Pregnancy after renal transplantation is now more feasible, with documented rates of successful live births. However, pregnancy after renal transplantation is associated with an increased risk of maternal complications, including preeclampsia, graft dysfunction, hypertension, and diabetes, as well as adverse perinatal outcomes such as preterm birth and intrauterine growth restriction. Effective care requires a multidisciplinary approach. This report discusses a case of a primigravida patient who successfully delivered at term three years after kidney transplantation, with an emphasis on the cl

Extremely preterm births at the edge of viability are associated with high mortality and significant long-term morbidity. This is largely due to the sudden and involuntary shift from fetal placental support to neonatal lung breathing, accompanied by systemic organ injury. Between 22 and 24 weeks of gestation, the fetal lungs, brain, and metabolic regulatory systems are not biologically prepared for extrauterine life. As a result, extremely preterm infants are highly vulnerable to ventilator-induced lung injury, oxygen toxicity, hemodynamic instability, and inflammatory cascades that contribute

Pregnancy provides a unique physiological stress test for the cardiovascular system, during which, adverse pregnancy outcomes (APOs) can unmask latent susceptibility to future disease. Common complications, including hypertensive disorders of pregnancy (HDP), gestational diabetes, and preterm birth (delivery before 37 weeks' gestation), identify women at substantially higher long-term risk of cardiovascular morbidity and mortality compared with women without a history of APOs. These excess risks likely reflect the combined effects of pre-existing cardiometabolic and genetic susceptibility, as

Preeclampsia (PE) is a multifactorial hypertensive disorder of pregnancy and a leading cause of maternal and perinatal morbidity and mortality worldwide. Despite decades of research, its precise pathophysiology remains incompletely understood, though abnormal placentation, endothelial dysfunction, angiogenic imbalance, oxidative stress, and immune dysregulation are recognized contributors. Low-dose aspirin (LDA) has emerged as a safe and effective prophylactic intervention in high-risk pregnancies. It acts primarily by irreversibly inhibiting cyclooxygenase enzymes, restoring the thromboxane A

BACKGROUND: Reproductive outcomes following pregnancy in premenopausal women with breast cancer (BC) remain contentious, and few meta-analyses have adequately addressed these questions. This systematic review and meta-analysis aimed to provide the most up-to-date and comprehensive evidence on the subject.
METHODS: Ten electronic databases were searched in September 2024 using the terms "Breast Neoplasms" AND "Pregnancy OR Fertilization OR Parturition OR Fertility OR Obstetrics". Key inclusion criteria focused on reproductive outcomes in premenopausal women with BC compared to healthy populati

BACKGROUND: Obesity is an increasing problem, even in young women of reproductive age. Obesity has a negative impact on conception, the course of pregnancy, and neonatal outcomes. Caring for obese pregnant women has becoming an important aspect of standard prenatal care. The Guideline "Obesity and Pregnancy" of the German Society of Gynecology and Obstetrics aims to create evidence-based recommendations which can be used to improve the care of obese pregnant women. As obesity is a worldwide problem, many societies for obstetrics and gynecology have created national guidelines.
METHODS: We rev

BACKGROUND: Preeclampsia is a pregnancy-specific hypertensive disorder associated with an imbalance in circulating proangiogenic and antiangiogenic proteins. Preclinical evidence implicates microvascular dysfunction as a potential mediator of preeclampsia-associated cardiovascular risk.
METHODS: Women with singleton pregnancies complicated by severe antepartum-onset preeclampsia and a comparator group with normotensive deliveries underwent cardiac positron emission tomography within 4 weeks of delivery. A control group of premenopausal, nonpostpartum women was also included. Myocardial flow r

Cardiogenic shock (CS) occurs infrequently in pregnancy and has a high mortality rate. Medical treatment options are few, with limited evidence of efficacy. Temporary mechanical circulatory supports (tMCS) may play a key role in addressing this therapeutic lacuna. We report successfully managing second-trimester CS using an Impella 5.5 micro-axial pump. Our patient presented in the second-trimester with CS. Hemodynamic parameters indicated biventricular dysfunction (low cardiac index, low pulmonary artery pulsatility index). She received diuresis and inotropic support to optimize her fluid sta

Cardiovascular disease (CVD) is globally the leading cause of death and disability. Sex-specific causes of female CVD are under-investigated. Pregnancy remains an underinvestigated sex-specific stress test for future CVD and a hitherto missed opportunity to initiate prevention of CVD at a young age. Population-based studies show a strong association between female CVD and hypertensive disorders of pregnancy. This association is also present after other pregnancy complications that are associated with placental dysfunction, including fetal growth restriction, preterm delivery and gestational di

The rate of solid organ transplant in reproductive-aged patients has increased in the past 3 decades. Concurrently, the range of medical immunosuppressive agents has increased, making it safer for reproductive-aged individuals who have received transplants to attempt and continue a pregnancy. In this Consult, we review the general considerations and contemporary approach to medical and obstetrical management of pregnant solid organ transplant recipients, discuss the perinatal outcomes and incidence of graft rejection specific to the most common types of organ transplants, and provide managemen

OBJECTIVE: Women can develop posttraumatic stress disorder in response to experienced or perceived traumatic, often medically complicated, childbirth; the prevalence of these events remains high in the United States. Currently, no recommended treatment exists in routine care to prevent or mitigate maternal childbirth-related posttraumatic stress disorder. We conducted a systematic review and meta-analysis of clinical trials that evaluated any therapy to prevent or treat childbirth-related posttraumatic stress disorder.

