Case Report: Spontaneous dissection of the renal pelvis and ureter.

Spontaneous dissection of the renal pelvis and ureter is a rare urinary system disease characterized by separation between the muscularis and mucosal layers or within the muscularis of the pyeloureteral wall, with urine extravasation into the false lumen causing renal colic and hydronephrosis. We report a 30-year-old woman with no prior kidney stone history who presented with paroxysmal colic, nausea, and a 1-month history of abdominal pain that had exacerbated over 1 day; urinalysis showed multiple abnormalities. Non-contrast computed tomography (CT) revealed pyeloureteral dilation without ca

Ischemic Colitis Associated With Tirzepatide Therapy in a Young Female Patient: A Case Report.

Tirzepatide has been approved for the management of obesity, obstructive sleep apnea, and type 2 diabetes mellitus; while gastrointestinal side effects are commonly seen, ischemic colitis has been only rarely described. We present the case of a 32-year-old woman with no prior medical history who developed hematochezia and abdominal pain after six weeks of tirzepatide therapy for weight loss. Laboratory evaluation showed neutrophilic leukocytosis without anemia or electrolyte abnormalities, and stool studies were negative for infection. A CT of the abdomen and pelvis excluded obstruction or wal

Diagnostic Yield of Upper-Tract Imaging in Visible Haematuria: A Single-Centre Retrospective Clinical Audit.

Introduction Visible haematuria (VH) is a common presenting symptom of urological malignancy, yet the optimal upper-tract imaging modality remains debated. Recent Getting It Right First Time (GIRFT) Greener Pathway guidance recommends ultrasound as the first-line investigation, reserving CT urogram (CTU) for high-risk patients. Our District General Hospital (DGH) currently performs CTU for all VH patients, regardless of risk factors, with adequate renal function. This audit aimed to evaluate the diagnostic yield of upper-tract imaging in our local VH pathway and to review our current local hae

Non-traumatic Intramural Duodenal Hematoma Secondary to Alcoholic Pancreatitis.

Duodenal hematomas are a rare phenomenon, with the most common etiology being traumatic in origin. We describe the case of a spontaneous intramural duodenal hematoma in a 56-year-old male as a complication of alcohol-induced pancreatitis. Initial endoscopy showed evidence of a bulging deformity in the second and third parts of the duodenum. Further imaging using endoscopic ultrasound (EUS) demonstrated a round intramural cystic lesion in the third portion of the duodenum that was hypoechoic, septated, and measuring approximately 6.0 x 7.6 cm in diameter and yielded a bloody aspirate. A compute

Rethinking Right Upper Quadrant (RUQ) Pain: The Role of Lemmel Syndrome in Biliary Obstruction.

Lemmel syndrome involves a periampullary duodenal diverticulum (PAD), a pouch-like outpouching near the ampulla of Vater, compressing the common bile duct. We describe a case of severe abdominal pain in a patient who had a large periampullary diverticulum, managed with surgical intervention after an initial failed endoscopic retrograde cholangiopancreatography (ERCP). An elderly female patient in her early 90s arrived at the emergency department with severe cramping pain localized to the right upper quadrant of her abdomen, progressively intensifying over several weeks. Her blood pressure meas

Megacolon diagnosis in pregnancy: A case report and literature review.

Megacolon is a rare clinical condition consisting of an abnormally dilated colon in the absence of mechanical obstruction. Megacolon can complicate pregnancy in terms of maternal morbidity and mortality (volvulus, ileus, systemic toxicity, bowel perforation, sepsis) and obstetrical outcomes (preterm birth, premature rupture of membranes, dystocia). Pregnancy, on the other hand, can exacerbate chronic constipation through hormonal and mechanical mechanisms. A case of megacolon, first detected during pregnancy in an otherwise healthy nulliparous woman, is reported. The diagnosis was suspected on

Genetically proxied low-density lipoprotein cholesterol lowering via PCSK9-inhibitor drug targets and risk of congenital malformations.

AIMS: Current guidelines advise against the use of lipid-lowering drugs during pregnancy. This is based only on previous observational evidence demonstrating an association between statin use and congenital malformations, which is increasingly controversial. In the absence of clinical trial data, we aimed to use drug-target Mendelian randomization to model the potential impact of fetal LDL-lowering, overall and through PCSK9 drug targets, on congenital malformations. METHODS AND RESULTS: Instrumental variants influencing LDL levels overall and through PCSK9-inhibitor drug targets were extract

Successful pregnancy in the blind hemicavity of Robert's uterus: a rare case report and brief literature review.

BACKGROUND: Robert's uterus is a rare congenital anomaly, characterized as an asymmetric septate uterus that has a blind hemicavity with unilateral menstrual fluid retention and a unicornuate hemicavity connecting to the cervix unimpededly. Patients with Robert's uterus generally present with menstrual disorders and dysmenorrhea, and some may have reproductive problems as well, including infertility, recurrent miscarriage, preterm labor and obstetric complications. In this case, we describe a successful pregnancy implanted on the obstructed hemicavity and delivered a liveborn girl. Meanwhile,

The VASCERN-VASCA working group diagnostic and management pathways for severe and/or rare infantile hemangiomas.

