Vascular segmental exclusion for recurrent vulvar squamous cell carcinoma invading the femoral vessels.

A 55-year-old woman presented with a recurrent left inguinal necrotic mass caused by locally invasive vulvar carcinoma. Computed tomography of the abdomen pelvis showed compression of the femoral vein and superficial femoral artery. The patient underwent a left axillary to popliteal artery bypass with left superficial femoral artery embolization in preparation for possible wide resection. Subsequent resection was aborted because of extensive local invasion and rapid spread. Three-month follow-up revealed a patent graft. The patient died after 4 months from progression of oncologic disease and

Endovascular management of pelvic congestion syndrome: An expert consensus statement from the French Society of Cardiovascular Imaging (SFICV), Interventional Radiology Federation (FRI), College of French Radiology Teachers (CERF), and French Society of Women's Imaging (SIFEM).

PURPOSE: Pelvic congestion syndrome (PCS) is a major cause of chronic pelvic pain in women of reproductive age. It is often associated with pelvic venous insufficiency and venous dilatation of the ovarian and uterine veins, resulting in a variety of symptoms exacerbated by venous hypertension. Despite its prevalence, PCS lacks standardized diagnostic and management protocols, making effective treatment challenging. The purpose of this expert consensus statement was to summarize the opinions of French radiologists and gynecologists regarding the diagnosis, imaging, treatment, and management of

Age-Related Dynamics in Endometrial Vascularity: A Comprehensive Three-Dimensional Ultrasound Evaluation During Follicular and Luteal Phases.

Objective: Transvaginal ultrasonography plays a crucial role in contemporary fertility management, offering insights into uterine and endometrial blood flow. Three-dimensional ultrasonography utilizing power Doppler angiography (3D-CPA) allows precise measurement of endometrial volume and vascular parameters, such as the vascularization index (VI), blood flow index (FI), and vascularization flow index (VFI); variables that indirectly assess endometrial receptivity and integrity. Doppler technology allows for the capture of changes in the uterus induced by hormonal-related fluctuations during t

A Novel Pneumatic Balloon Saved the Lives of Three Patients With Refractory Pelvic Hemorrhage After Peripartum Hysterectomy.

This case report discusses three patients who presented in our obstetric emergency with refractory pelvic hemorrhage following cesarean hysterectomy due to placenta accreta. They were referred from district hospitals with pelvic towel packing as a lifesaving procedure after failure of pelvic angiographic embolization or internal iliac artery ligation. After stabilization, we did a relaparotomy to remove the surgical towels, which led to the recurrence of excessive pelvic hemorrhage. The new pneumatic balloon was prepared from two surgical rubber gloves inserted into each other and ligated arou

A Near Catastrophe: Antiphospholipid Syndrome and Catastrophic Antiphospholipid Syndrome Complicating Emergency Department Care.

BACKGROUND: Antiphospholipid syndrome (APS) is an autoimmune syndrome that often clinically manifests as venous thrombosis, arterial thrombosis, pregnancy morbidity, and thrombocytopenia. While APS is rare and difficult to diagnose, it can lead to devastating complications, most notably catastrophic antiphospholipid syndrome (CAPS). Both APS and CAPS have a strong association with pregnancy, which is often what reveals a new diagnosis of APS. Recurrent spontaneous abortions and pre-eclampsia are the most common pregnancy-related triggers for APS, while Hemolysis, Elevated Liver Enzymes, Low Pl

Robot-assisted hysterectomy for endometrial cancer: is there a correlation between uterine diameters, pelvic volume, and surgical outcomes?

Hysterectomy could be performed through a vaginal approach, laparotomy, or laparoscopy. Robotic surgery is a technique that can be defined as a particular type of laparoscopy, often used in interventions where microsurgery can help improve radicality and effectiveness. In choosing patients for the robotic technique, many characteristics could be studied to tailor the surgical choice. Our aim is to describe the anthropomorphic and radiological parameters linked to worse surgical outcomes for the potential stratification of patients in the preoperative decision about the type of surgery. In our

Damage Control Surgery in Obstetrics and Gynecology: Abdomino-Pelvic Packing in Multimodal Hemorrhage Management.

Damage control surgery (DCS) is a staged surgical strategy for rapid control of life-threatening bleeding, followed by physiological stabilization and delayed definitive repair. Abdomino-pelvic packing (APP)-placing compressive material within the pelvis and/or abdomen to tamponade bleeding-is a cornerstone of DCS as a temporizing measure to achieve hemostasis and stabilization in critically unstable patients. This narrative review synthesizes current evidence on DCS with a focus on APP-a technique historically developed in trauma and orthopedic surgery for exsanguinating pelvic bleeding but a

Morbidity Associated with Autologous Fascia Lata Harvesting for Pelvic Floor Surgery.

INTRODUCTION AND HYPOTHESIS: Fascia lata (FL) is an alternative to mesh for pubovaginal sling (PVS) or abdominal sacrocolpopexy/hysteropexy (ASC). Literature on perioperative complications is sparse. We aim to evaluate the morbidity associated with the FL harvest for treatment of female pelvic floor dysfunction. METHODS: Women undergoing FL harvest Feb 2021-May 2025 at a surgeon's private practice and tertiary public hospital. The same harvest technique was used, with graft size depending on the indication (mean size ASC; 13 × 4 cm; PVS 10 × 2 cm). Thigh issues were ascertained by direct ques
