Neonatal Outcomes of Umbilical Cord Milking, Early Cord Clamping and Delayed Cord Clamping in Term Infants: A Randomised Controlled Trial.

Introduction Optimal umbilical cord management at birth plays a crucial role in neonatal outcomes. While delayed cord clamping (DCC) has been associated with improved neonatal haematologic and hemodynamic status, early cord clamping (ECC) remains widely practised. Umbilical cord milking (UCM) has emerged as an alternative to DCC, potentially offering similar benefits while reducing the delay in neonatal resuscitation. This study aims to compare the effects of UCM, ECC, and DCC on neonatal outcomes in term infants born between 37 and 42 weeks of gestation. Methods A total of 225 pregnant women

Pharmacokinetics of Intrapartum Benzylpenicillin: Insights Into Candidate Regimens to Prevent Early Onset Neonatal Group B Streptococcus Disease.

Early onset neonatal Group B Streptococcus (GBS) infection accounts for significant global morbidity and mortality. Intrapartum prophylaxis with benzylpenicillin is advised for women at high risk of having a baby affected by early onset GBS disease. Pregnancy-related physiological changes can alter pharmacokinetics. To estimate the intrapartum pharmacokinetics of penicillin, women (n = 12) at risk of GBS disease were enrolled. A fixed regimen of intravenous benzylpenicillin 3 g at onset of labor and 1.5 g every 4 h until delivery was used. Benzylpenicillin concentrations in plasma and umbilica

Neurosurgical Challenges in Recurrent Adhesive Arachnoiditis After Spinal Anesthesia for Cesarean Delivery: Case Report and Literature Review.

Adhesive arachnoiditis (AA) is a rare, chronic inflammatory condition affecting the pia-arachnoid layers, characterized by fibrosis and adhesion formation. It leads to nerve root adhesion and spinal cord compression, often resulting in severe neurological impairment. Commonly associated etiologies include prior spinal surgeries, infections, subarachnoid hemorrhage, epidural anesthesia, myelography, contrast agents, chemical irritation, and, occasionally, idiopathic factors. Following the literature review, 12 cases of AA related to anesthesia and obstetric procedures were identified.  We repor

Chronic, Battery-Free, Fully Implantable Multimodal Spinal Cord Stimulator for Pain Modulation in Small Animal Models.

Spinal cord stimulation (SCS) for chronic pain management is an invasive therapy involving surgical implantation of electrodes into spinal epidural space. While the clinical value and mechanistic action of the therapy is debated considerably in recent years, preclinical chronic studies employing rodent models can provide invaluable insights regarding the balance between efficacy and complications as well as mechanistic understanding of SCS therapy. However, current rodent compatible devices require tethered power delivery or bulky batteries, severely limiting the ability to probe long-term eff

A Rare Case of Double True Umbilical Cord Knots With a Nuchal Cord: A Case Report.

A double true umbilical cord knot (TUCK) is a rare complication of pregnancy that is often missed on ultrasonography. The stricture caused by TUCK can lead to occlusion of fetal circulation, fetal asphyxia, and subsequent death. Despite these risks, there is a lack of evidence and no specific consensus on both antepartum and intrapartum management of TUCK. We report a rare case of a healthy male neonate with a double TUCK and nuchal cord delivered vaginally with a vacuum assist. A 26-year-old female, G2P1001, at 39 weeks of gestation, presented to the labor and delivery unit for induction of l

Topical Dexmedetomidine Pharyngeal Spraying Mitigates Severe Postoperative Sore Throat Following Rigid Bronchoscopy Despite Vocal Cord Injury: A Case Report and Clinical Insight.

Severe postoperative sore throat (POST) following rigid bronchoscopy (RB) is a common complication, with incidence rates reaching 62% due to mucosal trauma from large-diameter scopes. Conventional preventive measures often prove inadequate. This case report demonstrates the efficacy of topical dexmedetomidine (DEX) pharyngeal spraying in a 36-year-old male with recurrent tracheal adenoid cystic carcinoma and refractory POST after three prior RBs. Under bronchoscopic guidance, 48 µg of DEX, prepared by diluting 0.5 mL of 100 µg/mL commercial DEX solution in 4.5 mL of sterile saline, was sprayed

Unraveling Perinatal Risks in Small-for-Gestational-Age vs. Adequate-for-Gestational-Age Fetuses: Is a Pathological Cerebroplacental Ratio a Red Flag for All?

Introduction The association between pathological cerebroplacental ratio (CPR) and perinatal outcomes has garnered significant attention in obstetric research, particularly with regard to fetal growth patterns. This study aims to explore the implications of pathological CPR in small-for-gestational-age (SGA) fetuses compared with those classified as adequate-for-gestational-age (AGA). In clinical practice, CPR assessment has gained prominence due to its potential to improve decision-making regarding the timing and mode of delivery in pregnancies complicated by growth restriction. Previous rese

Evaluating the Role of Ultrasonographic Measurements in Assessing Macrosomia Among the Fetuses of High-Risk Antenatal Women in a Tertiary Care Hospital.

Background and objective Fetal macrosomia, defined as a birth weight >4.0 kg, is associated with serious maternal and neonatal complications such as prolonged labor, cesarean delivery, postpartum hemorrhage, birth trauma, and long-term metabolic risks. Maternal factors, particularly gestational diabetes mellitus (GDM), excessive weight gain, and certain comorbidities, increase the risk of macrosomia. Early detection in high-risk pregnancies is crucial to guide clinical management and improve outcomes. Conventional sonographic estimated fetal weight (EFW) provides important information but may

Radiofrequency Cautery: A Safe Option for Patients With Indwelling Devices.

