Traditional approaches to intrapartum fetal monitoring, based on interpretation of fetal heart rate (FHR) tracings, have high false positive rates for detection of fetuses at risk of birth asphyxia. Transabdominal Fetal Pulse Oximetry (TFO) promises to supplement FHR trace interpretation through non-invasive sensing of fetal blood oxygen saturation fSpO 2 from photoplethysmography (PPG) signals acquired through the maternal abdomen. However, the acquired signals, referred to as mixed PPG, contain contributions from both maternal superficial tissue layers and fetal tissue, as well as oth

BACKGROUND: Early postnatal hypothermia (EPH) remains a significant clinical concern in critically ill transported neonates. A previous regional study reported an alarming EPH incidence rate of 82.1%, with wide variation across delivery facilities, underscoring the urgent need for quality improvement (QI) in early body temperature management. This study aimed to reduce the percentage of EPH in critically ill transported newborns by 20% to 25% within 1 year.
METHODS: A standardized, evidence-based thermal care bundle and comprehensive staff training were implemented using multiple Plan-Do-Stud

The precision and effectiveness of nano-diagnostic platforms rely on the deliberate design of advanced nanomaterials, aiming to address the clinical challenge, such as sensitive diagnosis of infectious chorioamnionitis-associated fever (CAM) versus non-infectious epidural-related maternal fever (ERMF), while also expanding the scope of nano-diagnostic technologies to include bio-detection like glycomics, which are cohesively linked to disease but require complex procedures. Here, we designed a hierarchically channeled graphitized nanoarchitecture (HPGC-Z67) as a novel nano-diagnostic platform.

INTRODUCTION: Labour epidural analgesia is the most effective method for intrapartum pain relief and is associated with improved maternal outcomes. However, concerns have been raised regarding potential associations between labour epidural analgesia and adverse psycho-emotional outcomes in children. Evidence from large epidemiological studies is inconsistent and potential biological mechanisms remain unclear. Maternal immune activation during pregnancy may play a role. We aimed to investigate behavioural and psycho-emotional outcomes in children of mothers who received epidural analgesia durin

BACKGROUND: Continuous glucose monitoring (CGM) improves maternal and neonatal outcomes in pregnancies complicated by preconception diabetes. CGM measurements may help optimize intrapartum and postpartum glycemic management in pregnant women with GDM. We investigated the use of blinded CGM-derived metrics during labor, delivery, and postpartum supplementary to normally monitored glucose measures in women with GDM to study neonatal outcomes and postpartum glucose metabolism. METHODS: This prospective observational study evaluated 60 pregnant women with gestational diabetes mellitus (GDM) during

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a rare form of heart failure occurring during the last month of pregnancy or within five months postpartum. While levosimendan is considered beneficial in heart failure and cardiogenic shock, evidence supporting its use in PPCM is limited. This study investigated the prevalence of PPCM in Germany and evaluated outcomes associated with levosimendan use.
METHODS: Using national data from the German Statistical Office, all pregnant women diagnosed with PPCM and hospitalized between 2009 and 2022 were included. Patients were categorized into groups

BACKGROUND: Cesarean delivery is a major risk factor for postpartum hemorrhage. A priority for postpartum hemorrhage safety bundles is the assessment of hemorrhage and transfusion risk following cesarean delivery and the identification of patients at increased risk for postpartum hemorrhage requiring red blood cell transfusion. A prediction model based on antenatal and intrapartum risk factors could be used in a standardized manner among all patients when considering crossmatch or other preparatory measures prior to cesarean delivery.
OBJECTIVE: To develop a multivariable prediction model for

Postpartum hemorrhage remains a major obstetric emergency and one of the most important contributors to maternal morbidity and mortality worldwide. Effective management depends on early recognition, systematic evaluation, and rapid implementation of evidence-based therapeutic strategies. A structured clinical approach facilitates identification of the underlying cause and supports timely intervention to prevent progression to severe hypovolemia and organ dysfunction. Risk assessment should begin during the antenatal period, as several maternal and obstetric conditions increase the likelihood o

