Minimal access techniques for fetal surgery.

The impetus for the development of minimal access techniques for fetal surgery was the unusual occurrence with open hysterotomy of preterm labor, premature rupture of membranes, and the maternal complications resulting from tocolytic therapy. This strategy involves a constellation of techniques that allow surgical procedures to be performed inside the uterus without a hysterotomy. The unique requirements of this approach necessitated modifications of existing endoscopic techniques, development of novel fetoscopic instruments, and utilization of the expertise of a wide variety of specialists. Huge acoustic neurinomas presenting in the late stage of pregnancy. Treatment options and review of literature.

BACKGROUND: Even though vestibular schwannomas rarely present during pregnancy, symptoms may appear or worsen particularly in this period. The clinical picture may include tinnitus, hearing abnormalities, and in large tumors, brain-stem and cerebellar compression with involvement of additional cranial nerves. Large vestibular schwannomas (also known as Acoustic Neurinomas) present a great challenge in peripartum management of both the mother and the fetus. MATERIAL AND METHOD: We present a case of a 24-year old woman, with headache, papilledema, ataxia, and multiple cranial nerve weakness, di

Molecular determinants of left and right outflow tract obstruction.

Congenital heart defects represent the most common group of human birth defects; they occur in 0.8-1% of live births and in 10% of spontaneously aborted fetuses. Heart defects seen in newborns typically represent specific morphogenetic defects of individual chambers or regions of the heart, with the remaining portions of the heart developing relatively normally. These developmental defects are commonly compatible with the intrauterine circulation, where the pulmonary circulation and systemic circulation work in concert, resulting in adequate embryonic growth and development. After delivery, ho

[Prenatal ultrasound diagnosis in congenital abnormalities of the kidneys and efferent urinary tract from the pediatric nephrology viewpoint].

Antenatal foetal abnormalities are found in 0.5% of all pregnancies. 30-50% of these abnormalities are related to the urinary tract. Although congenital abnormalities of the urinary tract can be detected as early as in the 15th-18th weeks of gestation, most abnormalities are detected between the 28th-30th weeks of gestation. At this time irreversible damage of the kidneys has taken place and the foetus will not benefit from antenatal therapy. On the other hand, a foetus with late onset of obstruction will benefit from antenatal intervention. Repeated ultrasonography is crucial for initiating f

[Prenatal ultrasound diagnosis of the kidneys and efferent urinary tract. Possibilities, applications and dangers].

Congenital abnormalities of the urinary tract can be detected as early as during the 15th-18th week of gestation. Nevertheless, most abnormalities are detected between the 28th-30th week of gestation. Antenatal fetal abnormalities vary between physiological dilatation to bilateral hydronephrosis and polycystic kidney disease. Abnormalities in antenatal fetal ultrasonography are found in 0.5% of all pregnancies, 30-50% of these abnormalities are related to the urinary tract. Though most of the abnormalities are diagnosed late in pregnancy, irreversible damage of the kidneys has already taken pl

Evaluation of fetal renal function: unreliability of fetal urinary electrolytes.

In the fetus with bilateral hydronephrosis it has been reported that a normal volume of amniotic fluid, absence of renal cortical cysts, urinary sodium less than 100 mEq./l., chloride less than 90 mEq./l. and osmolarity less than 210 mOsm. are prognostic factors indicative of good renal function, whereas oligohydramnios, cortical cysts and higher urinary levels of sodium, chloride or osmolarity suggest irreversible renal dysplasia. We report 5 cases in which the fetal urinary electrolytes were not predictive of ultimate renal function. In 3 instances fetal urinary electrolytes and osmolarity w

[Prenatal diagnosis of non-viable fetuses caused by developmental disorders--an indication for abortion for fetal reasons?].

Between October 1st 1983 and January 31st 1986 we diagnosed various anomalies on 116 foetuses at the Department of Obstetrics and Gynaecology, Klinikum Grosshadern, Munich. In 103 cases we made a correct diagnosis. This group of patients included 45 non-viable foetuses. Severe anomalies of the foetal central nervous system and lethal dwarfism were diagnosed correctly. On the other hand, antenatal diagnosis of bilateral nonfunctional kidneys and non-correctable cardiac malformations poses problems. We feel that in the case of a definitive prenatal diagnosis of an anomaly for which there is no k
