A Qualitative Exploration of Clinicians' Perspectives on Obstructive Sleep Apnea Facilitators and Barriers to Care.

BACKGROUND: Literature has suggested that children experience wide variation in diagnosis and treatment of obstructive sleep-disordered breathing (SDB), however, multidisciplinary perspectives on barriers and facilitators to care are not well delineated among clinicians. METHODS: This qualitative study adhered to the Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines. Participants were recruited until thematic saturation was achieved across the overall dataset. Key informant interviews were conducted with physicians and advanced practice providers from primary care, o

The lived experiences and coping mechanisms of women with obstetric fistula in Ethiopia: A systematic review and meta-synthesis of qualitative evidence.

BACKGROUND: Obstetric fistula, a preventable consequence of prolonged obstructed labor, inflicts profound physical, psychological, and social suffering, leaving many women described as "living dead." In Ethiopia, where the burden remains high, little is known about women lived experiences and coping strategies beyond clinical outcomes. This review was conducted to synthesize qualitative evidence, illuminate these challenges, and inform holistic interventions. OBJECTIVES: To synthesize available evidence on the lived experiences and coping mechanisms of women with obstetric fistula in Ethiopia

Alternating Edge- and Face-Sharing Octahedra with Partial Super-Exchange Enabling High-Voltage Layered Cathodes.

Traditional O3-type layered oxides with edge-sharing (ES) octahedra suffer from structural instability under high-voltage operation. To address this, we propose a stable edge-sharing coplanar (ESC) O2-type layered structure. In this structure, Li-O octahedra share edges on one side and faces on the other with Co-O octahedra. This ESC-configuration effectively suppresses Co migration and enhances intrinsic structural stability. Furthermore, the 180°Ni-O-TM (TM = paramagnetic transition metal) super-exchange interactions along the edge-sharing directions are introduced to improve high-voltage cy

Optimizing COPD Care in Belgium: A Multidisciplinary Expert Consensus on Cardiopulmonary Risk Management.

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) represents a major health and economic challenge in Belgium, affecting approximately 800,000 individuals, half of whom remain undiagnosed. Beyond respiratory morbidity, COPD patients face substantial cardiopulmonary (CP) risk-encompassing severe exacerbations and cardiovascular (CV) events-that is often under-recognized and insufficiently addressed due to limited clinical awareness, fragmented care, and the absence of national guidance. METHODS: A multidisciplinary Belgian taskforce (general practitioners, pulmonologists, cardiologist

Malignant Hemispheric Stroke Secondary to Cavernous Internal Carotid Artery Stenosis in the Setting of Sepsis: A Case Report.

This is a novel case of a rapidly progressive right hemisphere stroke in the setting of multiple infections that highlights the impact of comorbidities on stroke progression. We hope that reporting this case draws attention to the need for a multidisciplinary focus on the progression of stroke symptoms in the face of septic processes. A 60-year-old African American man with a past medical history of hypertension, diabetes, bilateral above-the-knee amputations, and frequent cocaine use presented to the hospital with left-sided deficits and suspected seizures. Inpatient hospitalizations in adults with congenital heart disease and down syndrome: A national perspective.

BACKGROUND: Adults with Down syndrome (DS) have a greater likelihood of being diagnosed with congenital heart disease (CHD) and may face unique cardiovascular, respiratory, and immunologic health burdens during hospitalization. While CHD survival has improved in recent decades, national data on hospitalization outcomes of patients diagnosed with DS remain limited. OBJECTIVES: The present study aimed to evaluate trends in hospital admissions, inpatient management, and perioperative outcomes stratified by the presence of DS among CHD patients. METHODS: This was a retrospective cohort of the 20

Diagnosis of Lisinopril-Induced Small Bowel Angioedema After a Seven-Year Diagnostic Journey: A Case Report With Patient Perspective.

Angiotensin-converting enzyme inhibitors (ACEIs) are widely prescribed for managing hypertension and improving cardiovascular and renal health while being relatively low-cost and generally well-tolerated. ACEI-induced angioedema is an uncommon but potentially serious adverse effect that may present with swelling of the face, lips, tongue, and pharynx, raising concern for possible airway obstruction, hypoxia, and even death. Other presentations have been documented, including small bowel angioedema (SBAE). SBAE is diagnostically challenging due to its nonspecific gastrointestinal symptoms. This

Proximal policy optimisation based black-box optimizer for tailoring inhalation instructions of an inhaler for minimising mouth-throat deposition.

Inhalation therapy using dry-powder inhalers (DPIs) offers targeted drug delivery for asthma and chronic obstructive pulmonary disease (COPD). Variation in deposition rate in the mouth-throat region depends on patient inhalation manoeuvres and techniques. The present study aimed to identify optimal combinations of head position, oral opening size, and mouthpiece insertion depth for three representative inhalation manoeuvres, corresponding to low, medium and high peak inspiratory flow rates (PIFRs). An idealised adult airway model was reconstructed and coupled with transient computational fluid

Facial Wrinkling is Not Associated with Chronic Obstructive Pulmonary Disease Severity in Thai Patients: A Cross-Sectional Study.