The increasing prevalence of non-cardiac implantable electrical devices (NCIEDs), including deep brain stimulators (DBS), spinal cord stimulators (SCS), and an increasing list of other devices, presents a significant challenge to conscientious perioperative care teams. The major anticipated operative complications of having a pre-existing indwelling device are twofold. For the first category, there is the profound threat of direct thermal injury to tissue surrounding the device in question, as well as the lateral thermal spread. Of course, when the surrounding tissue is neural and may involve

Evaluation and Association of Meconium-Stained Amniotic Fluid With Fetal Distress.

Background Meconium-stained amniotic fluid (MSAF) often leads to complicated deliveries ranging from instrumental delivery, cesarean delivery, and neonatal complications like fetal distress, neonatal intensive care unit (NICU) admission, and neonatal death. Therefore, this study aimed to evaluate the prevalence of MSAF in fetal distress, to determine the clinical profile of newborns in terms of risk factors, and to study the association between antenatal or intra-natal risk factors with MSAF, between umbilical cord pH and MSAF, between stage of labor and MSAF, and between fetal distress and MS

Design and rationale of CATO, a Phase IIA, randomized, double-blind, placebo-controlled study of single or repeated intravenous administration of umbilical cord-derived mesenchymal stromal cells in ischemic cardiomyopathy.

RATIONALE: To date, almost all studies of cell therapy in chronic heart failure (HF) have delivered cells transendocardially or intracoronarily and all have used a single cell dose, which limits therapeutic efficacy because transplanted cells disappear rapidly. Repeated administrations are necessary to replace the cells that disappear, but this is difficult or impossible when cells are delivered invasively. Further, transendocardial delivery is not feasible in many patients, carries risks, and requires specialized training and equipment, thereby limiting its widespread applicability. Intraveno

Intrathecal drug delivery for chronic abdominal pain: A case series.

INTRODUCTION: Chronic Abdominal Pain (CAP) is a pervasive and challenging global health issue. In a 2023 United States National Health Interview Survey, CAP was present in almost 3 % of all respondents. Various interventional procedures, such as sympathetic nerve blocks (SNBs), thoracolumbar sympathetic ablations, and spinal cord stimulation (SCS), have shown mixed efficacy in managing CAP. This case series describes refractory CAP that is effectively managed with IDD after a treatment algorithm to first distinguish nociceptive pain from a centrally mediated process, but only when the AGA ROME

Furcate Cord Insertions: A Comprehensive Review of Reported Cases, Outcomes, and Management Implications.

IMPORTANCE: Furcate umbilical cord insertion is a rare anomaly in which the umbilical vessels separate and lose Wharton jelly before placental insertion. This condition increases the risk of complications such as vessel avulsion and intrauterine fetal demise (IUFD). Despite these risks, data on this condition are limited, and no formal management guidelines exist for prenatally diagnosed cases. OBJECTIVE: To provide an overview of abnormal umbilical cord insertions, with a focus on furcate cord insertions, as well as the evidence for the management of patients with furcate cord insertions. Persistent Cord Presentation Due to Marginal Cord Insertion at the Lower Edge of a Low-Lying Placenta: A Case of Successful Vaginal Delivery.

The position of the umbilical cord within the uterus is influenced by its insertion site, with low insertion near the internal os raising concerns regarding the risk of cord prolapse and feasibility of vaginal delivery. This report describes a case of persistent cord presentation caused by marginal cord insertion at the lower edge of a low-lying placenta, further complicated by preterm premature rupture of membranes (pPROM) at 30 weeks. Given the fetal immaturity, expectant management was pursued despite the potential risk of cord prolapse. Although persistent amniotic fluid leakage occurred,

Hypoxic-ischemic encephalopathy following intrapartum asphyxia: is it avoidable?

BACKGROUND: The proportion of term hypoxic-ischemic encephalopathy arising during intrapartum fetal surveillance remains unclear. Moreover, recent Cochrane review and other studies emphasized the need for research on the impact of admission cardiotocography and highlighted the necessity for a definition of "avoidable perinatal brain injury". OBJECTIVE: To assess the impact of intrapartum asphyxia on neonatal hypoxic-ischemic encephalopathy occurrence and identify the proportion of cases that benefit from preventive measures. STUDY DESIGN: This retrospective 20-year birth cohort study include

Outcomes with cesarean delivery vs vaginal birth in extremely preterm breech singletons.

BACKGROUND: Decision-making regarding breech extremely preterm mode of birth is difficult. Seeming benefits with cesarean delivery in previous studies may have been due to other factors that had not been adequately accounted for. OBJECTIVE: To determine the association of cesarean delivery vs vaginal birth with neonatal outcomes among live-born, extremely preterm, breech singletons who received active care at birth. STUDY DESIGN: This population-based cohort study included all live-born breech singletons between 23 and 27 weeks of gestation from 2010 to 2022, who received active resuscitatio

A Case Report on an Intricate Forensic Dilemma: Precipitate Labor or Neonaticide?

Precipitate labor and neonaticide can present with overlapping features, often posing significant challenges in forensic interpretation. Differentiating between these possibilities is essential for reaching accurate medicolegal conclusions. This case report examines the forensic complexities involved in distinguishing precipitate labor from neonaticide. A father brought a deceased neonate to the hospital, alleging that the mother had fatally slit the infant's throat. In contrast, the mother claimed the injury occurred accidentally during an unexpected bathroom delivery. She stated that while a
