Tissue concentrations of T-1982 (cefbuperazone) at 100-220 minutes after the completion of 1 g/60 minutes dripping infusion were 8.9-10.6 micrograms/g in the uterus, ovary and oviduct. These concentrations were higher than 80% MIC of the causative organisms, including E. coli, K. pneumoniae, P. mirabilis, Bacteroides and Peptococcus isolated from infections in the field of obstetrics and gynecology. Human pharmacokinetics and clinical studies of T-1982 (cefbuperazone) were carried out and the following results were obtained. Transference into pelvic cavity fluid T-1982 level in pelvic cavity fluid showed the peak level of 48.0 micrograms/ml at 30 minutes after 2 hours drip infusion of 1 g in 1 case. The concentration tended to be more sustained than that of venous blood. The use of the vacuum extractor over a 5-year period is reviewed. The instrument was used on 319 occasions, with a 2.8% failure rate and a 25 per thousand corrected perinatal mortality. The maternal morbidity attributable to the use of the instrument was low. The main indications for use were to expedite labor either in the first or second stage because of maternal or fetal distress, and in the rotation and subsequent delivery of the malpositioned vertex. It was found to be particularly useful for the platypellic pelvis with an android tendency, a type rather common in Nigerian parturients.

The maternal deaths in a supra-regional reference hospital between 1971 and 1977 are reviewed and avoidable factors discussed. Improvements in ante-natal care have diminished the number of deaths from obstetric causes and as a result deaths from associated diseases are assuming greater significance. Particular attention is drawn to the enterocolitis group of diseases which account for 15% of the total deaths in this series. The effect of pregnancy on amoebic colitis has not been widely appreciated.

A study was performed of 102 obstetric-gynecologic patients who were thought to have sepsis or a pelvic abscess. Fifty-three of these women received chloramphenicol and 49 received clindamycin. In addition, all patients received penicillin or a similar antibiotic and an aminoglycoside. Similar clinical results were observed with the two treatment regimens. Ligation of one or both internal iliac arteries is described in 5 obstetric and 5 gynaecological patients. The procedure was performed for control of severe pelvic haemorrhage. The anatomy and technique are briefly outlined, and the freedom from complications is stressed. Modern views as to the probable postoperative haemodynamics are discussed. A plea is made for more frequent use of this simple operation, and for its routine teaching during the training of pelvic surgeons.
