Massive Puerperal Vulvovaginal Hematoma Following Vaginal Delivery: A Case Report.

Puerperal hematomas are rare but potentially life-threatening complications of vaginal delivery. They may expand rapidly, causing significant blood loss and hemodynamic instability. We report a case of a massive vulvovaginal hematoma occurring shortly after an apparently uncomplicated vaginal delivery in a 30-year-old multipara, presenting with severe perineal pain and signs of hypovolemia. Prompt surgical exploration with incision, drainage, and ligation of bleeding vessels resulted in complete recovery. This case highlights the importance of early clinical recognition and timely surgical man

A Swift Solution: Early Removal of the JADA Device in the Management of Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) continues to be a major threat to maternal health worldwide. Current management strategies include uterotonic agents, intrauterine tamponade devices, and surgical interventions. These methods have variable efficacy, carry potential complications, and contribute to significant healthcare costs. The JADA device is an emerging and promising tool for managing PPH. We report the case of a 19-year-old G2P1011 at 39w3d who experienced uterine atony following a spontaneous vaginal delivery. Despite administration of Pitocin and tranexamic acid, the patient continued to have

Acute Puerperal Uterine Inversion Following a Vaginal Delivery: A Case Report.

Puerperal uterine inversion is a rare and life-threatening obstetric emergency that occurs during the third stage of labor. We report the case of a 27-year-old primigravida who developed an acute third-degree uterine inversion immediately after a vaginal delivery, with partial placental tissue remaining in situ. The patient presented with severe postpartum hemorrhage, hemodynamic shock, and intense pelvic pain. The diagnosis was made clinically, and prompt resuscitative measures (including arrangement of O-negative blood at midnight) followed by uterine reduction via the modified Johnson metho

Successful Management of Acute Puerperal Uterine Inversion After Vaginal Delivery: A Case Report of a 43-Year-Old Multiparous Woman.

Acute puerperal uterine inversion is a rare but life-threatening obstetric emergency, with a reported incidence ranging from approximately 1 in 2,000 to 1 in 20,000 deliveries. It is characterized by inward displacement of the uterine fundus into or through the endometrial cavity following childbirth. This report highlights a case of acute uterine inversion in a 43-year-old multiparous woman who developed severe postpartum hemorrhage after vaginal delivery. Physical examination revealed a protruding mass through the cervix, consistent with complete uterine inversion. Following prompt fluid res

Pregnancy Outcomes Associated With Anemia in the First Trimester and Anemia Resolution by Late Pregnancy.

OBJECTIVE: Evidence related to anemia in early pregnancy, and its resolution or persistence by late pregnancy, is limited. We evaluated pregnancy outcomes associated with anemia in early pregnancy and resolution compared with persistence in late pregnancy. METHODS: We used the Merative™ Marketscan ® Commercial Database of nationwide insurance claims (2018-2023) and included pregnant individuals without hereditary anemias. We used hemoglobin and hematocrit to identify anemia in early pregnancy (before 14 weeks of gestation) and late pregnancy (at or after 24 weeks of gestation). Pregnancy outc

From guesstimates to game changers: Introducing objective measurement of blood loss for timely postpartum hemorrhage detection in Nigeria, Zambia, and Kenya.

Accurate, timely diagnosis of postpartum hemorrhage (PPH) is critical for effective management. However, in low-resource settings, reliance on visual estimation leads to missed diagnoses in up to 50% of cases, underscoring the need for more objective and reliable methods. Evidence suggests that the use of tools for objective measurement of blood loss, such as calibrated drapes, can improve early detection and prompt treatment of PPH. This article shares learning from the Accelerating Measurable Progress and Leveraging Investments for Postpartum Hemorrhage Impact (AMPLI-PPHI) project's experien

Diagnostic Accuracy of Transvaginal Sonography in the Detection of Retained Products of Conception: A Cross-Sectional Study Using Histopathology As the Reference Standard.

