BACKGROUND: Morbidities associated with vagina deliveries are many but under reported. Where data exist, women had been shown to suffer myriad of problems following vaginal birth. These include immediate trauma to the perineum from episiotomy and perineal laceration at vaginal deliveries. These women may also have other problems such as pelvic pain, uterine descent and incontinence to urine, stool or flatus as well as sexual, social and psychological dysfunction on the long term. The objective of the study was to determine the prevalence and predisposing factors of perineal trauma amongst wome

We describe the case of a healthy, pregnant female who developed endogenous endophthalmitis at the time of delivery, and discuss the possible mechanism of infection and the management of this case. A 26-year-old Asian woman presented with a 3-week history of visual deterioration and pain in the right eye. There was no history of ocular trauma or surgery. The ocular symptoms developed one day after vaginal delivery of a healthy baby. The pregnancy had been uncomplicated until premature rupture of membranes one week prior to delivery. Right visual acuity was light perception. There was marked ri

OBJECTIVE: Statistical methods that measure the independent contribution of individual factors for third-/fourth-degree perineal laceration (TFPL) fall short when the clinician is faced with a combination of factors. Our objective was to demonstrate how a statistical technique, classification and regression trees (CART), can identify high-risk clinical clusters.
STUDY DESIGN: We performed multivariable logistic regression, and CART analysis on data from 25,150 term vaginal births.
RESULTS: Multivariable analyses found strong associations with the use of episiotomy, forceps, vacuum, nullipari

BACKGROUND: Maternal death and disability remain significant problems in developing countries and are predominantly caused by preeclampsia and postpartum haemorrhage. The diagnostic criteria for preeclampsia and postpartum haemorrhage require medical technologies not readily available in underdeveloped areas.
OBJECTIVE: To determine the correlates of pre-eclampsia and postpartum haemorrhage using symptoms in a rural setting.
METHODS: This was a cross-sectional study in which 577 women from the Kwahu South District of the Eastern Region of Ghana completed questionnaires that sought for signs

One third of women will suffer from urinary incontinence 3 to 6 months after birth, one in ten from anal incontinence and one percent from prolapse. The risk for uterine prolapse increases with future vaginal pregnancies, an indication for an elective cesarean section however must be balanced based on the multifactorial nature and the low prevalence of prolapse against the risks of a cesarean section especially of placental pathology in future pregnancies. The correlation between mode of delivery and urinary incontinence decreases with increasing number of pregnancies and with follow up after

Vulvovaginal candidiasis (VVC), particularly the recurrent form, remains an intractable problem for clinicians, microbiologists, and patients. It is essential to confirm the clinical diagnosis by mycological methods and avoid empirical therapy. The recovery of yeast in fungal culture, such as on Sabouraud dextrose agar, remains the gold standard for diagnosis. In this investigation, we examined 474 participants, including 122 (25.7%) with acute VVC cases, 249 (52.5%) who had recurrent VVC (RVVC) cases, and 103 (21.7%) healthy controls. We also administered a questionnaire to obtain information

OBJECTIVE: Determine whether dyssynergic defecation is a risk factor for third- or fourth-degree tear during a first vaginal delivery.
STUDY DESIGN: A retrospective case-control study was conducted on 549 primiparous women. The case group (n = 140) sustained an anal sphincter tear and the control group (n = 409) had a perineal laceration lower or equal to a second-degree tear. The Knowles-Eccersley-Scott Symptom questionnaire identified women who had dyssynergic defecation. A logistic regression analysis was performed.
RESULTS: Anal sphincter tear were 2.94 times higher for women reporting d

OBJECTIVE: To document an unusual case of water douche injury in a prepubescent girl.
DESIGN: Case report.
RESULTS: After sitting atop a high-pressure water jet in a public fountain, a 9-year-old girl experienced pain and vaginal bleeding. She sustained a laceration high in her vaginal vault with an estimated total blood loss of 750 mL. Examination and vaginal packing were done under general anesthesia resulting in cessation of bleeding.
CONCLUSIONS: High-pressure water douche is recognized as producing serious vaginal injury in adult women but is not well reported as a cause of genital tra

The Agency for Healthcare Research and Quality Quality Indicator tools were used to identify risk factors for maternal birth-related trauma rates in the 2003 Nationwide Inpatient Sample and the 2002-2004 Iowa State Inpatient Database. Risk-adjusted analyses of these datasets isolated salient risk factors for maternal trauma. The rates of Iowa's risk factors for the most serious types of trauma--third/fourth-degree lacerations--were compared with national rates. The comparisons suggest that episiotomy, artificial rupture of membranes, obstructed labor, and late pregnancies are the most salient
