during the previous year in a postal questionnaire in 1979 were included in the follow-up study in 1992 (n = 537). In total, 366 (68%) of these farmers were interviewed by telephone. In 1992, the one-year prevalence rates of unspecified low-back pain (13.3%) and sciatic pain (9.6%) were low. OBJECTIVE: Premature rupture of membranes in multifetal gestations during the second trimester has an ominous prognosis and the majority of the fetuses die after preterm delivery.
STUDY DESIGN: We used cervical cerclage, tocolysis, and antibiotic therapy after vaginal delivery of the fetus with ruptured membranes in eight patients with multifetal pregnancies to extend the intrauterine life and improve the outcome of the remaining fetuses.

In a cohort analytical study 47 primigravidas in spontaneous normal labour at term were divided into two groups depending on the presence or absence of coupled uterine contractions during active labour. The optimal management of prolonged second stage of labour remains a confused area of thought and action. This confusion is particularly evident when epidural analgesia is also being used. A review of the literature indicates that prolonged duration of the second stage, i.e. from full dilation of the cervix until delivery, with or without epidural analgesia, has little adverse effect on perinatal outcome. More attention should be paid to the expulsive phase of the second stage and greater efforts made to reduce unnecessary interventions.

To elucidate the importance of hand eczema in the population of Gothenburg, a questionnaire was sent to 20,000 individuals aged 20-65 years, randomly selected from the population register of the city. After two reminders, a response rate of 83% was obtained. The effectiveness of vacuum extraction with the Silastic Obstetrical Vacuum Cup (SOVC), which has a soft, maleable cup, was assessed by two family physicians in a small rural hospital. Vacuum extraction was attempted in 35 of 231 deliveries over an 18-month period, with an overall success rate of 66%. The early phase of diaplacental transfer of diazepam was studied in 39 women given the drug as a basic anaesthetic for operative vaginal delivery indicated by prolonged second stage of labour (9 cases), breech delivery (19 cases) and intrauterine hypoxia (11 cases).
