Application of Predictive Analytics in Pregnancy, Birth, and Postpartum Nursing Care.

Predictive analytics has emerged as a promising approach for improving reproductive health care and patient outcomes. During pregnancy and birth, the ability to accurately predict risks and complications could enable earlier interventions and reduce adverse events. However, there are challenges and ethical considerations for implementing predictive models in perinatal care settings. We introduce major concepts in predictive analytics and describe application of predictive modeling to perinatal care topics such as fertility, preeclampsia, labor onset, vaginal birth after cesarean, uterine ruptu

Age-Related Dynamics in Endometrial Vascularity: A Comprehensive Three-Dimensional Ultrasound Evaluation During Follicular and Luteal Phases.

Objective: Transvaginal ultrasonography plays a crucial role in contemporary fertility management, offering insights into uterine and endometrial blood flow. Three-dimensional ultrasonography utilizing power Doppler angiography (3D-CPA) allows precise measurement of endometrial volume and vascular parameters, such as the vascularization index (VI), blood flow index (FI), and vascularization flow index (VFI); variables that indirectly assess endometrial receptivity and integrity. Doppler technology allows for the capture of changes in the uterus induced by hormonal-related fluctuations during t

A Novel Pneumatic Balloon Saved the Lives of Three Patients With Refractory Pelvic Hemorrhage After Peripartum Hysterectomy.

This case report discusses three patients who presented in our obstetric emergency with refractory pelvic hemorrhage following cesarean hysterectomy due to placenta accreta. They were referred from district hospitals with pelvic towel packing as a lifesaving procedure after failure of pelvic angiographic embolization or internal iliac artery ligation. After stabilization, we did a relaparotomy to remove the surgical towels, which led to the recurrence of excessive pelvic hemorrhage. The new pneumatic balloon was prepared from two surgical rubber gloves inserted into each other and ligated arou

Risk factors and outcomes of uterine rupture before onset of labor vs. during labor: a multicenter study.

OBJECTIVES: This study aims to reveal the distinctions between uterine rupture (UR) that occurs before onset of labor and during labor. METHODS: This multicenter study was conducted across three tertiary hospitals in South China involving obstetrical UR from January 2009 to April 2022. Cases were categorized into two groups based on the timing of UR: prelabor and labor. RESULTS: Among the cases of UR, 42 were classified in the prelabor group, and 27 in the labor group. The prelabor group had a higher prevalence of histories of myomectomies, ectopic pregnancy surgeries, congenital uterine ano

A Near Catastrophe: Antiphospholipid Syndrome and Catastrophic Antiphospholipid Syndrome Complicating Emergency Department Care.

BACKGROUND: Antiphospholipid syndrome (APS) is an autoimmune syndrome that often clinically manifests as venous thrombosis, arterial thrombosis, pregnancy morbidity, and thrombocytopenia. While APS is rare and difficult to diagnose, it can lead to devastating complications, most notably catastrophic antiphospholipid syndrome (CAPS). Both APS and CAPS have a strong association with pregnancy, which is often what reveals a new diagnosis of APS. Recurrent spontaneous abortions and pre-eclampsia are the most common pregnancy-related triggers for APS, while Hemolysis, Elevated Liver Enzymes, Low Pl

Continued versus Discontinued Oxytocin in the Active Phase of Induced Labor in Term Pregnancies: An Updated Systematic Review and Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Oxytocin has long been used for the induction of labor, but it can be associated with fetal and maternal complications that could potentially be reduced by discontinuing the treatment during labor. We performed this meta-analysis to determine whether discontinuation of oxytocin, once the active phase of induced labor is achieved, affects the second stage of labor and the rate of various maternal and fetal outcomes. METHODS: We searched for randomized controlled trials (RCTs) comparing discontinuing oxytocin after the active stage of labor is established versus continuing to give

Robotic Versus Laparoscopic Pelvic Lymphadenectomy for Endometrial Cancer Under the Japanese Public Health Insurance System.

