Indigo carmine extravasation to upper limb after pelvic reconstructive surgery.

BACKGROUND: The use of dyes during cystoscopy to visualize the ureters adequately is prevalent in gynecologic surgery. Observing ureteral patency after procedures such as a hysterectomy or pelvic reconstruction is important for identifying injury to the upper urinary tract. Indigo carmine is commonly used. Knowing the possible adverse effects and being aware of unusual presentations with the use of indigo carmine are important in managing and counseling patients. CASE: We present a case in which a patient developed indigo carmine extravasation to her upper limb after pelvic reconstructive sur

Diagnosis of levator avulsion injury: a comparison of three methods.

OBJECTIVE: Levator avulsion is common after vaginal delivery and is strongly associated with prolapse and prolapse recurrence. The aim of this study was to compare assessment by digital palpation and two ultrasound methods, one using rendered volumes and the other multislice imaging, for the diagnosis of levator avulsion. METHODS: We retrospectively analyzed randomly identified datasets of 266 women seen at a tertiary urogynecology unit. Each patient had undergone an interview, vaginal examination and 3D/4D translabial ultrasound examination. Analysis of the retrieved ultrasound volumes was p

A retrospective analysis of postoperative complications with or without para-aortic lymphadenectomy in endometrial cancer.

INTRODUCTION: Although para-aortic lymphadenectomy (PALX) has not been accepted as a standard treatment for patients with endometrial cancer, it is possible that systematic lymphadenectomy including PALX has therapeutic significance for patients with intermediate-/high-risk endometrial cancer. On the other hand, a consensus regarding the safety of PALX has not been reached. The aim of this study was to compare the incidence rates of postoperative complications after pelvic lymphadenectomy (PLX) with or without PALX in patients with uterine corpus cancer. METHODS: A retrospective chart review

FINHYST, a prospective study of 5279 hysterectomies: complications and their risk factors.

BACKGROUND: Hysterectomy guidelines highlight an increase in urinary tract injuries with laparoscopic hysterectomy (LH). This national survey analyses complications of LH, abdominal hysterectomy (AH) and vaginal hysterectomy (VH). METHODS: A prospective cohort undergoing hysterectomy for benign indications during 2006 was drawn from 53 hospitals in Finland; all communal hospitals participated. Detailed questionnaires covered surgical data and intra- and post-operative major and minor complications, for which risk factors were analysed by a multivariate logistic regression model adjusted for s

Does mediolateral episiotomy decrease central defects of the anterior vaginal wall?

OBJECTIVE: This retrospective study investigates the effects of mediolateral episiotomy on the pelvic floor. METHODS: Premenopausal women suffering from urinary incontinence/genital prolapse who delivered only by vaginal route were enrolled into the study. History of diabetes, morbid obesity (BMI > 40 kg/m(2)), vacuum/forceps extraction, perineal lacerations that warranted repair during labor and any pelvic surgery were the exclusion criteria. Evaluation of the patients included pelvic organ prolapse quantification scores, presence of stress incontinence, urethral hypermobility, and questionn

Delayed onset advanced pelvic organ prolapse after pelvic trauma in a nulliparous young female: case report.

Reports have shown that high-energy pelvic trauma might be associated with advanced pelvic organ prolapse in conjunction with other causes. We report the case of a 21-year-old nulliparous, premenopausal, non-overweight woman with no predisposing factors for prolapse who developed stage IV massive pelvic organ prolapse and stress urinary incontinence 4 years after a severe pelvic traumatic accident. Most likely, the pelvic trauma, as a single factor, was the causative factor for the massive procidentia. The onset on the prolapse was delayed until 4 years after the pelvic accident. The patient w

[Pregnancy, delivery and pelvic floor disorders].

Pregnancy and vaginal delivery affect pelvic floor by increased intra-abdominal pressure, direct muscle trauma, nerve injury and connective tissue damage. Even if pelvic floor dysfunction is more common after vaginal delivery compared to cesarean section, the differences are less clear after long-term follow up. This implies that other factors such as age, estrogen levels, genetic factors and overweight have also a role in the pathophysiology of pelvic floor dysfunctions. It is possible to prevent some of the birth injuries by careful obstetric management. Pelvic floor muscle training during p

Pelvic organ prolapse and relationship with skeletal integrity.

