Rectus sheath haematoma following emergency caesarean section: diagnosis and management.

Rectus sheath haematoma is a rare condition characterised by bleeding within the rectus abdominis muscle sheath. While often self-limiting, large haematomas may result in significant morbidity and require prompt intervention. A woman in her mid-20s with severe pre-eclampsia underwent an emergency caesarean section. Postoperatively, she developed severe abdominal pain and a palpable right-sided mass. Imaging revealed a large Type III rectus sheath haematoma extending from the hypochondrium to the pelvis. Surgical exploration evacuated a 600-mL haematoma, and bleeding was controlled by ligating

Abdominal bleeding secondary to isolated mesenteric injury following blunt abdominal trauma: A case report.

RATIONALE: Isolated mesenteric injury often results from traffic accidents causing blunt abdominal trauma, exhibit nonspecific symptoms and signs. Most mesenteric injuries occur concurrently with injuries to other organs, such as the liver and spleen. As a result, the incidence of isolated mesenteric injury is very low and often misdiagnosed. PATIENT CONCERNS: A 49-year-old male patient presented with abdominal pain following a traffic accident more than 5 hours before admission. Upon admission, his blood pressure was normal but dropped sharply in a short time and presented with shock. Anemia

Enterocervical Fistula Six Weeks After Laparoscopic Mesh Sacral Colposuspension: A Case Report.

Laparoscopic colposuspension with mesh is a common procedure performed to treat apical pelvic organ prolapse refractory to non-surgical interventions. Mesh complications involving a fistula are an uncommon event that typically occurs months or years later. We report a case complicated by enterocervical fistula that occurred six weeks after mesh placement. This was the case of a 67-year-old female with no surgical history who underwent a laparoscopic supracervical hysterectomy, bilateral salpingo-oopherectomy, laparoscopic colposuspension with mesh, and midurethral sling for treatment of stage

Incidence and impact of urogenital sequelae in women following pelvic-ring injuries: a retrospective cohort study.

PURPOSE: Pelvic-ring injuries in women often result in urinary dysfunction owing to the proximity of pelvic organs to the urinary tract, significantly affecting quality of life. However, detailed research on urinary sequelae remains limited. This study aimed to assess the incidence of urinary dysfunction in women after pelvic-ring injuries and to identify risk factors influencing urinary function. METHODS: We conducted a retrospective cohort study of women who underwent osteosynthesis for pelvic-ring injuries between January 2022 and June 2023 with ≥ 12 months of follow-up. Urinary dysfunctio

CT angiography of the lower abdomen should be considered with vaginal trauma and excessive bleeding, even though there are no pelvic fractures present.

A healthy woman in her 20s presented to the emergency department (ED) after she endured a high-energy trauma during a dirt bike race. Physical examination showed no signs of thoracic or abdominal injury. Her pubic bone was painful at palpation, with a large amount of vaginal bleeding and a rupture of the vaginal back wall. Additional imaging with X-rays and extended focused assessment with sonography in trauma showed no fractures or free fluid in the abdomen, and the patient was then taken to the operating room for closure of the rupture. This patient became haemodynamically unstable during th

Radiological evaluation of pelvic venous Disorders: A comprehensive review.

Pelvic venous disorder (PVD) is a common gynecological disorder caused by chronic pelvic venous congestion. Clinically, it primarily manifests as lower abdominal pain, lower back pain, dyspareunia, increased menstrual flow, and increased vaginal discharge, though positive physical signs are few. The gynecological examination may reveal bilateral ovarian tenderness. The clinical symptoms of PVD are nonspecific and overlap with those of other gynecological conditions, leading to a high rate of missed diagnoses and making accurate diagnoses challenging. The exact etiology of PVD is not fully unde

SGK1 inhibits oxidative injury and extracellular matrix degradation by activating the GSK-3β (Ser9)/Fyn/NRF2 pathway in pelvic organ prolapse.

AIMS: Sustained oxidative stress (OS) promotes the development of pelvic organ prolapse (POP); however, the pathogenesis of POP under OS conditions remains unclear. This study aimed to investigate the role of serum and glucocorticoid-induced protein kinase 1 (SGK1) in the progression of POP in OS and elucidate its potential molecular mechanisms. MATERIALS AND METHODS: The protein levels of SGK1 in fibroblasts and other cells within the uterosacral ligament tissues (ULTs) from patients with POP in OS were measured by immunofluorescence (IF). Human uterosacral ligament fibroblasts (hUSLFs) were

A systematic approach to laparoscopic hysterectomy for enlarged uteri: The Ship Theory.

Total laparoscopic hysterectomy (TLH) for enlarged uteri presents a significant challenge for surgeons due to limited surgical field exposure, increasing the risk of injury to the bowel, bladder, ureters, and blood vessels. To minimize these intraoperative complications, a surgical approach known as "The Ship Theory" has been developed at our center. According to this concept, the uterus is likened to a large vessel moored within the pelvis. As its supporting ligaments ("anchors") are progressively released, the uterus gains mobility, allowing it to migrate into the abdominal cavity. This enha

Clinical utility of ultrasonography in pediatric and adolescent gynecology: retrospective review of 1313 ultrasound examinations.

OBJECTIVES: Ultrasound is the first-line imaging modality of the pelvis in the pediatric and adolescent gynecology (PAG) population. Ultrasound findings in pre- and postpubertal PAG patients differ from those in adults. Diagnostic models for adnexal pathology have not been validated in this cohort. The primary aim of this study was to evaluate normative findings and the incidence of pathology in this cohort. The secondary aim was to assess the performance of expert opinion alone, as well as using retrospective application of the International Ovarian Tumor Analysis (IOTA) simple rules (SRs) an

Role of uterine artery occlusion at origin during total laparoscopic hysterectomy in non-malignant cases for optimal surgical outcome.

OBJECTIVE: Total laparoscopic hysterectomy (TLH) offers faster recovery and less discomfort, but complex pelvic pathologies, adhesions, and scar tissue from prior conditions can complicate the procedure. This study compared the benefits of ligating the uterine arteries at their origin during a total laparoscopic hysterectomy to ligating the uterine arteries subsequent to the cornual pedicles. METHODS: A retrospective observational cohort was conducted from March 2022 to March 2024 to compare patients in the two groups pertaining to clinical and surgical outcome measures. The sampling techniqu