More than half of pregnant women are usually affected by odontogenic pain affects. Pain often accompanies periapical or pulp infections and increases the risks to pregnant patients and their fetuses. The American Dental Association, in partnership with the American College of Obstetricians and Gynecologists, has offered a strong declaration reaffirming the significance of suitable and timely oral health care as an indispensable constituent of a healthy pregnancy. However, there is lack of knowledge about the use of antibiotics in endodontic treatment. Therefore, the present study would review

BACKGROUND: Gestational diabetes mellitus (GDM) is carbohydrate intolerance first recognised during pregnancy and associated with complications for mothers and babies. Probiotics are naturally occurring micro-organisms, which when ingested in adequate amounts, may confer health benefits. Evidence of the role of probiotics as treatment for GDM is limited.
OBJECTIVES: To evaluate the safety and effectiveness of probiotics in treating women with GDM on maternal and infant outcomes.

IMPORTANCE: The association of poor family functioning, a potent stressor, with child behavior is potentially long term and relevant for a person's well-being later in life. Whether changes in brain development underlie the associations with preadolescent behavior and help identify periods of vulnerability is unclear.
OBJECTIVE: To assess the associations of poor family functioning from pregnancy onward with cortical, white matter, and subcortical volumes, and to examine the extent to which, in particular, hippocampal volume mediates the association of prenatal parental environmental exposure

BACKGROUND: The negative synergy between poor nutritional status and infectious diseases is doubly detrimental in pregnancy. In Sierra Leone, maternal malnutrition is amongst the highest in the world, while maternal mortality is high at 1320/100,000 live births and stunting in under-five is 37.9%, ranked 110/132 worldwide. Maternal malnutrition has been associated with preterm birth, small-for-gestational age infants, and poor maternal outcomes. Infants born prematurely or small-for-gestational age experience higher mortality and are at risk for stunting and decreased cognitive performance. Nu

BACKGROUND: High caesarean rates are of concern given associated risks. International migrant women (women born abroad) represent a substantial proportion of women giving birth in high-income countries (HICs) and face social conditions that may exacerbate childbearing health risks. Among migrant women, emergency rather than planned caesareans, tend to be more prevalent. This method of delivery can be stressful, physically harmful and result in an overall negative birth experience. Research establishing evidence of risk factors for emergency caesareans in migrants is insufficient.

INTRODUCTION: Pregestational and gestational diabetes mellitus (PGDM; GDM) are significant health concerns because they are associated with an increased rate of malformations and maternal health complications.
METHODS: We reviewed the data that help us to understand the effects of diabetes in pregnancy.
RESULTS: Diabetic embryopathy can affect any developing organ system, but cardiovascular and neural tube defects are among the most frequent anomalies. Other complications include preeclampsia, preterm delivery, fetal growth abnormalities, and perinatal mortality. Neurodevelopmental studies o

There is an increasing evidence that prenatal and early postnatal stressors have life long impacts on physical and mental health problems. Animal studies have shown that this could include enduring changes to brain serotonin neurotransmission. In the present study, we tested whether perinatal adversity in humans has a long-term impact on brain serotonin neurotransmission in adulthood. Twenty-six healthy males, recruited from a 27-year longitudinal study, underwent a positron emission tomography scan with the tracer alpha-[¹¹C]methyl-L-tryptophan (¹¹C-AMT), as an index of serotonin synthesis ca

Uric acid is a terminal metabolite of the degradation of nucleotides, which increases their blood levels in patients with preeclampsia-eclampsia, increasing its synthesis by damage and death of trophoblastic cells in proliferation and decreased urinary excretion due a lower glomerular filtration rate and increased absorption in the proximal tubule. Hyperuricemia (> 4.5 mg/dL) is the first biomarker of the clinical chemistry considered as an early evidence of disease (< or = 20 weeks gestation). Uric acid concentrations are not only a criterion for establishing the correct diagnosis and the dif

This paper reviews the literature on obstetric complications (e.g. premature births and hypoxia at birth) and the later development of schizophrenia. Some forty studies and two meta-analyses are summarised concerning methods and results. The balance of evidence today supports an association between obstetric complications at different points of time: during foetal life, delivery, as well as during neonatal life. This is supported by other studies of early life risk factors, e.g. infections, maternal stress and famine. Presuming there is a causal relationship, the results may support the view t
