Prognostic Potential of Clinical Factors, Serological Indicators, and Pelvic Floor Ultrasound Parameters in Women With Postpartum Stress Urinary Incontinence.

Aims/Background Numerous studies have assessed factors influencing stress urinary incontinence (SUI); however, their findings are somewhat inconsistent. Therefore, this study aimed to explore the predictive potential of clinical characteristics and serological indicators combined with pelvic floor ultrasound parameters for postpartum SUI. Furthermore, the analysis was intended to determine the prevalence and factors affecting postpartum SUI among women in China. Methods This retrospective analysis included 511 parturient women who underwent prenatal examinations and gave birth at Yongkang Wome

Mifepristone versus placebo to increase the rate of spontaneous labour in people with a prior caesarean: a double-blind randomised controlled trial (Mi-labourTrial).

BACKGROUND: For people with a prior caesarean, avoiding a repeat caesarean and undergoing vaginal birth after caesarean (VBAC) may decrease both short- and long-term complications. Current options include awaiting labour or undergoing induction of labour (IOL) with mechanical cervical dilation followed by amniotomy and oxytocin infusion. Spontaneous labour is preferred as the use of exogenous oxytocin increases the rate of uterine rupture, a feared complication of VBAC. Additional options are vital to improve the chance of labour in people with a prior caesarean scar. Mifepristone, a progester

Ovarian Collision Tumor in a Pediatric Patient: A Mature Teratoma Associated with a Combined Tumor Containing a Mucinous Cystadenocarcinoma Component.

Background: Collision tumors, especially in the ovary, are a rare phenomenon where two distinct types of tumors develop adjacent to each other within the same organ but remain separate histologically. We present a case of the first collision ovarian tumor in a 14-year-old girl consisting of a combined tumor and a mature teratoma. Case Report: A 14-year-old girl presented with abdominal swelling for the past three months, without other symptoms. Ultrasound (US) examination of the abdomen revealed a large cystic mass with multiple septa, filling the entire abdomen from the diaphragm to the pelvi

Evaluation of a new prediction model for the estimation of risk of obstetrical anal sphincter injuries.

BACKGROUND: Obstetrical anal sphincter injuries are complications of vaginal birth that have the potential to cause substantial maternal morbidity. Predicting these injuries might help to improve maternal care as well as antenatal counseling and patient education. Previous attempts to create prediction models have in many cases involved variables only known postpartum, which limits their use in an antenatal setting. Other models include parameters that are not applicable to a Northern European population. OBJECTIVE: This study aimed to develop and validate a clinically useful model for the pr

Mode of delivery and maternal outcome in subsequent delivery after an obstetric anal sphincter injury: a Finnish retrospective cohort study.

BACKGROUND: Obstetric anal sphincter injury (OASI) is a severe complication of vaginal delivery. In previous studies, parturients with a preceding OASI are at increased risk of a recurrent OASI (rOASI) in subsequent vaginal deliveries. In Finland, the rate of OASI is low compared to other countries, at 1.4% of deliveries, and the incidence of rOASI is not well-known. This study examined recurrence and mode of delivery after an OASI. METHODS: This historical cohort study includes 278 women who have experienced an OASI and have delivered again in Tampere University Hospital. Deliveries complica

Effects of Obstetric Anal Sphincter Injuries (OASIS) on Incidence of Urinary and Anal Incontinence After Childbirth in Nigeria: A Prospective Cohort Study.

INTRODUCTION AND HYPOTHESIS: Obstetric anal sphincter injuries (OASIS) are serious complications of vaginal birth. The burden is expected to be high in Nigeria due to high fertility and aversion to caesarean section (C/S); however, the incidence, effects on urinary incontinence (UI)/anal incontinence (AI), and quality of life (QoL) are largely unknown. This study determined the incidence of and risk factors for OASIS, as well as the effects of OASIS on UI/AI after childbirth, and QoL. METHODS: A prospective cohort study of post-partum women in Nigeria over a 16-month period, from January 2022

Perineal structural changes on 3-dimensional endovaginal ultrasound 6 months after difficult vaginal birth: the Hourglass-Band pattern and how it relates to levator ani avulsion and hiatus size.

BACKGROUND: Current evidence indicates that pelvic organ prolapse development after childbirth is strongly associated with an enlarged hiatus. However, few childbirth-related injuries beyond anal sphincter lacerations and levator ani avulsion-which explain less than a quarter of hiatal enlargement-have been investigated. Other types of injuries that lead to an enlarged hiatus and contribute to prolapse development remain incompletely explored. The perineal complex components (perineal membrane, perineal body, and levator ani) are responsible for hiatal closure and form a distinct anatomical an

Risk factors for obstetric anal sphincter injury: an exploratory Norwegian regional cohort study including amniotomy as a potential novel determinant.