BACKGROUND/AIM: Surgery is the preferred treatment modality for endometrial cancer. In recent years, minimally invasive surgery (MIS) has become increasingly popular, and in Japan, robotic surgery has been covered by national insurance in addition to laparoscopic surgery for apparent stage IA cases since 2018. We conducted a comparative analysis of laparoscopic and robotic surgeries, including pelvic lymphadenectomy for stage IA endometrial cancer, with the aim of evaluating the efficacy of robotic surgery. PATIENTS AND METHODS: A retrospective analysis was conducted for patients who underwen

Endometriosis and associations with risks of adverse pregnancy and perinatal outcomes: a case-control study in Egypt.

BACKGROUND: Endometriosis and its association with adverse pregnancy and perinatal outcomes have recently drawn attention, pointing to increased risks of repeated caesarean sections, the occurrence of preterm births, and stillbirths. PATIENTS AND METHODS: This study included 25 pregnant women diagnosed with endometriosis and 25 pregnant women without endometriosis (control group). Maternal, fetal, and neonatal data were collected and compared between the endometriosis group and the control group concerning various maternal and neonatal parameters. RESULTS: Patients with endometriosis were sl

FIGO good practice recommendations for vaginal birth after cesarean section.

The rising global rate of cesarean section (CS) has prompted renewed focus on vaginal birth after cesarean (VBAC) as a safe and effective alternative to repeat CS in properly selected women. The FIGO good practice recommendations provide evidence-based recommendations to guide VBAC care. Success rates for VBAC range from 60% to 80%, with the highest likelihood in women with prior vaginal birth, non-recurrent CS indications, interbirth intervals ≥18 months, BMI <30 kg/m2, and spontaneous labor onset. While uterine rupture is the primary concern, its risk remains low at approximately 0.3%-0.7% f

Pelvic packing as a life-saving intervention in the referral of patients with obstetric hemorrhage: comparing outcomes in referred and non-referred patients.

BACKGROUND: Obstetric hemorrhage continues to be a major contributor to maternal deaths and health complications globally. Pelvic packing, a method used in trauma surgery to control severe bleeding, has been successfully applied to treat obstetric hemorrhages. This research aimed to evaluate the appropriateness and effectiveness of pelvic packing for patients with obstetric hemorrhage, especially in the context of patient referrals. MATERIALS AND METHODS: A comparative observational study was carried out on 45 patients who received pelvic packing for obstetric hemorrhage at a hospital in sout

Formation of the placental membranes and pathophysiological origin of associated great obstetrical syndromes.

Formation of the smooth membranes is an essential phase of human placentation to allow safe rupture of the chorionic sac and birth of the fetus without damaging the placenta. The membranes form through regression of two-thirds of the villi that cover the early gestational sac shortly after implantation. Regression is associated with locally high levels of oxidative stress secondary to partial onset of the maternal arterial circulation to the placenta. Onset starts preferentially in the peripheral zone from ∼6 to 8 weeks of gestation, reflecting the lesser extent of plugging of maternal spiral

Robot-assisted hysterectomy for endometrial cancer: is there a correlation between uterine diameters, pelvic volume, and surgical outcomes?

Hysterectomy could be performed through a vaginal approach, laparotomy, or laparoscopy. Robotic surgery is a technique that can be defined as a particular type of laparoscopy, often used in interventions where microsurgery can help improve radicality and effectiveness. In choosing patients for the robotic technique, many characteristics could be studied to tailor the surgical choice. Our aim is to describe the anthropomorphic and radiological parameters linked to worse surgical outcomes for the potential stratification of patients in the preoperative decision about the type of surgery. In our

Damage Control Surgery in Obstetrics and Gynecology: Abdomino-Pelvic Packing in Multimodal Hemorrhage Management.

