Beyond the Usual Suspects: Gastrointestinal Histoplasmosis as a Marker of Undiagnosed Common Variable Immunodeficiency.

A 28-year-old woman presented with chronic diarrhea, weight loss, and recurrent gastrointestinal infections. Intestinal biopsies unexpectedly revealed Histoplasma capsulatum, prompting immunologic evaluation that demonstrated severe hypogammaglobulinemia and reduced B-cell counts, consistent with common variable immunodeficiency. She improved with intravenous immunoglobulin and antifungal therapy. This case highlights gastrointestinal histoplasmosis as a potential early marker of undiagnosed common variable immunodeficiency and underscores the importance of considering primary immunodeficiency

Isolated hepatic histoplasmosis in an immunocompetent patient.

Isolated involvement of the liver in histoplasmosis is rare. We report a case of an immunocompetent patient in their mid-40s who presented with prolonged fever and elevated liver enzymes. Liver biopsy revealed granulomas, and histoplasmosis was confirmed by Periodic Acid-Schiff staining and positive urinary Histoplasma antigen test. No other organ involvement was found. Treatment with Amphotericin B and Itraconazole resulted in complete recovery, and Urinary Histoplasma antigen was negative 6 weeks after treatment initiation. This report highlights the importance of considering histoplasmosis

Misdiagnosis and Coinfection of Localized Pulmonary Histoplasmosis with Pulmonary Tuberculosis: A Systematic Review of Published Cases.

Pulmonary histoplasmosis is often misdiagnosed as or coinfected with pulmonary tuberculosis (TB). This study aims to analyze the misdiagnosis or co-occurrence of published cases of pulmonary TB and pulmonary histoplasmosis. Cases of histoplasmosis with dissemination were excluded, as it affects other organs. Systematic research was conducted using PubMed, EBSCOhost, ProQuest, BioRxiv, and MedRxiv databases. Twenty-seven articles were included, covering a total of 51 cases. Males were predominantly affected, with a median age of 54 years. Exposure to caves and farming occupations were identifie

Disseminated histoplasmosis presenting as primary adrenal insufficiency in an immunocompetent host in the United States: a case report.

BACKGROUND: Histoplasma capsulatum is a dimorphic fungus that is mostly seen in immunocompromised individuals. Its presentation ranges from acute pulmonary infections to disseminated disease that can affect multiple organ systems, including the adrenal glands. Dissemination is uncommon in immunocompetent hosts, but involvement of the adrenal glands is largely reported in those with a competent immune system. Subsequent primary adrenal insufficiency is a rare finding, especially in the United States with only two such cases reported before. CASE PRESENTATION: We report the case of 62-year-old i

Multiple Opportunistic Infections as the Initial Presentation of Human Immunodeficiency Virus (HIV)/Acquired Immunodeficiency Syndrome (AIDS) in an Ecuadorian Immigrant.

Advanced human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) continues to occur in those with social, educational, and cultural barriers to accessing healthcare. A 27-year-old Ecuadorian man presented with vomiting, epigastric pain, fever, weakness, and weight loss. He had advanced AIDS, with opportunistic infections including disseminated histoplasmosis, cerebral toxoplasmosis, and JC virus brain infection. Despite prior repeated contact with local, US-based healthcare, sexual history and HIV screening were never performed. Missed opportunities to identify HIV infecti

A silent aggressor: disseminated histoplasmosis with laryngeal and adrenal involvement in a diabetic patient.

SUMMARY: A 65-year-old male with poorly controlled type 2 diabetes mellitus presented with 7 months of progressive hoarseness of voice, fever, anorexia, cough, and weight loss. Just before admission, he developed drowsiness and memory impairment. Examination showed a drowsy but stable patient without meningeal signs and hyperpigmentation. Persistent hoarseness prompted laryngoscopy, which revealed a vocal cord nodule. Imaging demonstrated bilateral adrenal enlargement with hypodense lesions in liver, pancreas, spleen, and kidney. Hormonal evaluation showed preserved adrenal function. Histopath

Shunt nephritis with right-sided infective endocarditis caused by Staphylococcus epidermidis in a patient with prolonged pyrexia and renal dysfunction.

Shunt nephritis is a rare immune-complex mediated glomerulonephritis associated with infected ventriculo-atrial (VA) shunts and is increasingly uncommon in contemporary practice. We report a diagnostically challenging case of shunt nephritis with concomitant right-sided infective endocarditis caused by Staphylococcus epidermidis, presenting with prolonged pyrexia and renal dysfunction. A 62-year-old woman presented with an 18-month history of fever, weight loss, and malaise, initially treated as recurrent urinary tract infection and later as histoplasmosis without improvement. She had a prior

Infectious adverse events in patients treated with biotherapies in a tropical Amazonian context, French Guiana.

BACKGROUND: Data on infectious complications associated with biologic therapies in tropical settings are scarce. French Guiana, an Amazonian French overseas territory, combines an important burden of infectious diseases with a high use of biologic therapies. Our objective was to assess the incidence, spectrum, and risk factors for infections in patients treated with biotherapies for inflammatory and autoimmune diseases in French Guiana. METHODS: We performed a multicentre retrospective study of adult patients receiving biotherapies between January 2021 and December 2023 in the French Guiana U

Spore-casting outcomes: Histoplasma serology in lung nodule risk stratification.

