The Retrograde Migration of Foreign Bodies inside the Female Genital Tract: A Step-by-Step Hysteroscopic Approach.

Foreign bodies within the female genital tract are uncommon findings that may remain asymptomatic or undetected for extended periods. In rare instances, these objects undergo retrograde migration towards the uterine cavity, creating diagnostic and therapeutic challenges. Such occurrences may follow self-insertion, accidental introduction, or retention of materials after medical or hygienic procedures. We describe two illustrative cases managed successfully with a minimally invasive hysteroscopic approach combining electrosurgical radial incision and Foley catheter-assisted mechanical dilation.

Double Trouble: Primary Heterotopic Pregnancy with Early Rupture.

A 34-year-old G1P0 patient presented to the emergency room (ER) at 7-weeks with cramping and vaginal spotting for 12 hours. Her pregnancy was conceived naturally. Serum β-human chorionic gonadotropin (β-hCG) level on arrival was 83 200 IU/L. Transvaginal ultrasonography (TVUS) of the pelvis showed an intrauterine and concomitant right tubal ectopic pregnancy (Figs. 1 and 2). Both embryos of the heterotopic pregnancy showed cardiac activity (Fig. 2). A moderate amount of free fluid was noted (Fig. 1 and Supplemental Video 1), concerning for tubal rupture. The patient underwent an uncomplicated

Effects of Acupuncture on Labor Progression in Term Pregnancies with Prelabor Rupture of Membranes: A Randomized Controlled Trial.

INTRODUCTION: Prelabor rupture of membranes (PROM) at term increases the risk of maternal and neonatal complications. While expectant management aims to allow spontaneous labor, it can lead to infection, whereas active management may result in unnecessary interventions. Acupuncture (ACU) has been suggested as a nonpharmacological method to promote labor onset and reduce the need for induction. This randomized controlled trial aimed to assess the efficacy of ACU in women with PROM at ≥37 weeks' gestation. MATERIALS AND METHODS: Eligible women with singleton pregnancies, negative Group B Strept

Phase-specific impact of patient anatomy on rectal cancer surgery: robotic assistance neutralizes dissection difficulty.

This study aimed to determine how patient anatomical characteristics influence the technical demands of rectal cancer surgery and to assess whether robotic assistance can moderate these effects. A retrospective analysis was conducted on 131 individuals who underwent minimally invasive low anterior resection at Kobe University Hospital (laparoscopic: n = 74; robotic-assisted: n = 57). The interval between the start of pneumoperitoneum and rectal transection—representing the dissection phase—was used as a proxy for technical difficulty. Associations between dissection time and body mass index (B

Prevalence of Labor Induction, its Indication, and Feto-Maternal Outcomes in Ethiopia: A Systematic Review and Meta-Analysis.

Labor induction (IOL) is a common obstetric intervention to stimulate uterine contractions before the spontaneous onset of labor, with the goal of achieving vaginal delivery. It is indicated for various reasons, including post-term pregnancy, premature rupture of membranes (PROM), maternal medical conditions, and fetal growth restriction. This study aims to evaluate the prevalence of labor induction, its indications, and the associated maternal and fetal outcomes in Ethiopia. A comprehensive search for relevant articles was conducted using widely recognized databases such as Google Scholar, Co

Reference Handbook of Gynecologic Pelvic MRI.

Gynecologic pelvic MRI is a crucial imaging modality for problem solving, offering superior soft-tissue contrast and multiplanar capabilities for the detailed evaluation of female patients across a range of indications. Accurate interpretation of gynecologic pelvic MRI depends on using protocols tailored to the clinical indication, reviewing the patient's clinical history (such as age, symptoms, medical and surgical history, and genetic mutations), and considering laboratory data. A comprehensive understanding of the female pelvic anatomy, best visualized on high-resolution multiplanar T2-weig

Vaginal birth after two cesarean sections (VBAC-2) under a standardized protocol: success rates, safety, and cesarean after spontaneous labor as an alternative.

BACKGROUND: An increasing number of women with two previous cesarean sections are seeking personalized delivery options. However, evidence on the safety of a vaginal birth after two cesarean sections (VBAC-2) under standardized, non-induction protocols remains limited. This study aimed (1) to assess success rates and safety of vaginal birth after two cesarean sections (VBAC-2) under a standardized non-induction protocol and (2) to compare maternal and neonatal outcomes between elective repeat cesarean (ERCS-2) and cesarean after spontaneous labor onset (CSAOL-2). METHODS: We conducted a retros

Uterine rupture risk during trial of labor after one cesarean in a population-based cohort study of induction method and labor management.

