Multiple Sclerosis During Pregnancy and the Postpartum Period: Diagnostic and Multidisciplinary Challenges in Two Cases.

BACKGROUND: Pregnancy may complicate the recognition and evaluation of new neurological symptoms suggestive of a central nervous system demyelinating disease. Diagnostic uncertainty may arise when clinical progression, neuroimaging findings, and supportive investigations are incomplete or inconclusive. CASE PRESENTATION: We report two pregnant women with progressive focal neurological symptoms and radiological findings suggestive of a demyelinating disease. Case 1 involved a 27-year-old woman with a history of autoimmune spondyloarthritis who developed dizziness, gait instability, asymmetric

Metagenomic analysis of tongue samples from healthy subjects identifies distinct microbiome orotypes.

IMPORTANCE: Two-thirds of women in labor receive supplemental oxygen to reverse perceived fetal hypoxemia and prevent acidemia. Oxygen is routinely administered for category II fetal heart tracings, a class of fetal tracing used to designate intermediate risk for acidemia. This liberal use of oxygen may not be beneficial, particularly because neonatal hyperoxygenation is harmful. OBJECTIVE: To test the hypothesis that room air is noninferior to oxygen in improving fetal metabolic status among patients with category II fetal heart tracings. DESIGN, SETTING, AND PARTICIPANTS: This was a random

Modern neuraxial labor analgesia: techniques, pharmacologic strategies, and maternal-fetal outcomes.

Neuraxial labor analgesia (NLA) remains the gold standard for intrapartum pain management, providing superior analgesia, higher maternal satisfaction, and improved fetal safety compared with systemic or inhalational techniques. Recent advances in initiation methods- including epidural analgesia, combined spinal-epidural, and dural puncture epidural techniques-as well as optimized pharmacological regimens using low-concentration local anesthetics combined with lipophilic opioids, have improved analgesic onset, sacral coverage, and block uniformity while minimizing motor blockade. Safety and biodistribution of intrathecal administration of mesenchymal stem cells (MSCs) and neurotrophin-releasing nanoparticles in a porcine CSF-guided delivery model for amyotrophic lateral sclerosis (ALS) drug discovery.

Amyotrophic lateral sclerosis (ALS) is a multifactorial neurodegenerative disorder that complicates the identification of effective therapeutic targets. The potential of stem cells and neurotrophins as promising candidates has become increasingly evident, owing to their neuroprotective and anti-inflammatory properties. In this study, a preclinical evaluation of the safety and biodistribution of mesenchymal stromal/stem cells (MSCs) combined with neurotrophin-releasing polyelectrolyte nanoparticles (NTs) was conducted in a porcine intrathecal delivery model relevant to ALS therapy development.

Cesarean delivery for placenta previa.

Placenta previa is an absolute indication for cesarean delivery and is associated with serious risks for maternal and neonatal health. At delivery, its prevalence is 0.5% to 1%, after being observed in up to 10% at the mid-trimester scan. Risk is highest with prior cesarean delivery and recurrence after a prior placenta previa is 4% to 8%. Twin gestations have a higher absolute prevalence (3.9 vs 2.8 per 1000 live births), and assisted reproduction carries a 6-fold risk compared to spontaneous conception. Maternal morbidity is dominated by hemorrhage: antepartum bleeding occurs in 40% to 60% a

The amniotic fluid lactate level as a possible diagnostic tool for uterine fatigue, leading to prolonged labour.

OBJECTIVES: Arrested labour is a common and dangerous complication of delivery leading to adverse outcomes for both the mother and child. Early identification of slow labour is essential for managing the condition. Currently, clinical assessment consists of vaginal examination, abdominal palpation, and the use of a partogram, but there are no well-described, objective and quantifiable markers. One possible cause of labour arrest is uterine fatigue. The aim of the study was to assess the feasibility of using lactic acid concentration in amniotic fluid as a marker of uterine fatigue contributing

Velamentous cord insertion in singleton pregnancies: ultrasound diagnostic accuracy, risk factor analysis, and perinatal outcomes.

BACKGROUND: Velamentous cord insertion (VCI) complicates 0.9-1.9% of singleton pregnancies and is associated with adverse perinatal outcomes due to unprotected fetal vessels traversing the membranes. Color Doppler ultrasound enables prenatal diagnosis, yet systematic evaluation of diagnostic performance and risk factor association models remains limited. We hypothesized that systematic second-trimester screening achieves superior diagnostic accuracy before 28 weeks, and that identification of clinical risk factors associated with VCI would enhance understanding of its etiology and inform scree

Obstetrical and Neonatal Outcomes of Term Pregnancies Complicated by Unstable Fetal Lie.

BACKGROUND: Unstable fetal lie, characterized by frequent changes in fetal presentation during late pregnancy, poses challenges in pregnancy management and delivery planning. We aimed to examine the obstetrical and neonatal outcomes associated with term pregnancies complicated by an unstable fetal lie. METHODS: This retrospective cohort study included data recorded during 2012-2022 of women with term singleton pregnancies with an unstable fetal lie who attempted vaginal delivery after stabilizing to vertex. We compared their obstetrical and neonatal outcomes in a 1:2 ratio to those of women w

Predictors of General Anesthesia Receipt During Cesarean Deliveries in the U.S. Military Health System: A Retrospective Cohort Study.

