Influence of perineal tears and body image disturbance on postpartum sexual quality of life: a cross-sectional study.

BACKGROUND: Whether perineal injury severity independently determines postpartum sexual quality of life (SQoL) is unclear. This study evaluated perineal tears and body image as determinants of SQoL and sexual resumption timing. METHODS: A total of 173 women (2-24 months postpartum, vaginal delivery) completed an online questionnaire including the SQoL-F and Body Image Scale (BIS). Women with perineal tear/episiotomy (PT/E; n = 118) were compared with those with an intact perineum (n = 51) using multivariate regression. RESULTS: SQoL-F scores were similar between groups (71.1 ± 23.3 vs 76.9 ±

Women's experiences of wound dehiscence of a second-degree perineal tear and choice of treatment in Denmark: a qualitative interview study.

OBJECTIVE: This study aimed to describe women's experiences of perineal wound dehiscence of a second-degree perineal tear and choice of resuturing or conservative treatment in the first two months after birth. DESIGN: A qualitative descriptive study using individual semistructured interviews was conducted. Data were analysed using Braun and Clarke's reflexive thematic analysis approach to explore women's experiences and perspectives on dehisced perineal wounds of a second-degree perineal tear. Interviews were audio-recorded, transcribed verbatim and analysed concurrently with data collection.

Perineal body length as a predictor of severe perineal lacerations in a Turkish obstetric cohort.

This study aimed to investigate the association between perineal body length (PBL) and the occurrence of severe perineal trauma, defined as third- and fourth-degree lacerations, in a Turkish obstetric population. A prospective observational study was conducted at a tertiary maternity hospital between April 2024 and March 2025. A total of 495 women meeting predefined eligibility criteria were included. PBL was measured once during the first stage of labor at 4 cm cervical dilation using a sterile paper tape. Maternal demographic, obstetric, and neonatal characteristics were prospectively collec

Implementing a Four-Step Evidence-Based Pushing Protocol for Labor and Birth.

OBJECTIVE: First, to determine whether implementing an evidence-based practice pushing protocol for pregnant individuals in the second stage of labor with an epidural affects the observed duration of the second stage of labor, the degree of perineal laceration, and the use of operative vaginal birth compared with standard practice. Second, to determine whether implementing an educational session on the protocol increases the frequency of nursing documentation of flexible sacrum positioning, investigation of fetal position, interventions to prevent lower-extremity nerve injury, and use of warm

Photobiomodulation for Pain, Healing, and Analgesic Use after Perineal Lacerations: A Double-Blind Randomized Controlled Trial.

INTRODUCTION AND HYPOTHESIS: Approximately 60% of women experience some degree of perineal laceration following vaginal delivery. Although there is evidence supporting the use of photobiomodulation (PBM) for pain relief and wound healing in various conditions, there is a lack of studies on its effect on these lacerations. Our hypothesis is that PBM aids in pain management and healing in perineal lacerations/episiotomies in hospitalized women. MATERIALS AND METHODS: A randomized, controlled, double-blind clinical trial in women with grade I/II lacerations and/or episiotomy, assigned to the int

Occurrence of wound infections after perineal tears and perineal assessment in postpartum care: A prospective cohort study.

INTRODUCTION: There is considerable variation in reported perineal wound infection occurrences and limited knowledge of postpartum follow-up for perineal injuries. The primary aim of this study was to assess the occurrence of symptoms of perineal wound infections and infections requiring medical intervention within the first 30 days postpartum. The secondary aim was to investigate perineal assessment by healthcare providers in postpartum care. MATERIAL AND METHODS: This single-center cohort study was conducted between October 2020 and October 2022. In this analysis, participants with perineal

[Does Delivery During Holidays Increase the Risk of Neonatal Hypoglycemia in Women with Gestational Diabetes?].

INTRODUCTION: Throughout the 9-months of pregnancy, a woman with gestational diabetes (GDM) is expected to celebrate holidays, which makes coping with diabetes difficult and alters routine glucose control. AIMS: We hypothesized that delivery in the first week following a holiday, might be associated with altered glucose homeostasis, causing more neonatal hypoglycemia. METHODS: A retrospective study was conducted in a tertiary center between the years 2007-2023. Eligible patients were women with a term singleton pregnancy, diagnosed with GDM. Women were allocated into two groups based on the

Intrapartum Febrile Morbidity and Maternal Birth Trauma After Vaginal Delivery: A Parity-Stratified Retrospective Cohort Study.

Objective: We aimed to evaluate the association between intrapartum febrile morbidity and maternal birth trauma among women undergoing vaginal delivery, with stratification by parity. Methods: This retrospective cohort study included women with singleton, cephalic, vaginal deliveries at a university-affiliated medical center between 2018 and 2024, excluding multiple gestations and cesarean deliveries. The primary exposure was intrapartum febrile morbidity, defined as maternal intrapartum temperature ≥38.0 °C, with or without clinical chorioamnionitis. The primary outcome was maternal birth tra

Association Between Intrapartum Papaverine Administration and the Risk of Obstetric Anal Sphincter Injuries: A Retrospective Cohort Study.

