HIV and Hickam's Dictum: Multiple Opportunistic Infections.

Opportunistic infections are common in patients with acquired immunodeficiency syndrome (AIDS) or advanced human immunodeficiency virus infection when adherence to anti-retroviral is poor. We present a case report of multiple opportunistic infections simultaneously present in a patient with AIDS who presented with fevers, abdominal pain, and diarrhea. He was ultimately diagnosed with Streptococcus pneumoniae pneumonia, oral thrush, esophageal histoplasmosis, cytomegalovirus esophagitis and herpes simplex esophagitis. This case is a reminder of the severity of illness in AIDS and the need for t

Comparative Analysis of Oral and Oropharyngeal Mucosal Lesions of American Tegumentary Leishmaniasis and Other Infectious Granulomatous Diseases and Squamous Cell Carcinoma.

American tegumentary leishmaniasis (ATL) and other infectious granulomatous diseases (IGDs) may present with oral/oropharyngeal mucosal lesions (OOPML). IGD-OOPML can result from fungal, parasitic, or bacterial infections, and squamous cell carcinoma (SCC) represents the main differential diagnosis. ATL, other IGD, and SCC share overlapping clinical and epidemiological features, making diagnostic suspicion challenging. This study compared sociodemographic and clinical characteristics among ATL, other IGD, and SCC. Descriptive, comparative, and multivariable logistic regression analyses were pe

Multiple liver lesions as an extraordinary finding of disseminated histoplasmosis in an immunocompromised patient.

Histoplasmosis is an opportunistic infection originating from endemic regions. Current factors such as immigration and immunosuppression associated with new therapeutic approaches increase the likelihood of disease dissemination. Hepatic involvement is uncommon and leads to an extensive differential diagnosis, potentially delaying diagnosis and resulting in a fatal outcome. We present the case of a male patient with human immunodeficiency virus (HIV) infection, whose clinical onset occurred in the context of disseminated histoplasmosis with hepatic involvement.

New 2030 Global Targets for Histoplasmosis from International Society for Human and Animal Mycology (ISHAM) 2025 Histoplasmosis Working Group.

Histoplasmosis remains a neglected yet deadly fungal infection, disproportionately affecting persons living with HIV/AIDS and other immunocompromised populations in endemic regions. Despite the World Health Organization's designation of Histoplasma as a high-priority pathogen, the disease remains underdiagnosed and excluded from national surveillance systems, resulting in delayed treatment and high death rates. To coordinate a global response, the International Society for Human and Animal Mycology convened a Histoplasmosis Working Group during its 2025 congress in Brazil. Experts engaged in s

Disseminated Histoplasmosis in a Patient With HIV Infection Untreated for 23 Years.

Histoplasmosis is a potentially life-threatening opportunistic fungal infection in patients with advanced human immunodeficiency virus (HIV), particularly in endemic regions such as the Ohio and Mississippi River valleys. Despite advances in antiretroviral therapy, delayed diagnosis, lack of linkage to care, and treatment nonadherence continue to contribute to severe opportunistic infections in individuals with untreated HIV. We present the case of a 43-year-old woman with a 23-year-long history of untreated HIV infection who presented with fever, weight loss, diarrhea, and malaise. Laboratory

Predictors of mortality in hospitalized patients with disseminated histoplasmosis.

INTRODUCTION: Histoplasmosis is an important opportunistic infection among People Living with HIV (PLHIV) and still presents significant mortality. Reliable prognostic markers are essential to guide early management and appropriate clinical decision-making. This study aimed to evaluate predictors of death at admission among patients with disseminated histoplasmosis. METHODS: This was a single-center retrospective cohort study including adults with HIV and disseminated histoplasmosis admitted to a Brazilian tertiary hospital between 2009 and 2023. The primary outcome was in-hospital mortality.

Predictors of mortality among individuals with advanced HIV disease in a contemporary Brazilian cohort.

OBJECTIVE: To identify clinical characteristics and risk factors associated with mortality, with a focus on opportunistic infections (OIs), in patients with advanced HIV in Brazil. METHODS: A prospective cohort study was conducted in five Brazilian tertiary hospitals, including 419 adults with advanced HIV. Baseline demographic and clinical data were collected during hospital admission, and participants were screened for tuberculosis, cryptococcosis, and histoplasmosis using rapid diagnostic tests. Participants were followed for 90 days to assess mortality, with causes of death classified usi

Clinical and laboratory predictors of mortality in HIV/AIDS patients with disseminated histoplasmosis: A cohort study in a hyperendemic region of Northeast Brazil.

