Prehospital Blood Transfusion for Medical Causes of Hemorrhage: A Case Series.

OBJECTIVES: Much attention has been given to prehospital blood use for trauma, but evidence is lacking for non-traumatic hemorrhage. Therefore, we sought to evaluate the epidemiology of patients receiving prehospital whole blood for nontraumatic hemorrhage. METHODS: We conducted a retrospective observational study of patients receiving prehospital blood transfusions for non-traumatic hemorrhagic shock within two fire-based emergency medical services (EMS) systems, between February 20, 2024, and April 23, 2026. Patients were included if blood products were administered for a non-traumatic caus

Spontaneous fundal rupture of an unscarred uterus in the second trimester of twin gestation, presenting with sepsis: a case report.

BACKGROUND: Spontaneous rupture of the uterus during the early weeks of gestation is rare. When it occurs, it is a life-threatening obstetric emergency. Most uterine ruptures happen in scarred uteri and during the late third trimester. This case is unique because it involves a spontaneous rupture in an unscarred uterus during the second trimester. Identified risk factors include antepartum hemorrhage in a previous pregnancy, history of retained placenta, prior manual vacuum aspiration for incomplete abortion, and use of herbal medicine. This case adds to the limited global literature indicatin

Comprehensive Management of Obstetric Rectal Buttonhole Tears: Insights From a Case Report and Literature Analysis.

BACKGROUND Obstetric rectal buttonhole tears (ORBT) are rare and underreported complications during vaginal births. They offen go undiagnosed, as they appear with intact perineum or with a low-degree perineal laceration. If missed, they can progress to a rectovaginal fistula. They are not a part of the Sultan classification and there is no consensus on their management. Various repair techniques have been proposed, but the limited evidence and experience make it challenging to standardize an approach. CASE REPORT We present a case of a 2-cm ORBT with intact anal sphincters revealed after a dig

Predictors and Unexplained Variability of Obstetric Anal Sphincter Injuries: A Population-Based Cross-Sectional Analysis.

INTRODUCTION AND HYPOTHESIS: The objective was to determine the incidence of obstetric anal sphincter injuries (OASIs) and assess the predictive power of clinical risk factors for OASIs, both individually and collectively, using a US population-based birth registry. METHODS: We conducted a cross-sectional study using birth data from the National Vital Statistics System from 2016 to 2021. All vaginal births were included, excluding those with unspecified delivery route or perineal laceration status. Logistic regression models assessed the relationship between potential clinical predictors, det

Traumatic brain injury during pregnancy is associated with increased rate of cesarean section: a nationwide multi-register study in Finland.

OBJECTIVE: The literature on the incidence of traumatic brain injury (TBI) during pregnancy is lacking. Furthermore, only studies with small sample size have analyzed the impact of TBI during pregnancy to maternal and fetal outcomes. Thus, we aim to report the incidence of TBIs during pregnancy and study the pregnancy outcomes using nationwide high-quality registers. METHODS: This nationwide retrospective register-based matched cohort study utilized two national registers. All fertile-aged (15-49 years) women with a TBI hospitalization period during pregnancy were retrieved the Care Register

Surgical approach to hysterectomy for benign gynaecological disease.

BACKGROUND: Currently, there are five major approaches to hysterectomy for benign gynaecological disease: abdominal hysterectomy (AH), vaginal hysterectomy (VH), laparoscopic hysterectomy (LH), robotic-assisted hysterectomy (RH) and vaginal natural orifice hysterectomy (V-NOTES). Within the LH category we further differentiate the laparoscopic-assisted vaginal hysterectomy (LAVH) from the total laparoscopic hysterectomy (TLH) and single-port laparoscopic hysterectomy (SP-LH). OBJECTIVES: To assess the effectiveness and safety of different surgical approaches to hysterectomy for women with ben

Psychological morbidity and chronic disease among adults with nontraumatic spinal cord injuries: a cohort study of privately insured beneficiaries.

BACKGROUND: Individuals living with a spinal cord injury (SCI) are at heightened risk for a number of chronic health conditions such as secondary comorbidities that may develop or be influenced by the injury, the presence of impairment, and/or the process of aging. However, very little is known about the development of secondary comorbidities among individuals living with nontraumatic SCIs (NTSCIs). PURPOSE: The objective of this study was to compare the prevalence of psychological morbidities and chronic diseases among adults with and without NTSCIs. DESIGN: Cross-sectional cohort from a na

Disparities in time trends of cervical cancer mortality rates in Brazil.

PURPOSE: This study aimed to correct and describe cervical cancer mortality rates and trends by regions and age-groups in Brazil. It may help planning and implementing policies for cervical cancer control. METHODS: Data from 2003 to 2012 were accessed through the centralized national mortality database. Correction of the age-specific mortality rates was done by proportional redistribution of ill-defined causes of death and deaths coded as 'uterine, part unspecified'. Annual percentage change (APC) was obtained by trend analysis (Joinpoint regression). RESULTS: In the 10-year period, cancer a

Funding flows to global surgery: an analysis of contributions from the USA.

BACKGROUND: In recent years, funds for global health have risen substantially, particularly for infectious diseases. Although conditions amenable to surgery account for 28% of the global burden of disease, the external funds directed towards global surgical delivery, capacity building, and research are currently unknown and presumed to be low. We aimed to describe external funds given to these efforts from the USA, the world's largest donor nation. METHODS: We searched the United States Agency for International Development (USAID), National Institute of Health (NIH), Foundation Center, and re

Long-term follow-up of the impacts on obstetric complications of trunk burn injuries sustained during childhood.

Limited data are available to assess the long-term effects of burns to the trunk sustained during early childhood on subsequent pregnancies. This population-based retrospective longitudinal study uses linked Western Australia hospital morbidity and midwives notification data for the period 1983-2008. During the study period, 824 girls younger than 15 years with non-erythema burns (partial thickness, full thickness, or unspecified burn depth) to the trunk were hospitalized in Western Australia. During the follow-up, 134 subjects with burns to the trunk during childhood were identified as having

A maternal and perinatal mortality in pregnancy complicated by the kyphoscoliotic form of Ehlers-Danlos syndrome.

BACKGROUND: Ehlers-Danlos syndrome is a group of inherited connective tissue diseases demonstrating autosomal-dominant, autosomal-recessive, and X-linked inheritance patterns. The diagnosis can be established by clinical, biochemical, and genetic findings. CASE: Our nulliparous patient presented with an unspecified diagnosis of Ehlers-Danlos syndrome. Laboratory testing confirmed the kyphoscoliotic type. Based on clinical and phenotypic similarities with the vascular type of Ehlers-Danlos syndrome, termination was advised. Minor trauma in the third trimester led to delivery of a stillborn fet