INTRODUCTION: Clinical practice guidelines indicate general anesthesia should be avoided for cesarean deliveries, when possible. In the U.S. Military Health System, understanding multifaceted predictors of general anesthesia for cesarean deliveries can be used to enhance value-based obstetric care efforts. MATERIALS AND METHODS: This retrospective observational cohort study included adult patients who experienced a cesarean delivery in a military treatment facility between January 2018 and October 2024. Potential covariates included patient characteristics, diagnoses, and pregnancy care infor

Velamentous Cord Insertion With Vaginal Delivery: A Case Report and Review of the Literature.

Velamentous cord insertion (VCI) is a rare placental abnormality in which the umbilical cord enters the chorionic membranes instead of the placenta, leaving the umbilical vessels unprotected. This pathological condition carries an increased risk of adverse perinatal outcomes, such as fetal growth restriction, preterm delivery, abnormal fetal heart rate patterns during labor, and perinatal morbidity, which often lead healthcare professionals to choose cesarean section to reduce postnatal risks. Early prenatal diagnosis using color Doppler imaging and comprehensive ultrasound is of utmost import

Reirradiation treatment effects in the clinic (ReTEC) proposal - proof of concept based on spinal cord dose tolerance for reirradiation with stereotactic body radiotherapy.

BACKGROUND: Since the inception of spinal radiosurgery decades ago, the factors influencing spinal cord recovery have remained unclear. Elucidating these factors would aid in reirradiation planning and delivery. We describe modeling reirradiation normal tissue complication probabilities (NTCP) from compiled published data, akin to the collaborative Quantitative Analysis of Normal Tissue Effects in the Clinic (QUANTEC) and High Dose per Fraction, Hypofractionated Treatment Effects in the Clinic (HyTEC) projects. PURPOSE: Simple dose-time recovery factors for the spinal cord serve as a basis to

Complicated Epidural Analgesia With Undiagnosed Spinal Tumor: A Case Report.

Neuraxial anesthesia is commonly used for labor analgesia, and postpartum back pain is often experienced, with or without an epidural. Although it usually improves with supportive care, back pain coupled with other findings (such as radiculopathy, bowel or bladder dysfunction, or inability to ambulate) warrants further investigation post epidural. Here, we present a 37-year-old patient who experienced inadequate pain relief with multiple epidural blocks during labor and worsened back pain after delivery, and was later determined to have a lumbar myxopapillary ependymoma. Neuraxial anesthesia i

The Pleiotropic Therapeutic Perspectives of GLP-1 and GIP Receptor Agonists in Spinal Cord Injury: A Narrative Review.

Spinal cord injury (SCI) constitutes a major global health challenge. The pathophysiology of SCI involves many aspects. Current treatments, such as early decompression and glucocorticoids, target single pathways and show limited efficacy with safety concerns. In this context, interventions based on the incretin system are now considered attractive candidates for SCI intervention. This review comprehensively outlines the mechanisms underlying microenvironmental imbalance following SCI and explores glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) receptor ag

Diabetes-Related Metabolic Osteoarthritis: Advanced Glycation-Collagen Axis, Cartilage Stiffening, and Biomaterials-Based Therapeutic Strategies.

Diabetes mellitus, particularly type 2 diabetes, has been associated in some studies with increased osteoarthritis (OA) risk, greater symptom burden, and structural progression; however, epidemiological evidence remains heterogeneous, and the extent to which diabetes independently contributes to OA after accounting for obesity, adiposity, physical inactivity, and other metabolic confounders remains debated. In metabolically susceptible patients, chronic hyperglycemia, insulin resistance, carbonyl stress, and low-grade inflammation may contribute to disruption of the joint microenvironment. One

Case Report: Postpartum diagnosis and microsurgical management of a cervicothoracic intramedullary spinal ependymoma clinically manifesting during the third trimester of pregnancy.

BACKGROUND: Intramedullary spinal cord ependymomas are the most common intramedullary tumours in adults. However, clinical manifestation during pregnancy is exceptionally rare, and diagnosis may be delayed because pain, sensory disturbances, and muscle weakness are often mistakenly attributed to the common musculoskeletal changes associated with pregnancy. CASE DESCRIPTION: A 29-year-old multiparous woman developed progressive back pain, numbness, and weakness in both lower extremities at 30 weeks of gestation. She was initially managed conservatively. Placental Adaptations Under Severe Anemia: Neonatal Outcomes at the Crossroads.

Anemia is a major global health problem, particularly in developing countries such as India, due to the greater prevalence of dietary iron deficiency, hemoglobinopathies, macronutrient deficiencies, and infections such as malaria, human immunodeficiency virus (HIV), and hookworm infestation. Anemia-induced hypoxia triggers a range of vascular and structural changes in the placenta, potentially leading to complications for both the mother and the fetus. Studies have shown that placentae from anemic mothers often exhibit hypertrophy, characterized by increased placental weight relative to fetal
