Pre-operative ultrasound before cesarean delivery: reviewing findings during the first surgical time-out to improve maternal and infant safety.

Twenty-nine million cesarean deliveries performed every year. Although often regarded as routine, even elective procedures can lead to serious complications for mothers and infants. Modern patient safety emphasizes that no incision should be undertaken without knowing what lies beneath, a principle codified by the World Health Organization in its Surgical Safety Checklist. A preoperative ultrasound represents an extension of this principle to obstetrics. Several complications exemplify face-valid safety signals: incision through an anterior placenta with maternal hemorrhage and neonatal anemia

Assessment of Satisfaction Rate After Closed Reductions of Nasal Bone Fractures: A Cross-Sectional Study.

BACKGROUND: The nose is the central and most protruded part of the face, most vulnerable to facial trauma. Treatment for nasal bone fractures involves open reduction and closed reduction. Closed nasal reduction is a simple procedure that can be performed under local or general anesthesia with an acceptable success rate. AIM: The current study aimed to assess the patient's satisfaction following the closed reduction of nasal bone fractures, both in aesthetic and functional values, and causes of dissatisfaction. METHODOLOGY: A community-based descriptive cross-sectional study was conducted in

Understanding the mismatch between in-vivo and in-silico rhinomanometry.

Numerical simulations and clinical measurements of nasal resistance are in quantitative disagreement. The order of magnitude of this mismatch, that sometimes exceeds 100%, is such that known sources of uncertainty cannot explain it. The goal of the present work is to examine a source of bias introduced by the design of medical devices, which has not been considered until now as a possible explanation. We study the effect of the location of the probe on the rhinomanometer that is meant to measure the ambient pressure. Rhinomanometry is carried out on a 3D silicone model of a patient-specific an

Ultra-low-power System-on-Chip for automated screening of central apnea and hypopnea via chin electromyography.

Central Apnea (CA) and Central Hypopnea (CH) are sleep disorders arising from the brain's inability to signal respiratory muscles, potentially leading to severe complications such as heart failure. This study presents a novel system for automating CA and CH event detection in sleep apnea patients using a feedforward neural network (FdNN) architecture integrated into an ultra-low-power System-on-Chip (SoC) with chin electromyography (EMG) signals. The SoC achieves sub-1-mW power consumption through careful co-optimization of the FdNN architecture, hardware design, and circuit-level consideratio

Treacher-Collins Syndrome With Anorectal Malformation: A Rarity and a Challenge for the Surgical Team.

Treacher-Collins syndrome (TCS) is a rare anomaly involving the first and second pharyngeal arches, resulting in structural anomalies to the bones and soft tissue in the region of the face, including the ears and pharynx. Due to the peculiarities of the face and pharynx, this condition presents an interesting challenge to the pediatric anesthetist. Anorectal malformation (ARM) is one of the most common causes of neonatal intestinal obstruction requiring emergency surgery. To our knowledge, there is no known association between TCS and ARM, and no reports of both conditions occurring in the sam

Extensive Surgical Emphysema and Pneumomediastinum Following Chest Drain Insertion in a Patient With Bullous Chronic Obstructive Pulmonary Disease (COPD): A Case Report.

Subcutaneous (surgical) emphysema and pneumomediastinum are recognized complications of thoracic procedures. They are often self-limiting but may occasionally indicate serious underlying pathology or precipitate life-threatening deterioration, particularly in patients with chronic obstructive pulmonary disease (COPD) and bullous lung disease. We present the case of an 87-year-old male patient with known COPD who was admitted with worsening dyspnoea and a productive cough. Imaging revealed a large left-sided pneumothorax, and emergency needle decompression followed by intercostal chest drain in

An Immunocompetent Host With Severe Disseminated Herpes Simplex Virus Type 2 Infection With Hepatorenal Involvement.

Herpes simplex virus type 2 (HSV-2) most commonly causes localized mucocutaneous infections, whereas disseminated disease with visceral involvement is rare, particularly in immunocompetent individuals. HSV hepatitis is a potentially life-threatening manifestation that often presents with nonspecific symptoms and markedly elevated liver enzymes, making early recognition difficult. We report a 58-year-old immunocompetent man with good performance status (Eastern Cooperative Oncology Group (ECOG) 0) who presented with fever, malaise, and a progressive vesicular rash involving the face, hands, and

A Rare Case of Idiopathic Angioedema Associated With Amlodipine Use.

Angioedema is a self-limited, non-pitting edema of subcutaneous or submucosal tissues often affecting the face, lips, oral cavity, and airway. It may be histamine or bradykinin-mediated and is most commonly triggered by allergic reactions, medications, or systemic conditions. Drug-induced angioedema is frequently associated with angiotensin-converting enzyme (ACE) inhibitors, whereas calcium channel blockers (CCBs) like amlodipine are rarely implicated. This case report describes a rare case of recurrent angioedema in a hypertensive patient, ultimately attributed to amlodipine, and underscores

Palliative Radiotherapy for Dysphagia in Esophageal Cancer: Short-Term Benefits and Late Toxicity Risks.

Introduction Esophageal cancer is often diagnosed at an advanced stage, with dysphagia being the most common and distressing symptom that significantly impairs quality of life (QOL). Palliative radiotherapy is frequently used to relieve dysphagia, but its benefits and risks remain unclear. This study aimed to evaluate short-term dysphagia improvement and treatment-related toxicities in patients with esophageal cancer receiving palliative radiotherapy. Materials and methods We retrospectively analyzed 58 patients with esophageal cancer and dysphagia treated with palliative radiotherapy (30-40 G

Severe Hepatitis in a Patient With Undifferentiated Connective Tissue Disease: Diagnostic Uncertainty Between Methotrexate Toxicity and Autoimmune Liver Involvement.

