In this article, we evaluate a possible postzone effect for the Histoplasma antigen lateral flow assay (LFA). We re-tested urine samples that were Histoplasma-positive with the LFA, both for an undiluted and diluted sample. Of the 12 samples, a postzone effect was observed in six, and in one sample this led to a false negative result in the undiluted sample. For two samples, dilution of the sample led to a weaker LFA result. We recommend cautiousness when interpreting negative samples based on LFA alone, especially when clinical suspicion of histoplasmosis is high.

BACKGROUND Fungal endocarditis constitutes up to 3% of diagnosed infective endocarditis cases, and Histoplasma spp. cause approximately 10% of fungal endocarditis cases. Histoplasma infection occurs through inhalation of spores typically found in contaminated soil. The clinical forms are variable, ranging from asymptomatic to fatal presentations. Asymptomatic individuals may develop reactivated disease years after the primary exposure. Most cases of histoplasmosis are confined to the pulmonary system. In rare instances, fungal elements disseminate to the heart valves, where destructive injury

The dose-dependent risk of opportunistic fungal infections and associated mortality from systemic glucocorticoids remains poorly defined in non-HIV, non-transplant (NHNT) populations. This study evaluated the cumulative incidence of opportunistic fungal infections and the association between glucocorticoid dose, infection risk, and 1-year mortality in NHNT adults. In this observational cohort study (NCT05707156), adults without HIV or solid organ transplants who received systemic glucocorticoids for ≥14 days between 2022 and 2024 were identified using the TriNetX research network. Histoplasma is a genus of human fungal pathogens that frequently affects immunosuppressed patients. Previous genetic surveys have largely focused on nucleotide-level variation, but much less attention has been given to more complex forms of mutation. Among these, transposable elements (TEs) represent an important class of mobile genetic elements that can alter genome size and play key roles in adaptation and speciation. In this study, we address this gap by examining the content and evolutionary dynamics of TEs in the human pathogen Histoplasma. Using previously published Histoplasma genome as

Pulmonary histoplasmosis has traditionally been considered geographically restricted to disease-endemic regions. Taiwan, historically nonendemic, has recently witnessed rising infections. We conducted a retrospective study by reviewing adult patients in Taiwan who had pathologically confirmed pulmonary histoplasmosis during June 1997-December 2024. We analyzed 14 cases with lung involvement. Eight case-patients were male and 6 female; mean age was 56.6 years. Of note, 11 case-patients (78.6%) had no history of travel to histoplasmosis-endemic regions; 10 (71.4%) were immunocompetent. Left uppe

Cardiac Implantable Electronic Device infections continue to pose a pivotal threat to the successful management of various cardiac electrical disturbances. We present a case of a 79-year-old male who had undergone a dual-chamber pacemaker implantation 10 years ago. Patient presented with a history of fluctuant swelling over the pacemaker pocket, which has been slowly increasing in size over the past 1 year. There were no other signs of infection. Patient was approaching the pacemaker generator replacement indication. The patient underwent complete enucleation of the pacemaker pocket and replac

Histoplasmosis is a fungal infection caused by the dimorphic fungus Histoplasma spp. Its clinical presentation is highly variable, ranging from asymptomatic cases to progressive disseminated infections. Disseminated histoplasmosis is the rarest form and is mostly found in immunocompromised patients, although rare cases have also been reported among immunocompetent individuals. In this study, we present a case of disseminated histoplasmosis in a middle-aged, immunocompetent patient from Sichuan Province, southwest China, diagnosed by mNGS. This condition is rarely documented in this region, and

Fibrosing mediastinitis (FM) is a rare inflammatory reaction within the mediastinum that causes diffuse fibrosis and can lead to compression of vascular structures in the thoracic and cervical regions. While typically benign and indolent, significant progression can lead to obstruction of vital organs, causing conditions like superior vena cava (SVC) syndrome, pulmonary hypertension, and potentially fatal right-sided heart failure. The clinical presentation often mimics malignancy, which necessitates exclusion. FM is a rare condition, with idiopathic cases being even rarer. While histoplasmosi

We describe an unusual case in which disseminated histoplasmosis was suspected at the time of of rapid on-site assessment (ROSE) performed during endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). The patient was a renal transplant recipient with fever, mild anemia, bilateral lung infiltrates and mediastinal lymphadenopathy who underwent EBUS-TBNA of station 11L and 7 lymph nodes. The diagnosis of disseminated histoplasmosis was suggested at the time of ROSE and confirmed several days later by microbiologic cultures. Cytomorphologic findings on routine stains can be