INTRODUCTION AND HYPOTHESIS: Perineal trauma during vaginal delivery may result in long-term morbidity. We aimed to test the hypothesis that intrapartum administration of papaverine is associated with a lower incidence of obstetric anal sphincter injuries (OASIS) among primiparous patients. METHODS: This retrospective cohort study included primiparous patients with singleton term (> 37 weeks) pregnancies who delivered vaginally at Galilee Medical Center, Nahariya, Israel, between March 2020 and June 2024. Patients who received intrapartum 80 mg intramuscular papaverine were compared with thos

Effect of Hip Extension Compared With Usual Positioning on Perineal Lacerations During First Vaginal Delivery: A Randomized Clinical Trial.

OBJECTIVE: To determine the effect of hip extension at the time of crowning on perineal lacerations and obstetric anal sphincter injuries. METHODS: This was a randomized, pragmatic, nonblinded clinical trial. Participants included nulliparous patients aged 18 years or older with singleton term gestations with nonanomalous fetuses. Immediately before crowning, participants in the intervention group were directed to lower their legs, extending the angle of their hips. Participants in the usual care group were undirected and typically delivered in the flexed position. The primary outcome was a c

Mapping historical diaries into knowledge graphs: a multi-granular approach.

PLAIN ENGLISH SUMMARY: There are new ways to engage people with science and research but many patient support groups and charitable organisations still hold traditional meetings to provide updates on their activities and to report new developments in their field of interest. These meetings often feature presentations given by medical doctors or, in the case of research-focussed organisations, by research scientists.Receiving feedback from people who are confused and sometimes upset by some types of information, and the way it is presented at meetings, made us think about better ways for resear

Acute pain management after vaginal delivery with perineal tears or episiotomy.

BACKGROUND: A vaginal delivery may be associated with acute postpartum pain, particularly after perineal trauma. However, pain management in this setting remains poorly explored. OBJECTIVE: The aim of this systematic review was to evaluate the literature and to develop recommendations for pain management after a vaginal delivery with perineal trauma. EVIDENCE REVIEW: MEDLINE, Embase, and Cochrane databases were searched for randomized controlled trials (RCTs) and systematic reviews assessing pain after a vaginal delivery with perineal tears or episiotomy until March 2023. Influencing Factors in Urinary Retention in Parturients With Epidural Anesthesia Following Term Vaginal Birth.

Background: Epidural analgesia is a widely employed method in obstetric care for labor pain management. Postpartum urinary retention is a common complication that can arise during the postnatal period. This study aimed to evaluate the current status and influencing factors of postpartum urinary retention in parturients who received epidural anesthesia. Methods: We retrospectively included parturients who underwent vaginal delivery at our institution from June 2022 to October 2024. The characteristics of parturients with and without postpartum urinary retention were analyzed and compared. Logis

Sexual Function After Vaginal Delivery in Primiparous Women: A Perspective in the First Months Postpartum.

Background/Objectives: Female sexual dysfunction (FSD) involves persistent issues with desire, arousal, orgasm, or pain during intercourse. The Female Sexual Function Index (FSFI), a validated 19-item questionnaire, is widely used to assess FSD. Childbirth, particularly vaginal delivery with perineal trauma, can increase FSD risk, with 41-83% of women affected at six months postpartum. However, early postpartum FSD remains underexplored. This study examines FSD risk factors in first-time mothers delivering vaginally, using longitudinal FSFI assessments before and after the delivery. Methods: A

Effects of manual perineal protection and pushing techniques used in the second stage of labor on perineal outcomes: a randomized controlled trial of combinations of strategies.

BACKGROUND: Manual perineal protection and pushing techniques can impact the incidence of perineal trauma. Limited data exist on the impact of combinations of management strategies employed during the second stage of labor on perineal outcomes. The objective of this study was to evaluate the combined impact of interventional and relatively more spontaneous techniques employed during the second stage of labor on perineal outcomes. MATERIALS AND METHODS: This randomized controlled study was carried out in the maternity clinic of a state hospital. Low-risk, term nulliparous women with vertex pre

Opioid consumption and pain scores following perineal lacerations: a retrospective cohort study (2019-2023).

BACKGROUND: Perineal lacerations frequently complicate vaginal deliveries and impact recovery. METHODS: We examined the relationship between laceration severity and opioid consumption with scheduled multimodal analgesia through a retrospective cohort study (2019-2023) of 14,477 vaginal deliveries. Propensity score matching was used to address baseline group imbalances, followed by logistic regression analysis. RESULTS: Distribution included: no lacerations (26.9%), first- or second-degree (69.0%), and third- or fourth-degree (4.0%). Patients with third- or fourth-degree lacerations more freq

Women's experience with a first- and second-degree perineal tear and episiotomy - a qualitative study.

BACKGROUND: Perineal tears are associated with both physical- and psychological complications, and studies have found that women’s individual experience of the pain from the tear does not necessarily reflect the extent and severity of the tear. Women seem to experience a lack of consistency and sufficient follow up in the post-partum period related to perineal trauma. The aim of this study was to explore women’s experiences with first- and second- degree perineal tears and episiotomy in the post-partum period in an outpatient perineal clinic at a tertiary hospital in Norway.
