Disseminated histoplasmosis with gastrointestinal involvement in a patient with HIV: Case report and systematic review of the literature.

Disseminated histoplasmosis is a life-threatening opportunistic infection in patients with advanced HIV. We report the case of a 43-year-old man with untreated HIV who presented with disseminated histoplasmosis and gastrointestinal involvement, including recurrent intestinal obstruction. Follow-up revealed colonic auto-amputation, a previously unreported complication. A systematic review of the literature was conducted following PRISMA guidelines. Most reported cases occurred in Southeast Asia and Latin America. Colonic and duodenal involvement were the most frequent gastrointestinal manifesta

The presence of Histoplasma capsulatum in environments from the state of Nayarit, Mexico, reveals a potential risk of infection for human populations.

The Histoplasma capsulatum complex clusters cryptic species with genetic diversities linked to their geographic distribution and virulence. The American continent harbours its highest diversity, making this region the main axis for H. capsulatum spread. Species of this complex cause respiratory-systemic mycosis known as histoplasmosis. Infection occurs through the inhalation of fungal propagules that grow in sites such as caves, mines, and abandoned buildings, where environments rich in organic matter containing micronutrients present in bat and bird guano favour the development of these funga

Unveiling Deep Mycoses: Histopathological Challenges in Diagnosing Deep Mycoses as a Neglected Tropical Disease.

Deep mycoses are rare invasive fungal infections involving subcutaneous or systemic tissues and remain underrecognized neglected tropical diseases in Indonesia. This study aims to describe clinicopathological features and highlight histopathological challenges in diagnosing cutaneous deep mycoses at a tertiary hospital in Yogyakarta. A retrospective review of skin biopsies submitted with a differential diagnosis of deep mycosis to the Anatomical Pathology Department, Dr Sardjito Hospital, between 2020 and 2024 was conducted, including cases with demonstrable fungal elements on histopathology a

Bat-borne zoonoses: transmission, impact, prevention, and management.

Climate change and biodiversity loss are global environmental crises driving emerging infectious diseases, including bat-borne pathogens of public health significance: Ebola, Marburg, Nipah, Hendra, classic rabies virus, and Australian bat lyssavirus, as well as the fungus Histoplasma This review presents the evidence on the mechanisms of transmission, impact, and prevention and management strategies relevant to each of these pathogens. Transmission pathways vary between pathogens but often involve exposure to body fluids or excreta of a bat or other infected animal or human. The diseases asso

A Case of Neurohistoplasmosis Successfully Treated With Single High-Dose Liposomal Amphotericin B.

Disseminated histoplasmosis is a leading AIDS-related opportunistic mycosis and carries high mortality risk. A simplified induction strategy using single high-dose of liposomal amphotericin B (L-AmB) has shown promise for disseminated disease, but persons with central nervous system (CNS) involvement were excluded from the clinical trial. We describe a 35-year-old man with advanced HIV disease (CD4 24 cells/µL) and a high fungal burden who was diagnosed with disseminated histoplasmosis and, subsequently, culture-confirmed CNS involvement. He was administratively withdrawn from the trial when C

Diagnostic Value of Skin Biopsy in Fatal Histoplasmosis-Associated Hemophagocytic Lymphohistiocytosis: A Case Report.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome most commonly triggered in adults by infection, malignancy, or autoimmune disease. Disseminated histoplasmosis is an uncommon but recognized infectious trigger, particularly in immunosuppressed patients. Its cutaneous manifestations may be subtle and nonspecific but can provide accessible tissue for diagnostic evaluation. CASE REPORT: A 41-year-old man with psoriasis treated with adalimumab presented with 5 months of progressive constitutional and abdominal symptoms followed by fever,

Disseminated Histoplasmosis Presenting as Mucocutaneous Lesions in an Immunocompetent Patient.

A case of an immunocompetent male patient in his 50s, an oil field worker from the northeastern part of India who presented with fever, weight loss over 4 months, and multiple umbilicated papulonodular lesions on his face and nodular lesions over the hard palate, is reported. The initial suspicion was talaromycosis, which is endemic in that part of the country. Periodic acid-Schiff and Grocott-Gomori's methenamine silver staining of the skin biopsy specimen from the forehead revealed numerous yeast-like bodies morphologically resembling Histoplasma within the histiocytes. A culture of the skin

Adrenal Infections: An Underrecognized Cause of Primary Adrenal Insufficiency.