Central nervous system (CNS) histoplasmosis is a rare but serious manifestation of Histoplasma capsulatum infection that often mimics other intracranial pathologies. We report the case of a 22-year-old man with untreated cancer who presented with progressive neurological deficits. Neuroimaging revealed multifocal ring-enhancing lesions, raising the possibility of a broad differential diagnosis including leukemic infiltration and other CNS infections. Extensive evaluation including cultures and molecular assays were negative. Diagnosis was confirmed by a positive urine Histoplasma antigen and i

Disseminated histoplasmosis is a potentially serious complication after organ transplantation; however, its occurrence is rare, even in endemic regions. Diagnosis can be challenging, particularly in patients with nonspecific symptoms such as intermittent fever or lethargy. We report the case of a man who presented with intermittent fever 10 years after kidney transplantation. His presentation initially raised suspicion for post-transplant lymphoproliferative disorder (PTLD) but was ultimately diagnosed as disseminated histoplasmosis following mediastinal lymph node biopsy and fungal tissue cul

OBJECTIVE: A commercially available Histoplasma urine antigen enzyme immunoassay (MV antigen EIA; MiraVista Diagnostics) is widely used to support the diagnosis of histoplasmosis in dogs; however, a standardized urine collection technique does not exist. The objective was to evaluate agreement between 3 urine collection techniques (free catch [FC], cystocentesis [cysto], and iatrogenically hospital-contaminated cystocentesis [contam]) using the MV antigen EIA in dogs and to determine associated false-positive rates (FPRs).
METHODS: Participants were 30 dogs suspected of histoplasmosis and 30

BACKGROUND: Richter Transformation (RT) is the rare progression of chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) into a more aggressive lymphoma, occurring in only about 5-10% of cases. RT has a poor prognosis with rapid progression and resistance to standard therapies. PET/CT scans are crucial in detecting RT with increased SUVmax indicating more aggressive disease.
CASE PRESENTATION: A 65-year-old male with 13q deletion CLL developed three relapses over the course of 13 years from initial CLL diagnosis, and work up of the third relapse revealed disseminated histoplasmosi

BACKGROUND: Epizootic lymphangitis (EEL), caused by Histoplasma capsulatum var. farciminosum (HCF), is a neglected equine fungal disease lacking effective vaccines. The newly developed inactivated '8ZH' vaccine requires safety validation in the target species.
OBJECTIVES: To evaluate the biochemical, haematological and clinical safety of the inactivated HCF '8ZH' vaccine in foals.
METHODS: A controlled, single-blinded study was conducted on 30 clinically healthy foals (4-6 months), randomized into vaccinated (n = 15) and control (n = 15) groups. Vaccinated animals received a 5 mL intramuscul

Ascomycetes belonging to the Histoplasma capsulatum species complex can cause severe manifestations in patients with disseminated histoplasmosis. Based on data from hemagglutination assays and histopathological findings related to H. capsulatum, we investigated their potential role in the disseminated intravascular hemagglutination events. Human erythrocyte hemagglutination was produced with H. capsulatum yeasts of 10 isolates/strains, belonging to different phylogenetic species and lineages. Hemagglutination assays were inhibited by pre-incubating their yeasts with β-galactose, ratifying prev

Adaptive responses to environmental stimuli are integral to the survival and virulence of microbial pathogens. The thermally dimorphic human fungal pathogen Histoplasma senses temperature to transition between a mold form in soil and a pathogenic yeast in mammalian hosts. The contributions of chromatin-modifying enzymes to the ability of Histoplasma to appropriately respond to temperature have never been explored. Through chemical inhibition and genetics, we determined that the class I histone deacetylase (HDAC) RPD3 is required for normal Histoplasma yeast morphology at 37 °C. Rpd3 regulated

Disseminated histoplasmosis is a fungal infection usually encountered in immunocompromised hosts. Hepatic involvement is common, but portal hypertension due to extrahepatic lymph node compression has not been described. We present a 68-year-old man without cirrhosis who developed noncirrhotic portal hypertension by reactive, fibrosing porta hepatis lymphadenopathy after disseminated histoplasmosis treatment. Despite prior exposure to immunosuppressive therapy for presumed sarcoidosis, liver biopsy demonstrated preserved hepatic architecture and normal hepatic venous pressure gradient, supporti

Hierarchical composite end points may better reflect patient outcomes, rather than mortality alone. We applied DOOR and Win-Ratio analyses to a phase II trial comparing single high-dose vs standard liposomal amphotericin B for HIV-associated histoplasmosis. No clear differences were observed, supporting feasibility and informing design of future phase III trials.

Histoplasmosis, caused by Histoplasma capsulatum, is a dimorphic fungus typically endemic to parts of the Americas, Africa, and Asia. Inhalational exposure leads primarily to pulmonary infection, and extrapulmonary or isolated hepatic presentations are rare, particularly in immuno-competent individuals. We present an unusual case of hepatic histoplasmosis with no evident lung involvement, highlighting a diagnostic challenge and a rewarding pathologic discovery.