Damage control surgery (DCS) is a staged surgical strategy for rapid control of life-threatening bleeding, followed by physiological stabilization and delayed definitive repair. Abdomino-pelvic packing (APP)-placing compressive material within the pelvis and/or abdomen to tamponade bleeding-is a cornerstone of DCS as a temporizing measure to achieve hemostasis and stabilization in critically unstable patients. This narrative review synthesizes current evidence on DCS with a focus on APP-a technique historically developed in trauma and orthopedic surgery for exsanguinating pelvic bleeding but a

Is it safe for women with a history of two cesarean deliveries to undergo a vaginal delivery attempt in comparison to patient with a history of one cesarean delivery?

INTRODUCTION: Worldwide, numbers of repeat cesarean sections continue to rise. Although there is a multitude of evidence about the safety of a vaginal delivery attempt after one cesarean section, data is scarce regarding the risks of one compared to two prior procedures. This study aims to determine whether vaginal childbirth is less safe and successful for both mother and child in patients with a history of two cesarean sections compared to those with only one. MATERIALS AND METHODS: This retrospective cohort study included all patients with a history of one or two prior cesarean deliveries

Morbidity Associated with Autologous Fascia Lata Harvesting for Pelvic Floor Surgery.

INTRODUCTION AND HYPOTHESIS: Fascia lata (FL) is an alternative to mesh for pubovaginal sling (PVS) or abdominal sacrocolpopexy/hysteropexy (ASC). Literature on perioperative complications is sparse. We aim to evaluate the morbidity associated with the FL harvest for treatment of female pelvic floor dysfunction. METHODS: Women undergoing FL harvest Feb 2021-May 2025 at a surgeon's private practice and tertiary public hospital. The same harvest technique was used, with graft size depending on the indication (mean size ASC; 13 × 4 cm; PVS 10 × 2 cm). Thigh issues were ascertained by direct ques

When the Unexpected Happens: Uterine Rupture in an Unscarred Uterus.

Uterine rupture in an unscarred uterus is extremely rare, but when it occurs, it can lead to the worst maternal and neonatal outcomes due to delayed diagnosis and management. A high index of suspicion is required due to variable presentation. We present here a case of a multiparous woman at term with a spontaneous onset of labor and progression to the second stage. However, when she did not deliver even after two hours, she was referred to our center with a non-reassuring fetal heart rate. After the initial evaluation of the patient, a provisional diagnosis of uterine rupture with fetal demise

Fatal acute uterine inversion post-vaginal delivery: an autopsy-based case report.

Uterine inversion refers to the condition where the fundus is turned inside and positioned within the uterine cavity. Uterine inversion is a life-threatening and uncommon obstetric emergency that can be fatal because of postpartum hemorrhage and shock. Acute uterine inversion is the most common type, which occurs within 24 h of delivery and is usually associated with untrained birth attendants and a lack of knowledge of labor-inducing drugs. We present a case of alleged medical negligence in the death of a 23-year-old female following vaginal delivery of a second child with autopsy findings su

Performance of POCUS for Pregnancy Evaluation using a Non-Piezoelectric Ultrasound Device in the Emergency Department.

BACKGROUND: Confirmation of pregnancy location using point of care ultrasound (POCUS) is an essential component of emergency obstetric care. The accuracy of a non-piezoelectric portable ultrasound device compared to obstetric sonographer-performed ultrasonography remains uncertain. OBJECTIVE: The objective of this study was to describe the practical utility of a non-piezoelectric POCUS system (Butterfly iQ+) in the emergency department, including characterizing the accuracy of findings relative to obstetric sonographer-performed ultrasound (OB-US) for obstetric and non-pregnant pelvis evaluat

[Intrauterine Inflammation and Short- to Long-Term neurodevelopment in preterm infants: An overview].

The aim of this review is to gather the most recent data on the association between intrauterine inflammation and neurodevelopmental disorders in preterm infants. Two major methodological challenges will be emphasized: first, the heterogeneity of definitions of intrauterine inflammation; and second, the heterogeneity of study populations, in which the causes of preterm birth (spontaneous labor, preterm premature rupture of membranes, maternal hypertensive disorders, etc.) are not always distinguished. These elements introduce a risk of confounding bias and partly explain the divergence between