The European Reference Network on Rare Multisystemic Vascular Diseases (VASCERN), is dedicated to gathering the best expertise in Europe and provide accessible cross-border healthcare to patients with rare vascular diseases. Infantile Hemangiomas (IH) are benign vascular tumors of infancy that rapidly growth in the first weeks of life, followed by stabilization and spontaneous regression. In rare cases the extent, the localization or the number of lesions may cause severe complications that need specific and careful management. Severe IH may be life-threatening due to airway obstruction, liver

Using Magnetic Resonance Imaging During Childbirth to Demonstrate Fetal Head Moldability and Brain Compression: Prospective Cohort Study.

BACKGROUND: Childbirth is a physiological process with significant medical risk, given that neurological impairment due to the birthing process can occur at any time. Improvements in risk assessment and anticipatory interventions are constantly needed; however, the birthing process is difficult to assess using simple imaging technology because the maternal bony pelvis and fetal skeleton interfere with visualizing the soft tissues. Magnetic resonance imaging (MRI) is a noninvasive technique with no ionizing radiation that can monitor the biomechanics of the birthing process. However, the effect

Definition, diagnosis and management of fetal lower urinary tract obstruction: consensus of the ERKNet CAKUT-Obstructive Uropathy Work Group.

Fetal lower urinary tract obstruction (LUTO) is associated with high mortality and postnatal morbidity caused by lung hypoplasia and impaired kidney function. Specific diagnostic features that can guide clinical approach and decisions are lacking; thus, the European Reference Network for Rare Kidney Diseases established a work group to develop recommendations regarding the clinical definition, diagnosis and management of prenatally detected LUTO. The work group recommends the use of antero-posterior diameter of renal pelvis as the most reliable parameter for suspecting obstructive uropathies a

Evolution of Lactase Persistence: Turbo-Charging Adaptation in Growth Under the Selective Pressure of Maternal Mortality?

The emergence of the capacity to digest milk in some populations represents a landmark in human evolution, linking genetic change with a component of niche construction, namely dairying. Alleles promoting continued activity of the enzyme lactase through the life-course (lactase persistence) evolved in several global regions within the last 7,000 years. In some European regions, these alleles underwent rapid selection and must have profoundly affected fertility or mortality. Elsewhere, alleles spread more locally. However, the functional benefits underlying the rapid spread of lactase persisten

Dynamic External Pelvimetry Test in Third Trimester Pregnant Women: Shifting Positions Affect Pelvic Biomechanics and Create More Room in Obstetric Diameters.

Dystocia in labor is still a clinical challenge. The "contracted pelvis" is the absence of pelvic mobility, which leads to fetal-pelvic disproportion, obstructed labor, and operative delivery. Maternal pelvis biomechanics studies by high technological techniques have shown that maternal shifting positions during pregnancy and labor can create more room in the pelvis for safe delivery. The external and internal pelvic diameters are related. The present study aims to evaluate the external obstetric pelvic diameters in shifting positions using a clinical technique suitable for daily practice in e

Dental Calculus as a Potential Biosource for Human Papillomavirus Detection in Oral Squamous Cell Carcinoma.

OBJECTIVE: The infection of human papillomaviruses (HPVs) plays a role in the development of oral squamous cell carcinoma (OSCC). A poor oral hygiene and dental calculus may cause the infection to persist. Therefore, this study aimed to assess whether this dental calculus could serve as a potential biosource in early detection of HPVs in patients with OSCC. METHODS: DNA was isolated from the dental calculus of people diagnosed with OSCC, and MY09/11 primer set was used to detect the presence of HPV. The positive samples were further sequenced and aligned using megablast NCBI BLAST tool to ide

Is Physical Activity Good or Bad for the Female Pelvic Floor? A Narrative Review.

More women participate in sports than ever before and the proportion of women athletes at the Olympic Games is nearly 50%. The pelvic floor in women may be the only area of the body where the positive effect of physical activity has been questioned. The aim of this narrative review is to present two widely held opposing hypotheses on the effect of general exercise on the pelvic floor and to discuss the evidence for each. Hypothesis 1: by strengthening the pelvic floor muscles (PFM) and decreasing the levator hiatus, exercise decreases the risk of urinary incontinence, anal incontinence and pel

Predictors of labor abnormalities in university hospital: unmatched case control study.

BACKGROUND: Abnormal labor is one of the common emergency obstetric problems contributing for more than two-thirds of the unplanned cesarean section. In Ethiopia, although labor abnormality and its complications like obstetric fistula are highly prevalent, there is no published study that determines the predictors of labor abnormalities. METHODS: The study design was an unmatched case control which included 844 women (408 cases and 436 controls). Cases were identified when a woman was diagnosed to have one of the labor abnormalities at term (prolonged latent stage, active phase disorder, prol

Herlyn-Werner-Wunderlich syndrome: a rare presentation with pyocolpos.

Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA), also known as Herlyn-Werner-Wunderlich syndrome, is a rare syndrome with only a few hundred reported cases described since 1922. Only a handful of these cases have been associated with pyocolpos. Mullerian duct anomalies have an incidence of 2-3%. While OHVIRA constitutes 0.16-10% of these Mullerian duct anomalies. Symptoms usually present shortly after menarche when hematocolpos develops during menstruation resulting in dysmenorrhea and a pelvic mass. The pelvic mass is the collection of blood products within the obstructed hemivag