INTRODUCTION: Adrenal infections are an underrecognized but clinically significant cause of primary adrenal insufficiency (PAI). OBJECTIVES: This review proposes a diagnostic algorithm for the evaluation of suspected cases. RESULTS: The most common causative pathogens are Mycobacterium tuberculosis and Histoplasma capsulatum; however, bacterial infections and viral pathogens such as cytomegalovirus (CMV) and SARS-CoV-2 (COVID-19) have also been reported. These infections, particularly in tuberculosis and histoplasmosis, can induce granulomatous inflammation, tissue necrosis, and destruction

Evaluation of a multiplex qPCR assay for rapid diagnosis of fungal pneumonia and tuberculosis in people with advanced HIV disease.

Fungal pneumonias and tuberculosis (TB) remain major causes of mortality among people living with advanced HIV disease (PLWHA). Their overlapping clinical manifestations delay diagnosis in developed and resource-limited settings, with greater impact where access to rapid diagnostic tools is limited. This study evaluated the performance of a multiplex quantitative PCR (qPCR) assay for simultaneous detection of Histoplasma capsulatum, Cryptococcus neoformans, Pneumocystis jirovecii, and Mycobacterium tuberculosis complex in respiratory samples from PLWHA. A cross-sectional study was conducted at

L5 Corpectomy in a 12-Year-Old Owing to Rosai-Dorfman Disease: A Case Report.

BACKGROUND: Assessment of preparedness of weaning has been recommended before extubation for mechanically ventilated patients. We aimed to understand the association of a structured assessment of weaning preparedness with successful liberation. METHODS: We retrospectively investigated patients with acute respiratory failure who experienced an extubation trial at the medical intensive care units of a medical center and compared the demographic and clinical characteristics between those patients with successful and failed extubation. A composite score to assess the preparedness of weaning, the

Gastrointestinal bleeding due to disseminated histoplasmosis in a patient under anti-TNF treatment.

A 62-year-old woman, originally from Peru, with rheumatoid arthritis under treatment with anti-tumor necrosis factor (anti-TNF) therapy, was admitted due to constitutional syndrome and suspicion of neoplasia. Computed tomography (CT) scan revealed involvement of three segments of the colon, ascites, and likely peritoneal implants. Ascitic fluid analysis showed elevated adenosine deaminase (ADA) levels and lymphocytosis. The patient presented with hematemesis and hematochezia with hemodynamic instability. Upper gastrointestinal endoscopy identified an extensive ulcer in the middle esophagus wit

A Preliminary Investigation on the Utility of Embedding Fluency-Building Strategies within the TAGteach Procedure.

UNLABELLED: TAGteach is a trademarked intervention package designed to promote skill acquisition by combining the use of an audible stimulus as immediate feedback with several other behavior analytic procedures. Although TAGteach effectively improves the accuracy of sports performance, previous researchers have noted that TAGteach does not reliably promote synchronized, coordinated (i.e., fluent) sport movements. To date, no study has evaluated the efficacy of embedding fluency-building strategies within the standard TAGteach procedure. We used a concurrent multiple baseline across skills desi

Rapid gain and loss of a chromosome drives key morphology and virulence phenotypes in the fungal pathogen Histoplasma.

Heritable phenotypic switches are fundamental to the ability of cells to respond to specific conditions. Such switches are key to the success of environmental pathogens, which encounter disparate conditions as they transition between the environment and host. We determine that the copy number of chromosome seven in the thermally dimorphic fungus Histoplasma dramatically affects the rate of transition. Though Histoplasma is haploid, a second copy of this chromosome is present in natural isolates of multiple Histoplasma species and is gained and lost at a high rate. Cells carrying two copies of

Efficacy, Effectiveness, and Safety of Treatment Regimens for Histoplasmosis and Tuberculosis Coinfection in Patients With Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome: A Systematic Review With Meta-Analysis Protocol.

BACKGROUND AND AIMS: While previous publications have briefly pointed out a few alternative therapeutic interventions for the treatment of histoplasmosis and tuberculosis coinfections in patients with Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS), there are no reviews evaluating the efficacy, effectiveness, and safety of different regimens. Thus, this protocol outlines the approach for systematically reviewing and synthesizing the existing literature regarding the efficacy, effectiveness, and safety of different regimens for histoplasmosis and tuberculosis co