BACKGROUND: Western studies have reported associations between facial wrinkling and chronic obstructive pulmonary disease (COPD), suggesting a shared susceptibility to tissue aging. However, data on Asian populations are limited, and population-specific differences in skin biology may modify these relationships. OBJECTIVE: To investigate the association between facial wrinkle severity and COPD characteristics in Thai patients and to identify independent predictors of wrinkle severity. METHODS: This cross-sectional study enrolled 93 Thai patients with COPD. Facial wrinkles were assessed using

Nonsurgical Treatment of Radiculopathy From a Lumbar Synovial Cyst Using the Minimally Invasive Lumbar Decompression (MILD) Procedure: A Case Report.

Lumbar synovial cysts are fluid-filled lesions arising from the facet joints that can cause lateral recess stenosis and neurogenic claudication. While surgical decompression is the gold standard for symptomatic cysts, patients with significant comorbidities, such as chronic obstructive pulmonary disease (COPD) and emphysema, face elevated perioperative risks under general anesthesia and delayed wound healing. This case report describes the successful application of the minimally invasive lumbar decompression (MILD) procedure as a safe alternative for a high-risk surgical candidate. A 68-year-o

An Oral Cavity Mass in a 5-Year-Old Girl.

BACKGROUND: Lymphatic malformations are rare congenital vascular anomalies that most commonly affect the head and neck regions. Cervicofacial lesions extending into the oral cavity or upper airway can result in dysphagia, bleeding, malnutrition, and airway obstruction. Emergency assessment is particularly challenging because significant anatomic distortion can exist even when physiologic parameters remain stable, necessitating thorough airway evaluation and early anticipation of potential clinical deterioration. CASE REPORT: A 5-year-old girl who had recently emigrated from Peru presented wit

Non-invasive ventilation in severe acute respiratory failure complicating Madelung's disease: a case series and management review.

Madelung's disease (Launois-Bensaude syndrome) presents a critical challenge when managing acute respiratory failure (ARF) due to the high risk of difficult airway and failed intubation posed by symmetric cervical lipomatosis. This case series investigates the feasibility of non-invasive ventilation (NIV) as a primary strategy for ARF in these high-risk patients. We present two cases of severe ARF triggered by COVID-19 pneumonia in patients with Madelung's disease. Both patients were successfully managed with NIV using a total face mask, which avoided the need for hazardous endotracheal intuba

Cocoon Syndrome: A Clinical Case of Intestinal Obstruction Highlighting the Diagnostic and Therapeutic Importance of Exploratory Laparoscopy in the Face of Ambiguous Imaging.

Cocoon syndrome, also known as sclerosing encapsulating peritonitis (SEP), is a rare condition characterized by the encapsulation of small bowel loops by a thick fibrocollagenous membrane. Its non-specific clinical presentation and ambiguous findings on conventional imaging often lead to significant diagnostic delays or misdiagnosis, frequently being mistaken for internal hernias or neoplastic processes. We present the case of a 61-year-old male patient with no prior abdominal surgical history who presented with a six-month history of recurrent abdominal pain, vomiting, and weight loss. While

Stabilizing telerobotic endobronchial imaging and interventions in breathing lungs with a helical brace.

Physiological motion in dynamic luminal organs, such as respiration-induced lung displacement, compromises the stability of tool-tissue interaction during telerobotic endoluminal imaging and interventions. However, existing devices for stabilizing the distal end of a catheter or endoscope face critical limitations: Rigid shape-locking mechanisms risk pressure-induced trauma, electrical hazard, and thermal injury, whereas balloon-based designs obstruct luminal patency. Moreover, they lack sufficient adjustable-diameter range for anatomies such as the bronchial tree. In this work, we present a r

Negative-pressure pulmonary oedema following upper airway obstruction after tracheal extubation in an adolescent undergoing general anaesthesia for dental extractions.

A healthy athletic adolescent male with a remote history of asymptomatic childhood asthma underwent outpatient multiple molar extractions under general anaesthesia with tracheal intubation. Shortly after gentle tracheal extubation, he developed acute upper airway obstruction with oxygen desaturation and increased work of breathing, despite careful oropharyngeal aspiration and confirmed reversal of neuromuscular blockade. Initial airway manoeuvres and face-mask ventilation with 100% oxygen against high airway resistance provided only transient improvement, requiring tracheal re-intubation and p

Traumatic canalicular laceration and complex eyelid injury in a newborn delivered by forceps.

We present a neonatal case of complex upper eyelid lacerations with canalicular injury following a forceps-assisted vaginal delivery. Mother's insistence on vaginal delivery despite obstetric advice of caesarean section led to obstructed labour requiring forceps delivery. The neonate presented with four full-thickness lacerations of the right upper eyelid, including injury to the upper canaliculus. Immediate surgical repair with canalicular stenting was performed. A corneal ulcer developed secondary to stent-induced mechanical trauma. Early stent removal with topical fortified antibiotics led