BACKGROUND: Obstetric anal sphincter injury refers to the unintended tearing of the anal sphincter and/or rectal mucosa during vaginal delivery, a complication associated with significant morbidity. Well-known risk factors for obstetric anal sphincter injury include primiparity, high maternal age, instrumental delivery, oxytocin augmentation, and large fetal size. Recently, we identified amniotomy as a potential novel independent risk factor, regardless of parity. This study aims to further explore whether amniotomy increases the risk of obstetric anal sphincter injury including known determin

Factors associated with severe obstetric anal sphincter injury following vaginal delivery.

AIM: Obstetric anal sphincter injury (OASI) is a major complication of vaginal delivery. Tear severity can be categorized according to the structures involved. To date, little is known regarding risk factors for severe OASI tears. We aimed to evaluate the association between known risk factors for OASI and tear severity. METHODS: This was a retrospective cohort study, performed at a tertiary university hospital. The study population included women aged 18-50 years who underwent a vaginal delivery and were diagnosed with OASI. The study cohort was divided into two groups based on OASI severity

Patient-reported outcomes in the subpopulation of patients with mismatch repair-deficient/microsatellite instability-high primary advanced or recurrent endometrial cancer treated with dostarlimab plus chemotherapy compared with chemotherapy alone in the ENGOT-EN6-NSGO/GOG3031/RUBY trial.

OBJECTIVE: In the ENGOT-EN6-NSGO/GOG3031/RUBY trial, dostarlimab+carboplatin-paclitaxel demonstrated significant improvement in progression free survival and a positive trend in overall survival compared with placebo+carboplatin-paclitaxel, with manageable toxicity, in patients with primary advanced or recurrent endometrial cancer. Here we report on patient-reported outcomes in the mismatch repair-deficient/microsatellite instability-high population, a secondary endpoint in the trial. METHODS: Patients were randomized 1:1 to dostarlimab+carboplatin-paclitaxel or placebo+carboplatin-paclitaxel

Complex cesarean section: Surgical approach to reduce the risks of intraoperative complications and postpartum hemorrhage.

The incidence of cesarean section is dramatically increasing worldwide, whereas the training opportunities for obstetrician/gynecologists to manage complex cesarean section appear to be decreasing. This may be attributed to changing working hours directives and the increasing use of laparoscopy for gynecological surgical procedures, including in gynecological oncology. Various situations can create surgical difficulties during a cesarean section; however, two of the most frequent are complications from previous cesarean (myometrial defects, with or without placental intrusion and peritoneal ad

Abdominopelvic complications of gynecologic malignancy: Essentials for radiologists.

Gynecologic cancers are among the leading causes of cancer-related deaths among female patients, with over 80 % of patients experiencing persistent or long-term effects even after curative treatment. Abdominopelvic complications can arise from the disease itself or treatment-related factors. Tumor-related complications include effects from locoregional invasion (malignant bowel obstruction, obstructive uropathy), tumor rupture (and associated hemorrhage), hypercoagulability (leading to deep vein thrombosis), and infections (including tumor fistulization to the bowel or lower urinary tract, abs

[Preexisting diabetes: Expert consensus from the College of French Gynecologists and Obstetricians and from the French Society of Diabetology].

In France, 0.2% of women who gave birth in 2021 had type 1 diabetes, and 0.3% had type 2 diabetes. Regarding preconception care, it is recommended that women with any type of diabetes achieve an HbA1c level of less than 6.5%. For women using continuous glucose monitoring (CGM), the recommended target range is 0.70-1.80g/L (3.9-10mmol/L), and it is recommended to achieve this range at least 70% of the time. The preconception assessment includes: 1) an HbA1c measurement, 2) an assessment of microangiopathic impact, 3) an assessment of macroangiopathic complications, 4) screening for associated c

Comprehensive Management of Obstetric Rectal Buttonhole Tears: Insights From a Case Report and Literature Analysis.

BACKGROUND Obstetric rectal buttonhole tears (ORBT) are rare and underreported complications during vaginal births. They offen go undiagnosed, as they appear with intact perineum or with a low-degree perineal laceration. If missed, they can progress to a rectovaginal fistula. They are not a part of the Sultan classification and there is no consensus on their management. Various repair techniques have been proposed, but the limited evidence and experience make it challenging to standardize an approach. CASE REPORT We present a case of a 2-cm ORBT with intact anal sphincters revealed after a dig