OBJECTIVE: Immune thrombocytopenic purpura (ITP) is a hematological disorder commonly affecting women of reproductive age, but limited studies explore the maternal morbidity in pregnant patients with ITP. This study aimed to investigate whether pregnant women with ITP are at higher risk of bleeding complications during and after delivery.
METHODS: We analyzed the National Inpatient Sample (NIS) database for pregnant patients admitted for delivery from 2016 to 2018. Patients were stratified into two cohorts: those with ITP and those without. The two cohorts were matched in a 1:1 ratio based on

INTRODUCTION: Data with a high level of evidence is lacking on the use of prophylactic uterotonic drug following vaginal delivery to prevent postpartum hemorrhage (PPH) among women who were considered high-risk for PPH. Our main objective was to compare the effectiveness of prophylactic carbetocin versus oxytocin in preventing PPH after vaginal delivery among women at high risk.
MATERIAL AND METHODS: We conducted a retrospective before-and-after single-center comparative study, including all high-risk women after vaginal births. Two consecutive 14-month periods were compared, where the prophy

AIMS/BACKGROUND: Although the efficacy of labor analgesia in pain relief is well-established, its impact on labor progression and maternal-neonatal outcomes remains inconclusive, particularly in parturients with gestational diabetes mellitus (GDM). This study aimed to examine the effects of labor analgesia on labor progression, delivery mode, and neonatal outcomes in parturients with GDM, thereby providing evidence to optimize clinical intervention strategies.
METHODS: A total of 201 GDM parturients meeting inclusion criteria at Beilun District People's Hospital of Ningbo City between 1 Janua

INTRODUCTION: Cardiotocography (CTG) interpretation is prone to inter-observer variability and may contribute to both missed fetal compromise and potentially avoidable intrapartum intervention. Physiological CTG interpretation (PCI) reframes fetal surveillance around fetal physiology and the intensity of hypoxic stress, but real-world outcome data after unit-wide implementation remain limited.
MATERIAL AND METHODS: Historical pre-post cohort study with case-mix adjustment at University Hospital Ulm, a tertiary perinatal center in Germany (~3200 births/year). We included term singleton pregnan

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes.
RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T

BACKGROUND: Peripartum hysterectomy is the removal of the uterus during a cesarean section, after vaginal delivery, or during puerperium. It is usually performed as a life-saving emergency during abnormal placentation, hemorrhage, or uterine rupture. It has a substantial role in the prevention of maternal mortality and morbidity. This study aimed to identify the determinants of emergency peripartum hysterectomy at Hawassa University Comprehensive Specialized Hospital, Ethiopia. METHODS: A hospital-based, unmatched case-control study was conducted among 66 cases and 198 controls with a case-to-

BACKGROUND: Although severe maternal morbidity (SMM), such as severe hemorrhage and sepsis, can occur from conception to 6 weeks postpartum, most surveillance studies in Canada are restricted to intrapartum cases, with rates approximating 17 per 1000 births. We aimed to characterize the frequency of SMM from conception to 6 weeks postpartum and describe its associations with maternal and clinical characteristics.
METHODS: We conducted a population-based cohort study of births at 20 or more weeks' gestation, using administrative data in Ontario (2012 to 2021), and identified SMM using the Cana

Disseminated intravascular coagulopathy (DIC) in pregnancy is a severe maternal morbidity that is associated with short- and long-term complications. However, information regarding the causes of DIC treatments and outcomes in pregnancy is based on local reports. To address this gap, 2 Scientific and Standardization Committees, the Women's Health Issues in Thrombosis and Hemostasis and Disseminated Intravascular Coagulation of the International Society on Thrombosis and Hemostasis, joined to establish an international global registry. This registry was developed to examine the current definitio

IMPORTANCE: Physiological changes in pregnancy pose unique challenges for cirrhosis and portal hypertension, particularly during the third trimester and intrapartum setting. Pregnancy may increase the risk of worsening of esophageal varices and life-threatening variceal hemorrhage. Therefore, it is important that clinicians recognize risk factors for appropriate referral and management for delivery planning to mitigate risks associated with acute variceal bleeding in pregnancy.
OBJECTIVE: Review preconception counseling, antepartum surveillance, and intrapartum management of patients with eso
