Abdominal packing in postpartum hemorrhage: A forgotten life-saving technique.

BACKGROUND: Postpartum hemorrhage is a major cause of maternal morbidity and mortality. Abdominal packing is a technique used to control bleeding when other methods fail. This study aimed to evaluate the outcomes of patients who underwent abdominal packing for postpartum hemorrhage. METHODS: This retrospective study included 11 patients who underwent abdominal packing for severe obstetric hemorrhage (10 cases of postpartum hemorrhage and one case of second-trimester pregnancy termination complicated by severe hemorrhage) at Mersin University Faculty of Medicine Hospital between 2005 and 2023.

Tranexamic acid use for the prevention of postpartum hemorrhage in high-income countries: a review of recent literature.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading global cause of maternal morbidity and mortality. Tranexamic acid (TXA), an antifibrinolytic agent that has been shown to reduce hemorrhage-related mortality in trauma, is used off-label for the prevention and treatment of PPH, typically in conjunction with uterotonic agents. If TXA is effective in the prevention and management of PPH, this inexpensive medication could improve PPH care and reduce PPH-related morbidity and mortality. The majority of evidence on TXA use for PPH prevention and treatment comes from studies conducted in low- an

Low-Dose Aspirin is Not Associated with Increased Risk of Postpartum Hemorrhage.

OBJECTIVES: Aspirin (ASA) is recommended for preeclampsia prophylaxis, but it is uncertain whether higher doses, such as the 150 mg/day used in the ASPRE trial, increase the risk of postpartum hemorrhage (PPH). In Canada, the available formulation is 81 mg/day. We hypothesized that ASA 162 mg/day may increase the risk of PPH and other bleeding complications. We aimed to evaluate whether ASA 162 mg/day for preeclampsia prevention is associated with increased risk of PPH or other maternal or neonatal bleeding events. METHODS: This retrospective cohort study included patients taking ASA 162 mg/d

A Prospective Observational Study on Serum Lactate Dehydrogenase and Uric Acid Levels in Hypertensive Disorders of Pregnancy and Prediction of Fetomaternal Outcome.

Introduction Hypertensive disorders of pregnancy are a major cause of maternal and perinatal morbidity and mortality, particularly in low- and middle-income countries. Early identification of women at risk using simple biochemical markers may aid in timely intervention and improved outcomes. Serum lactate dehydrogenase (LDH) and uric acid have been proposed as indicators of disease severity in hypertensive pregnancies. The study aimed to evaluate the association of serum LDH and uric acid levels with the severity of hypertensive disorders of pregnancy and their relationship with maternal and f

Maternal and perinatal outcomes after vacuum-assisted vaginal delivery versus second-stage caesarean delivery in a generalist-run rural district hospital: A retrospective cohort study.

BACKGROUND: Vacuum-assisted vaginal deliveries (VADs) are recommended as safe alternatives to second-stage caesarean deliveries (SSCDs); however, little is known about their use and associated outcomes in South African rural district hospitals without obstetric specialist support. METHODS: A 5-year retrospective cohort study was conducted at a rural district hospital in South Africa. Women who underwent a VAD attempt or SSCD in the second stage of labour were compared with respect to baseline sociodemographic and clinical characteristics, and maternal and perinatal outcomes. Multivariable log

Coadministered Influenza- and Pertussis-Containing Vaccines in Pregnant Women.

IMPORTANCE: Safety concerns are a barrier to vaccination during pregnancy. Although same-day administration of multiple vaccines may maximize uptake in pregnant women, few studies have focused on the safety of concomitant vaccination during pregnancy. OBJECTIVE: To assess rates of pregnancy, birth, and neonatal outcomes following concomitant administration of influenza- and pertussis-containing vaccines in pregnant women compared with a matched control group. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study retrospectively analyzed linked data from perinatal and immuniza

Prevalence, risk factors, and adverse maternal and fetal outcomes of anemia during pregnancy: a retrospective study in China, 2012-2020.

BACKGROUND: Anemia is a public world health problem among pregnant females especially in developing countries. China is a representatively developing country with vast territory and significant economic and cultural differences in different regions. Currently most studies on anemia in pregnancy ignored economically backward and resource-poor cities or districts and anemia during pregnancy in China was not accurately estimated. The goal of this study was to assess the prevalence, risk factors, and adverse outcomes of anemia among pregnant females across different regions of China using a large,

Determinants of primary postpartum hemorrhage among women who gave birth in Wachemo University Nigist Eleni Mohammed Memorial Comprehensive Specialized Hospital, Central Ethiopia: a case-control study.

BACKGROUND: Primary postpartum hemorrhage is a leading cause of maternal death worldwide, particularly in developing countries such as Ethiopia. A significant proportion of maternal deaths are attributed to postpartum hemorrhage. Despite its public health importance, there is a paucity of evidence regarding the determinants of primary postpartum hemorrhage in our country. OBJECTIVE: This study aimed to identify the determinants of primary postpartum hemorrhage among mothers who gave birth at Wachemo University Nigist Eleni Mohammed Memorial Comprehensive Specialized Hospital, Central Ethiopia

Impact of Maternal Risk Stratification on Clinical Characteristics and Perinatal Outcomes in Singleton Pregnancies With Preeclampsia.