Disseminated histoplasmosis (DH) is a severe opportunistic infection in people living with HIV/AIDS (PLWHA). We aimed to identify predictors of in-hospital death among PLWHA with DH and to assess the prognostic value of novel biomarkers. A cohort study was conducted at the São José Hospital of Infectious Diseases in Fortaleza, Brazil (March 2023-December 2024). Sixty-one patients were included; 15 (24.6%) died during hospitalization. Mortality was strongly associated with peripheral blood detection of Histoplasma capsulatum (relative risk, RR = 5.05; 95% confidence interval, CI = 2.30-11.10).

Research priorities for advanced HIV disease in Latin America and the Caribbean region: a modified Delphi study.

INTRODUCTION: Advanced HIV disease (AHD) remains a leading cause of mortality in Latin America and the Caribbean (LAC), driven by late diagnosis, treatment gaps and structural barriers, particularly among key populations and children. Persistent disparities in healthcare access, stigma, and limited health system capacity highlight the need for targeted research to improve AHD outcomes in the region. METHODS: The modified Delphi process to prioritize AHD research questions in LAC was conducted between August and November 2024. Systematic reviews, expert consultations and two Delphi rounds invo

Histoplasmosis: a missing piece in the global efforts to end HIV deaths.

Histoplasmosis is a leading opportunistic infection and cause of mortality in people with advanced HIV disease. Yet, histoplasmosis is largely neglected in the global HIV response. Previously considered endemic only in the region of the Americas, histoplasmosis is now recognised to be distributed throughout much of sub-Saharan Africa, southeast Asia, and other parts of the world. Making a diagnosis is the greatest challenge. Commercial Histoplasma antigen tests are highly sensitive and specific and provide the best tool for early diagnosis of disseminated histoplasmosis, but global access is e

Challenges in diagnosing mucocutaneous leishmaniasis caused by Leishmania (Mundinia) martiniquensis mimicking oral histoplasmosis in a patient with advanced HIV disease from Southern Thailand.

Differentiation between leishmaniasis and histoplasmosis remains a significant diagnostic conundrum, especially in immunosuppressed patients, due to overlapping clinical presentations and histopathological features. We report a case of a 53-year-old Thai man with advanced HIV disease from Southern Thailand who presented in 2020 with a chronic palate ulcer and hard palate perforation, initially diagnosed as histoplasmosis and treated with itraconazole for a year. Three years later (2023), the patient developed systemic symptoms, and biopsies confirmed visceral leishmaniasis. Given the increasin

Clinical characteristics and mortality risk factors among 400,509 individuals diagnosed with AIDS in Brazil: A nationwide observational study.

OBJECTIVES: AIDS remains a major global public-health challenge, with persistent care-cascade gaps driving preventable deaths, especially in LMICs. In Brazil, uneven progress and late presentation reflect social and regional inequities. We used the national notifiable diseases system (SINAN) to profile AIDS at diagnosis and identify mortality risk factors. STUDY DESIGN: National retrospective cohort study using the SINAN from 2007 to 2022. METHODS: We included all SINAN records of AIDS diagnoses in Brazil between 2007 and 2022. Extracted variables comprised demographics (age, sex, education,

Predictors of mortality in individuals with disseminated histoplasmosis and advanced HIV disease.

Disseminated histoplasmosis remains a major cause of morbidity and mortality among people living with advanced HIV disease (AHD), particularly in Latin America, where delayed diagnosis and limited access to optimal antifungal therapy persist. Accurate tools to assess disease severity and predict mortality are essential to guide clinical decision-making, including hospitalization, intensive care unit admission, and treatment strategies. We conducted a retrospective observational cohort study of hospitalized adults with AHD (CD4 ≤ 200 cells/mm³) and a first episode of probable or proven dissemin

Urine detection of Histoplasma antigens among symptomatic people living with HIV (PLWH) in Mato Grosso do Sul, Brazil: A comparative study of EIA and lateral flow assay.

Histoplasma spp., thermally dimorphic fungus acquired from environmental exposure, is associated with high mortality in people with advanced HIV disease. Early diagnosis remains challenging because conventional mycological methods have limited sensitivity, reinforcing the importance of urinary antigen detection. We conducted a cross-sectional study between 2022 and 2023 at a tertiary referral hospital in Campo Grande, Mato Grosso do Sul, Brazil, to assess the agreement between two urinary Histoplasma antigen detection platforms and to evaluate Histoplasma antigenuria among symptomatic people l

Disseminated histoplasmosis in a late post-liver transplant recipient: a case report and literature review.

Histoplasmosis is an endemic mycosis in certain regions of Brazil and represents a rare opportunistic infection in solid organ transplant recipients. This report describes the case of a 68-year-old female liver transplant recipient (due to Caroli disease) who presented with abdominal pain, nausea, vomiting, and a distended, tender abdomen. Abdominal computed tomography revealed parietal thickening of the bowel wall, and colonoscopy identified polymorphic ulcers and ileocecal stenosis. Given the suspicion of tuberculosis, chest computed tomography was performed, which demonstrated pulmonary opa