Health burden related to osteoporotic fractures in an aging female population far exceeds that imposed by other chronic disorders such as cardiovascular disease and breast cancer. Bone mineral density assessment and clinical risk factors provide independent insights into fracture risk in individuals. A finite list of clinical risk factors are identified as prognostic of fracture risk, namely among aging women, including low body mass, compromised reproductive physiology (e.g., prolonged periods of amenorrhea and early menopause), parental and personal histories of fracture, and alcohol and tob

Preventing pelvic floor dysfunction: childbearing women deserve better care.

Pregnancy and childbirth are a major cause of pelvic floor dysfunction in women. Historically, symptoms of pelvic floor dysfunction have attracted little attention from government policies, either in funding or targets, when compared with other conditions such as diabetes or cancer. With more than 30% of women in the childbearing years suffering from this debilitating and embarrassing condition, this article looks at National Institute of Health and Clinical Excellence (NICE) and other guidelines that justify spending time and money on managing this group of women. It also emphasises that pelv

[Different interventions during birth and delivery and their connection to pelvic floor damage].

Pelvic floor may be affected by birth and delivery since these are risk factors for muscles, connective tissue and nerve damage. Women with pelvic floor injuries may suffer in the long term from pelvic floor organs prolapse as well as from functional impairment such as urinary and fecal incontinence or other disturbances such as pain and sexual dysfunction. Several interventions have been conducted through birth and delivery by the midwifery team, in order to alleviate discomfort from the mother and to promote mother and infant wellness. Interventions pertaining to the mother's posture, mode o

Primary repair of advanced obstetric anal sphincter tears: should it be performed by the overlapping sphincteroplasty technique?

Advanced obstetric anal sphincter tears are often associated with a high incidence of fecal and flatus incontinence. We aimed to assess the clinical outcome of these repairs when done by the overlapping sphincteroplasty technique with reconstruction of the internal anal sphincter and perineum. Between August 2005 and December 2006, all grades 3 and 4 obstetric anal sphincter tears in our department were repaired by a reconstructive pelvic surgeon, primarily using the overlapping sphincteroplasty technique with reconstruction of the internal anal sphincter and perineum. All women were followed

Nerve-sparing radical hysterectomy for cervical carcinoma.

With the concept of the improvement of quality of life in the field of surgical oncology, recent studies have questioned the place of radical hysterectomy (RH) in the treatment of cervical carcinoma due to a high rate of long-term postoperative complications involving the pelvic autonomic nerve system. It has been demonstrated that RH frequently causes bladder dysfunction, anorectal mobility disorders, and sexual dissatisfaction in cervical cancer survivors due to surgical trauma involving the sympathetic and parasympathetic branches of the autonomous innervation of the pelvic organs. Elective cesarean delivery: the pelvic perspective.

Elective cesarean delivery, or cesarean delivery on maternal request, was the focus of a recent State-of-the-Science Conference sponsored by the National Institutes of Health. On the basis of the information from comprehensive literature review and expert speakers, the panelists determined that weak-quality evidence supported elective cesarean delivery over planned vaginal delivery for urinary incontinence, although the duration of effect is not clear. For other maternal outcomes related to pelvic floor function, including pelvic organ prolapse, fecal incontinence and other anorectal symptoms,

Current status of robotically assisted laparoscopic surgery in reproductive medicine and gynaecology.

Laparoscopic techniques have revolutionized the concept of minimally invasive surgery. Robotically assisted surgery is one of the latest innovations in this field and many operative laparoscopic procedures have been performed in urology, cardiac and general surgery. More recently, the use of robotically assisted techniques have been introduced in gynaecology, and most available studies have shown it to be a safe and effective alternative to conventional laparoscopic surgery. However, whether or not to approach the management of certain gynaecological pathologies with a laparotomy or laparoscop