BACKGROUND: Lung nodule evaluation is a vital practice in pulmonary medicine. While biopsy remains the diagnostic gold standard, it carries procedural risks. In regions endemic for fungal disease, low-cost, noninvasive testing may help reduce unnecessary procedures. AIM: We seek to evaluate the association between positive endemic histoplasma antibody titers and non-cancerous pathology in patients with pulmonary nodules. Methods: We conducted a simple, retrospective, single-center study of patients diagnosed with solitary (R91.1) or multiple (R91.8) pulmonary nodules. Data collected included

Pulmonary nodules in a histoplasmosis-endemic area: Diagnostic challenges and lessons from a case series.

INTRODUCTION: In histoplasmosis-endemic regions, pulmonary nodules often raise concern for malignancy. Distinguishing benign infectious granulomas from neoplastic lesions while limiting invasiveness is a major diagnostic challenge. MATERIALS AND METHODS: We report a case series of 11 patients diagnosed with nodular pulmonary histoplasmosis at the University Hospital of Guadeloupe, a French West Indies institution located in an endemic area. Clinical, radiologic, microbiologic, and histopathologic data were retrospectively reviewed. RESULTS: All 11 patients presented with solid pulmonary nodu

Broncholithiasis: Diagnostic and Therapeutic Insights From a Contemporary Case Series of Five Patients.

Broncholithiasis refers to the presence of calcified or ossified material within the tracheobronchial tree. This rare condition is more frequently reported in regions endemic for tuberculosis and histoplasmosis, where necrotizing granulomatous lymphadenitis may lead to dystrophic calcification and eventual erosion of calcified lymph nodes into the bronchial lumen. Clinical presentation is heterogeneous and ranges from incidental findings to hemoptysis, chronic cough, recurrent infection, and post-obstructive complications. We conducted a retrospective review of five patients diagnosed with bro

Histoplasma Urinary Antigen for the Diagnosis Histoplasmosis in Non-HIV Individuals.

BACKGROUND: Histoplasmosis is an endemic mycosis in the Americas. While urinary Histoplasma antigen enzyme immunoassay testing (HAET) has been validated mainly in HIV-infected patients with disseminated disease, its performance in HIV-negative populations with heterogeneous clinical presentations is unclear. OBJECTIVES: To describe the performance of urinary HAET for the diagnosis of histoplasmosis in a cohort of Brazilian HIV-negative patients. MATERIAL AND METHODS: We conducted a retrospective study at Einstein Hospital Israelita (Brazil) including HIV-negative patients whose urine samples

Schlaegel lines: History, etiology, multimodal imaging, and differential diagnosis of curvilinear streaks in chorioretinal disorders.

Schlaegel lines (SL) represent distinctive curvilinear patterns of chorioretinal atrophy that have emerged as a distinctive, yet non-pathognomonic, funduscopic sign across diverse ocular disorders. First described as related to presumed ocular histoplasmosis syndrome in the early 1980s in Histoplasma-endemic regions, these peripheral fundus lesions have since been documented across numerous other etiologies. The majority of SL arise in the context of inflammatory chorioretinal disorders, with idiopathic multifocal choroiditis and punctate inner choroiditis constituting the most frequent underl

Subcarinal Mass: An Unusual Presentation of Granulomatosis With Polyangiitis.

BACKGROUND Granulomatosis with polyangiitis (GPA) is a small-to-medium vessel necrotizing vasculitis that affects multiple organ systems. It commonly manifests as glomerulonephritis and inflammation involving the respiratory tract. Rarely, ocular, cutaneous, neurological, and musculoskeletal manifestations occur. Mass lesions are uncommon in GPA. Our patient had a subcarinal mass and was diagnosed with GPA. In our literature review, we identified several reports of mediastinal masses; however, no specific cases of subcarinal masses were found. CASE REPORT A 22-year-old woman presented with syn

Acute pulmonary histoplasmosis in a Belgian traveller returning from Suriname diagnosed with NGS: a case report.

We describe the case of a 48-year-old Belgian woman who developed fever, dry cough and dyspnoea shortly after returning from a two-week trip to Suriname. CT scan of the thorax revealed 2 nodular masses with surrounding ground glass alterations in the right lung as well as multiple hilar and mediastinal lymphadenopathies. Bronchoscopy with broncho-alveolar lavage and endobronchial ultrasound-guided transbronchial needle aspiration of thoracic lymph nodes were performed. Broncho-alveolar lavage fluid showed no abnormalities, including a negative PCR for Aspergillus spp., and negative cryptococca

A Case of Native Joint Septic Arthritis Due to Histoplasma Capsulatum in a Patient on Adalimumab.

Disseminated histoplasmosis is primarily a disease of the immunosuppressed, with clinically significant symptoms most commonly observed in the lungs, intra-abdominal organs, or central nervous system. We present a case of native joint septic arthritis due to Histoplasma capsulatum in an 88-year-old male on adalimumab for psoriasis with no other etiologies for immunosuppression. The patient presented with four weeks of right knee pain without inciting trauma. Synovial fungal cultures grew Histoplasma capsulatum. He underwent incision and drainage with anterior compartment synovectomy along with
