Cesarean delivery for placenta previa.

Placenta previa is an absolute indication for cesarean delivery and is associated with serious risks for maternal and neonatal health. At delivery, its prevalence is 0.5% to 1%, after being observed in up to 10% at the mid-trimester scan. Risk is highest with prior cesarean delivery and recurrence after a prior placenta previa is 4% to 8%. Twin gestations have a higher absolute prevalence (3.9 vs 2.8 per 1000 live births), and assisted reproduction carries a 6-fold risk compared to spontaneous conception. Maternal morbidity is dominated by hemorrhage: antepartum bleeding occurs in 40% to 60% a

Velamentous cord insertion in singleton pregnancies: ultrasound diagnostic accuracy, risk factor analysis, and perinatal outcomes.

BACKGROUND: Velamentous cord insertion (VCI) complicates 0.9-1.9% of singleton pregnancies and is associated with adverse perinatal outcomes due to unprotected fetal vessels traversing the membranes. Color Doppler ultrasound enables prenatal diagnosis, yet systematic evaluation of diagnostic performance and risk factor association models remains limited. We hypothesized that systematic second-trimester screening achieves superior diagnostic accuracy before 28 weeks, and that identification of clinical risk factors associated with VCI would enhance understanding of its etiology and inform scree

Vasa previa guidelines and their supporting evidence.

INTRODUCTION: Vasa previa (VP) is a serious pregnancy complication in which fetal vessels, unprotected by the umbilical cord, run across or within close proximity to the internal cervical os, which can potentially result in fetal exsanguination in the event of membrane rupture. There is global consensus that women with antenatally diagnosed VP should have caesarean delivery prior to onset of labour to prevent the catastrophic complications of VP. However, there is variability in the approach to management of these women antenatally, particularly regarding hospitalisation and timing of steroid

Benckiser's hemorrhage: An uncommon cause of genital hemorrhage occurring in the third trimester - A case report.

INTRODUCTION: Benckiser's hemorrhage is a rare obstetric emergency characterized by massive genital hemorrhage during the third trimester. This condition necessitates urgent fetal extraction. Prenatal diagnosis is not routinely recommended. This article reports a case where vasa previa was diagnosed antenatally, but the pregnancy outcome was unfavorable. CASE REPORT: A 32-year-old woman, G3P1, presented at 32 weeks gestation with pelvic pain resembling uterine contractions. A transvaginal ultrasound revealed a suspected vascular anomaly near the internal cervical os. However, at 35 WG, she re

Fetoscopic Laser Ablation of Type II Vasa Previa-Case Report and Systematic Review.

Vasa previa is a rare but potentially fatal obstetric condition in which fetal vessels traverse the internal cervical os without the protection of placental tissue or Wharton's jelly, making them highly vulnerable to rupture during labor or spontaneous membrane rupture. This can result in rapid fetal exsanguination and death. While current management involves planned cesarean delivery before the onset of labor, fetoscopic laser ablation (FLA) has recently emerged as a minimally invasive alternative for selected cases, particularly type II and III vasa previa, allowing for pregnancy prolongatio

Late Pregnancy Bleeding.

Late pregnancy bleeding, any vaginal bleeding that occurs after 20 weeks' gestation, can be due to obstetric emergencies or benign etiologies. The amount of blood loss does not always correlate with seriousness of the etiology, and the source of bleeding is often not immediately obvious. Even small amounts of blood loss should be evaluated carefully because this may be the initial symptom of emergent bleeding. Emergent obstetric etiologies include placental abruption, placenta previa, vasa previa, and uterine rupture. Placental abruption rarely presents with the classic triad of vaginal bleedi

Pre-operative ultrasound before cesarean delivery: reviewing findings during the first surgical time-out to improve maternal and infant safety.

Twenty-nine million cesarean deliveries performed every year. Although often regarded as routine, even elective procedures can lead to serious complications for mothers and infants. Modern patient safety emphasizes that no incision should be undertaken without knowing what lies beneath, a principle codified by the World Health Organization in its Surgical Safety Checklist. A preoperative ultrasound represents an extension of this principle to obstetrics. Several complications exemplify face-valid safety signals: incision through an anterior placenta with maternal hemorrhage and neonatal anemia

Association of Resolved Low-Lying Placentation With Risk of Postpartum Hemorrhage.