A prior cesarean section is the primary risk factor for uterine rupture in a trial of labor after cesarean, a rare event associated with severe maternal and neonatal morbidity and mortality. This population-based cohort study aimed to determine the risk of uterine rupture in women undergoing trial of labor after one previous cesarean section in the Stockholm-Gotland Region and to examine associations between induction methods, labor management, and rupture risk. The cohort included 11,947 women with a cephalic-presenting, singleton infant at ≥ 37 + 0/6 weeks of gestation. Incidence of uterine

High risk and low incidence diseases: Postpartum hemorrhage.

INTRODUCTION: Postpartum hemorrhage (PPH) is a leading cause of maternal morbidity and mortality worldwide, with rising incidence in the United States. Diagnosis can be particularly challenging in the emergency department (ED), where time-sensitive decisions are required and maternal physiologic adaptations may obscure early warning signs. Emergency physicians must be equipped to recognize subtle presentations, initiate resuscitation, and coordinate care across specialties. OBJECTIVE: This review highlights the pearls and pitfalls of PPH, including presentation, diagnosis, and management in t

Accuracy of Blood Loss Estimation and Identification of Factors Contributing to Early Postpartum Hemorrhage Following Vaginal Delivery.

Objective: The study aimed to assess the accuracy of two distinct methods for estimating blood loss (EBL) and to identify potential factors contributing to early-onset postpartum hemorrhage (PPH) following a vaginal delivery (VD). Methods: Women in singleton pregnancies undergoing spontaneous/induced VD were recruited for this prospective observational cohort study. Methods of EBL included: (1) visual assessment by an attending obstetrician (sEBL) and (2) implementation of a mathematical formula (fEBL). Early PPH was defined as a cumulative blood loss exceeding 500 mL within the first 24 h aft

Heterotopic Pregnancy After Ovulation Induction by Clomiphene Citrate: A Case Report.

Heterotopic pregnancy (HP), when intrauterine and ectopic pregnancies occur together, is uncommon but dangerous. While rare in spontaneous conception, the risk increases with fertility treatments like clomiphene citrate (CC). These cases are easily missed because doctors may be reassured by the intrauterine pregnancy and overlook the ectopic one. We describe a 31-year-old woman with primary infertility who conceived following clomiphene-induced ovulation. At seven weeks, she developed pelvic pain and vaginal bleeding. She was hemodynamically unstable with tachycardia and low blood pressure. Ul

Successful repair of spontaneous uterine rupture in the second trimester of pregnancy with progression to live birth.

OBJECTIVES: Uterine rupture before the onset of labor is a rare but life-threatening obstetric emergency. This condition carries a high risk of maternal and fetal morbidity and mortality and requires prompt and careful management. CASE PRESENTATION: A 26-year-old woman, gravida 3 and parity 2, was admitted to the gynecology department with complaints of abdominal pain in her 23rd week of pregnancy. During evaluation, a decrease in hemoglobin levels was noted, and abdominal MRI revealed uterine rupture. Uterine rupture was surgically repaired with a double-layer suture with fetal preservation.

Pushing using real-time sonographic ultrasound education (PURSUE): A study protocol for a randomized controlled trial.

INTRODUCTION: Maternal expulsive force is a key determinant of fetal head descent during the active second stage of labour. Pushing is most effective when abdominal muscle contraction is synchronised with relaxation of the levator ani muscle, thereby widening the pelvic hiatus. Trans-perineal ultrasound studies show that 15-20% of nulliparous women paradoxically contract the levator ani (co-activation) during pushing, narrowing the hiatus and slowing head descent. Consequently, about 20% of primigravidas experience a prolonged second stage, with higher rates of operative birth and perineal tra

A finite element study of soft-tissue deformation during birth in australopithecines: Assessing pelvic floor stress and fetal head rotation in early hominins.

The emergence of bipedal locomotion during human evolution has considerably reshaped the hominin pelvis, which also impacted the pelvic floor musculature. Among early hominins, australopithecines have an anteroposteriorly narrow, transversely wide bony pelvis. This study investigates whether this particular shape of the birth canal weakens the pelvic floor during childbirth in these hominins and if the pelvic floor contributes to rotational birth. As such, we explored the stress values at the pelvic floor that occur during the descent of the fetal head, using 3D models of extant humans based o