Background Retained products of conception (RPOC) constitute a clinically significant complication following spontaneous abortion, induced termination, or postpartum delivery, representing a notable contributor to maternal morbidity in resource-limited settings. Timely and accurate diagnosis is paramount to prevent life-threatening sequelae, including hemorrhage, sepsis, and intrauterine adhesive disease. This study aimed to evaluate the diagnostic accuracy of transvaginal sonography (TVS) in detecting RPOC among women of reproductive age, utilizing histopathological examination as the referen

A model to predict red blood cell transfusion during/after cesarean delivery.

BACKGROUND: Cesarean delivery is a major risk factor for postpartum hemorrhage. A priority for postpartum hemorrhage safety bundles is the assessment of hemorrhage and transfusion risk following cesarean delivery and the identification of patients at increased risk for postpartum hemorrhage requiring red blood cell transfusion. A prediction model based on antenatal and intrapartum risk factors could be used in a standardized manner among all patients when considering crossmatch or other preparatory measures prior to cesarean delivery. OBJECTIVE: To develop a multivariable prediction model for

Contemporary Approach to Postpartum Hemorrhage: Early Diagnosis and Evidence-Based Therapeutic Management.

Postpartum hemorrhage remains a major obstetric emergency and one of the most important contributors to maternal morbidity and mortality worldwide. Effective management depends on early recognition, systematic evaluation, and rapid implementation of evidence-based therapeutic strategies. A structured clinical approach facilitates identification of the underlying cause and supports timely intervention to prevent progression to severe hypovolemia and organ dysfunction. Risk assessment should begin during the antenatal period, as several maternal and obstetric conditions increase the likelihood o

Maternal Bleeding Outcomes in Pregnant Women With Immune Thrombocytopenia (ITP): A National Database Study.

OBJECTIVE: Immune thrombocytopenic purpura (ITP) is a hematological disorder commonly affecting women of reproductive age, but limited studies explore the maternal morbidity in pregnant patients with ITP. This study aimed to investigate whether pregnant women with ITP are at higher risk of bleeding complications during and after delivery. METHODS: We analyzed the National Inpatient Sample (NIS) database for pregnant patients admitted for delivery from 2016 to 2018. Patients were stratified into two cohorts: those with ITP and those without. The two cohorts were matched in a 1:1 ratio based on

Successful Pregnancy Post-Kidney Transplant: A Case Report.

Pregnancy after renal transplantation is now more feasible, with documented rates of successful live births. However, pregnancy after renal transplantation is associated with an increased risk of maternal complications, including preeclampsia, graft dysfunction, hypertension, and diabetes, as well as adverse perinatal outcomes such as preterm birth and intrauterine growth restriction. Effective care requires a multidisciplinary approach. This report discusses a case of a primigravida patient who successfully delivered at term three years after kidney transplantation, with an emphasis on the cl

Prophylactic carbetocin versus oxytocin following vaginal delivery among women with high risk for postpartum hemorrhage: A before-and-after study.

INTRODUCTION: Data with a high level of evidence is lacking on the use of prophylactic uterotonic drug following vaginal delivery to prevent postpartum hemorrhage (PPH) among women who were considered high-risk for PPH. Our main objective was to compare the effectiveness of prophylactic carbetocin versus oxytocin in preventing PPH after vaginal delivery among women at high risk. MATERIAL AND METHODS: We conducted a retrospective before-and-after single-center comparative study, including all high-risk women after vaginal births. Two consecutive 14-month periods were compared, where the prophy

Intramyometrial injection versus intravenous infusion of oxytocin for maintaining uterine contractility during elective caesarean delivery in a randomised controlled trial.

Oxytocin is commonly administered during caesarean delivery (CD) to prevent postpartum haemorrhage as an initial bolus followed by a maintenance dose. Nearly half of perinatal centres in Japan utilise intramyometrial (IMY) oxytocin. IMY oxytocin is less effective than intravenous (IV) oxytocin as the initial dose for inducing uterine contractions. However, its role as a maintenance dose remains unclear. We evaluated the efficacy of IMY oxytocin versus continuous IV infusion for maintaining uterine contractility during elective CD. Twenty-two women undergoing elective CD were randomised to rece