OBJECTIVE: To evaluate whether individuals who have a placenta previa or low-lying placenta that resolves before delivery are at increased risk of postpartum hemorrhage and postpartum hemorrhage-related morbidity. METHODS: This was a prospective, multicenter, 1:3 matched cohort study of pregnant individuals with singleton gestations diagnosed with low placentation by transvaginal scan at 19 to 23 weeks of gestation between January 2021 and December 2023 at nine academic maternity centers (exposed participants). Unexposed participants were those with a normally located placenta, matched in a 1

Prenatal diagnosis and postnatal outcome of Type-III vasa previa: systematic review of literature.

OBJECTIVE: Type-III vasa previa (VP) is a rare form of VP, not necessarily associated with other placental or vascular anomalies, in which aberrant vessels run from the placenta to the amniotic membranes, near the internal cervical os, before returning to the placenta. Early diagnosis of Type-III VP is important but technically challenging. The objective of this study was to gather the current available evidence on the perinatal diagnosis and outcome of Type-III VP. METHODS: A systematic review of the literature on the perinatal diagnosis of atypical Type-III VP was carried out in PubMed, MED

Mirroring Perinatal Outcomes in a Romanian Adolescent Cohort of Pregnant Women from 2015 to 2021.

Although the rates of adolescent pregnancies appear to have dropped according to the World Health Organization (WHO), the decrease in the age of the first menarche and better nutrition seems to contribute to the otherwise high rate of adolescent pregnancy worldwide, despite the efforts of different organizations to improve upon this trend. We conducted a population-based retrospective cohort study from January 2015 to December 2021 using our hospitals' database. We totaled 2.954 adolescent and 6.802 adult pregnancies. First, we compared younger adolescents' outcomes with those of older adolesc

Inpatient versus outpatient management of prenatally diagnosed vasa praevia: A systematic review and meta-analysis.

OBJECTIVE: Vasa praevia is a serious pregnancy complication that is potentially life-threatening for the fetus. The possible benefits of prophylactic hospital admission of asymptomatic women diagnosed with vasa praevia antenatally remain unclear. This study aims to compare the pregnancy outcomes of inpatient versus outpatient management in women with a prenatal diagnosis of vasa praevia. METHODS: A systematic search of four electronic databases was conducted and two reviewers independently screened studies for eligibility. The inclusion criteria incorporated studies with prenatally diagnosed

Are we over-diagnosing vasa praevia? The experience and lessons learned in a tertiary centre.

BACKGROUND: Vasa praevia (VP) is a rare obstetric condition in which unprotected fetal vessels transverse the cervix, are vulnerable to rupture during labour and may result in rapid exsanguination of the fetus. Antenatal diagnosis of VP has resulted in excellent outcomes. However, there are little data available on the false positive rates for the antenatal diagnosis of VP. Improving accuracy of the diagnosis of VP can potentially improve outcomes and reduce unnecessary intervention. AIMS: To assess our accuracy in the diagnosis of VP, examine our false positive diagnoses of VP and suggest st

Measurement of Marginal Placental Cord Insertion by Prenatal Ultrasound Was Found Not to Be Predictive of Adverse Perinatal Outcomes.

OBJECTIVES: The clinical importance of marginal cord insertion (MCI) is currently controversial. In this study, we examined the association between MCI and adverse perinatal outcomes. We also evaluated the ultrasound-measured distance from the site of placental cord insertion (PCI) to the placental margin (PCI distance) and perinatal outcomes. METHODS: This was a retrospective cohort study of MCI and control pregnancies presenting to a single institution between September 2014 and August 2016. Marginal cord insertion was diagnosed on routine anatomy ultrasound scans at 20 weeks' gestation. Th

Vasa previa and extreme prematurity: a population-based study.

OBJECTIVE: To determine population-based risks of preterm birth associated with vasa previa. STUDY DESIGN: Included were 945,950 singleton, live birth cesarean deliveries with and without vasa previa (gestational ages 24-41 weeks) in California between 2007 and 2012. Odds ratios (ORs) of preterm birth were estimated using logistic regression. RESULTS: In total, 586 were complicated by vasa previa (0.06%). In total, 369 (63%) of those with vasa previa were delivered <37 weeks, compared with 91,662 (10%) of those without. Odds of extreme and very preterm birth were substantially higher for pre

Frequency of spontaneous resolution of vasa previa with advancing gestational age.

BACKGROUND: Vasa previa is a serious obstetric complication that can result in fetal hemorrhage and death on spontaneous labor. Suggested management for vasa previa is elective hospitalization and cesarean delivery before spontaneous labor. There is little reported evidence of the rate of vasa previa resolution over the course of gestation. Identification of the resolution rate and of factors predictive of resolution potentially could improve clinical management and patient counseling. OBJECTIVE: The purpose of this study was to identify the resolution rate of vasa previa across gestation and