This retrospective study of 4143 women at Fujian Maternal and Child Health Hospital (2012-2022) investigated how maternal risk levels influence clinical characteristics and perinatal outcomes in singleton pregnancies complicated by preeclampsia. Utilizing class-imbalance-adjusted and random forest algorithms, participants were stratified into high-risk (17.38%), moderate-risk (34.08%), and low-risk (48.54%) groups. The analysis revealed that women in the high-risk category were diagnosed at a significantly earlier gestational age and faced higher rates of severe, early-onset, and preterm preec

How to avoid placental transection during low transverse cesarean delivery for anterior placenta previa.

Placenta previa, particularly when anterior, is a major risk factor for postpartum hemorrhage and may necessitate transplacental fetal extraction during cesarean delivery, which increases maternal and fetal blood loss. Standard strategies to minimize bleeding, such as fundal uterine incisions (when placenta accreta spectrum is suspected) or intraoperative ultrasonographic guidance, may be technically demanding, time-consuming, or unavailable in emergencies or resource-limited settings. Controlled separation and mobilization of a placental edge to avoid placental laceration offers a simple, rep

Recognition of intrapartum uterine rupture, anesthetic management, and maternal-fetal outcomes: strategies for rapid response.

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes. RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T

Postpartum Complications Modified TBL.

AUDIENCE: The intended audience for this modified TBL are resident physicians of all years, PGY1-4. INTRODUCTION: Biological females represent 50% of the population we encounter in the emergency department (ED), and following delivery, the ED is often the main resource for postpartum patients to seek urgent/emergent care. The US is one of the most dangerous developed nations for pregnant women, with about 1/3 of pregnancy related deaths occurring in the postpartum period.1,2 It is presumed that many of these deaths are related to lack of support or insurance for close follow up after delivery

Optimising gravimetric method interpretation using population-specific blood densities among Indonesian pregnant women: A cross-sectional study.

BACKGROUND: The diagnosis of postpartum haemorrhage (PPH) depends heavily on accurate and timely blood loss assessment, yet no gold standard assessment method is available. Gravimetric methods are recommended for low-resource settings, but they require an accurate blood density value for interpretation. Currently, there are no data on the blood density of pregnant or labour women. OBJECTIVE: This study measured the blood density of Indonesian pregnant women to improve the reliability of the gravimetric results in estimating postpartum blood loss. METHODS: This cross-sectional study was condu

Why the Gap Persists: A Systematic Review of Sociocultural, Clinical, and Systemic Factors Driving Maternal Health Disparities Between Black and White Women in the United Kingdom.

Despite overall low maternal mortality rates in the United Kingdom, Black women are approximately four times more likely to die during pregnancy, childbirth, or the postnatal period than White women. This disparity has persisted for over a decade despite national initiatives. This systematic review aimed to synthesize evidence from 2010-2025 on sociocultural, clinical, and systemic factors contributing to maternal health disparities between Black and White women in the United Kingdom (UK). Rectus Sheath Hematoma Following Cesarean Section: A Concealed Extrauterine Cause of Postpartum Hemorrhage.

Rectus sheath hematoma (RSH) is a rare but potentially life-threatening extrauterine cause of postpartum hemorrhage, particularly following cesarean delivery in high-risk patients, like those with severe preeclampsia. It usually results from injury to the epigastric vessels or rectus muscle and may be aggravated by preeclampsia-related endothelial dysfunction. Because its presentation can mimic common postoperative or postpartum complications, diagnosis is often delayed. Severe cases that go unnoticed or are missed result in concealed bleeding, shock, organ failure, or death. A 30-year-old pri

Effect of Assisted Reproductive Technologies in Women With Cardiovascular Disease.

BACKGROUND: Assisted reproductive technologies (ART) include medical interventions used to address infertility. As access to ART has expanded globally, more women with underlying cardiovascular disease (CVD) are pursuing these treatments. However, data regarding the safety profile and pregnancy outcomes of ART in women with CVD remain limited. In this study we evaluated whether ART is associated with adverse maternal cardiac, obstetric, and fetal and neonatal outcomes among women with CVD. METHODS: This was a retrospective case-control study of consecutive pregnancies achieved through ART, re

Challenging the paradigm that neonatal uterine bleeding represents the first menstrual episode.

Neonatal uterine bleeding (NUB) has been hypothesized as a relatively common early-life event potentially linked to the later development of endometriosis through retrograde menstruation. However, its true prevalence and biological nature remain uncertain. This study aimed to determine the prevalence, biological features, and maternal-neonatal correlates of reddish diaper staining in newborns, and to evaluate its purported association with endometriosis. A prospective cross-sectional study was conducted between September 2023 and March 2024 at a tertiary obstetric center with postpartum outpat