A patient in their mid-20s with a background of undifferentiated connective tissue disease (UCTD) presented with two days of worsening jaundice, pruritus, and vomiting. Methotrexate had been stopped six weeks earlier because of mildly elevated liver enzymes, yet the abnormalities progressed after withdrawal. On examination, there was marked jaundice and a non-specific maculopapular rash involving the limbs and face. Laboratory studies showed alanine aminotransferase (ALT) 1,500 IU/L, aspartate aminotransferase (AST) 1,200 IU/L, and bilirubin 238 µmol/L, with normal international normalised rat

Dexmedetomidine-Based Anesthesia for Pediatric Adenotonsillectomy: A Retrospective Observational Case Series.

Background Tonsillectomy is a surgical procedure primarily aimed at the removal of the tonsils, which are lymphoid tissues located at the back of the throat. This procedure is commonly indicated for conditions such as recurrent tonsillitis, chronic tonsillitis, or obstructive sleep-disordered breathing resulting from enlarged tonsils, which is often conducted under general anesthesia. The purpose of this research is to assess the safety and efficacy of dexmedetomidine-based anesthesia with minimal opioid supplementation in children undergoing adenotonsillectomy. Methods We studied pediatric pa

Bilateral intraocular masses in a child as a first presentation of disseminated tuberculosis: case report.

An 8-year-old girl presented with left leukocoria, prompting an evaluation that revealed bilateral intraocular masses, including a right upper nasal choroidal lesion and a large left heterogeneous mass with exudative detachment. Imaging and systemic assessment uncovered multiple intracranial lesions, miliary lung lesion, a facial lesion, and a right tibial lesion. A strongly positive tuberculin test and a history of close contact with a tuberculosis (TB) patient led to the diagnosis of disseminated TB. Treatment with anti-tuberculosis therapy and systemic steroids initially resulted in improve

Histological Changes and Immunohistological Analysis of Interstitial Cells of Cajal in Pelvi-Ureteric Junction Obstruction: Correlation With Preoperative Renal Function.

INTRODUCTION: A decrease in smooth muscle cells at the pelvi-ureteric junction (PUJ), along with abnormal muscle orientation, collagen deposition, and a reduction in Cajal cells, is proposed as the primary cause of obstruction at the PUJ. The findings on histology and the density of interstitial cells of Cajal (ICCs) in patients operated for pelvi-ureteric junction obstruction (PUJO) show variable observations. Clinicians often face a dilemma during the diagnosis and management of patients with PUJO. Hence, to address this knowledge gap, a study of the detailed microanatomy of PUJO and the num

Multisystem approach for management of OSA in Down syndrome: a case report.

UNLABELLED: Obstructive sleep apnea (OSA) is common in children with Down syndrome, with reported prevalence rates as high as 69-76%. Multiple factors predispose children with Down syndrome for OSA, including craniofacial hypoplasia (maxillary and mandibular), airway abnormalities, macroglossia, generalized hypotonia, airway hypotonia, adenotonsillar hypertrophy, and obesity. Despite the fact that the pathophysiology for OSA in children with Down syndrome is multifactorial in nature, treatment methods have focused on soft tissue in the upper airway using adenotonsillectomy and/or continuous po

Prognostic factors of MINOCA and their possible mechanisms.

OBJECTIVE: Despite not showing substantial stenosis of coronary arteries, Myocardial Infarction with Non-Obstructive Coronary Arteries (MINOCA) presents with myocardial ischemia injury, thus having a grave prognosis and a high risk of long-term complications. This necessitates increased clinical attention and exploration of its root causes to prevent a similar crisis. METHODS: Research on MINOCA is limited, especially in terms of its clinical attributes, long-term outlook, risk stratification, and prognosis-linked cardiometabolic risk factors. This review aims to fill these gaps, providing an

Method for evaluating the fit of custom-fit Continuous Positive Airway Pressure masks using finite element analysis.

Continuous Positive Airway Pressure (CPAP) is a common therapy used to treat breathing disorders such as obstructive sleep apnea. In previous work, we designed a custom-fit CPAP mask to address comfort and leakage issues patients often experience. This paper presents a method to create a finite element (FE) model to evaluate the fit of the custom-fit mask before fabrication. The model includes details such as a skull to represent the variable soft tissue thicknesses on the face, and two strap configurations, original and X. The model was tested on four subjects and results show that the X stra

Assessment of obstructive sleep apnoea in children: What are the challenges we face?

There is an increasing demand for the assessment of sleep-disordered breathing in children of all ages to prevent the deleterious neurocognitive and behaviour consequences of the under-diagnosis and under-treatment of obstructive sleep apnoea [OSA]. OSA can be considered in three broad categories based on predominating contributory features: OSA type 1 [enlarged tonsils and adenoids], type II [Obesity] and type III [craniofacial abnormalities, syndromal, storage diseases and neuromuscular conditions]. The reality is that sleep questionnaires or calculations of body mass index in isolation are

Comparative analysis of traction forces in normal and glaucomatous trabecular meshwork cells within a 3D, active fluid-structure interaction culture environment.

This study presents a 3D in vitro cell culture model, meticulously 3D printed to replicate the conventional aqueous outflow pathway anatomical structure, facilitating the study of trabecular meshwork (TM) cellular responses under glaucomatous conditions. Glaucoma affects TM cell functionality, leading to extracellular matrix (ECM) stiffening, enhanced cell-ECM adhesion, and obstructed aqueous humor outflow. Our model, reconstructed from polyacrylamide gel with elastic moduli of 1.5 and 21.7 kPa, is based on serial block-face scanning electron microscopy images of the outflow pathway. It allows
